研究目的
To present the initial clinical experience with laparoscopic partial adrenalectomy using indocyanine green dye with near-infrared fluorescence imaging.
研究成果
Laparoscopic partial adrenalectomy using intraoperative indocyanine green dye with near-infrared fluorescence imaging is safe and feasible. It may help in resecting lesions with more precise surgical margins by identifying the vascular structure during the procedure.
研究不足
The study is limited by its small sample size and the initial experience with the technique, which may not fully represent its efficacy and safety in a broader population.
1:Experimental Design and Method Selection:
The study involved eight patients undergoing transperitoneal laparoscopic partial adrenalectomy using indocyanine green dye with near-infrared fluorescence imaging. The methodology included the administration of 5 mg intravenous indocyanine green dye to guide the resection of the mass under near-infrared fluorescence imaging and white light visualization.
2:Sample Selection and Data Sources:
Patients were selected based on their adrenal pathologies, including functional adrenal mass due to Cushing’s syndrome, aldosterone-producing adenoma, and pheochromocytoma, as well as non-functional adrenal mass presenting with flank pain.
3:List of Experimental Equipment and Materials:
The NIRF system (Opal 1 TechnologyTM, Karl Storz, Tuttlingen, Germany) was used for imaging. Surgical devices for coagulation and sealing included LigaSureTM (Covidien, Mansfield, MA, USA) or ThunderbeatTM (Olympus Medical Systems Corp, Tokyo, Japan).
4:Experimental Procedures and Operational Workflow:
The procedure involved standard four-trocar access for the right side and three-trocar access for the left side, with patients in a 45° semi-lateral position. The main adrenal vein was preserved, and the tumor was resected with a 5 mm surgical device.
5:Data Analysis Methods:
Postoperative follow-up included checking blood pressure, serum cortisol and adrenocorticotropic hormone levels, plasma aldosterone concentration, renin activity, serum potassium level, and the use of antihypertensive agents.
