To review the application of femtosecond laser (FSL) in various anterior segment surgeries, including cornea, refractive, and cataract surgeries, and discuss the efficacy and safety of FSL based on study results.

FSL is capable of precise, accurate, and predictable tissue cutting with minimal collateral tissue damage and can create customized incisions with various shapes. It is a useful tool for improving the efficacy and safety of keratoplasty, refractive surgery, and cataract surgery. Experimental studies suggest novel applications of FSL, such as IOL power adjustment and presbyopic correction. Further development and validation through clinical trials are needed.

FSL increases operating time and costs in keratoplasty. Significant corneal opacity may hinder FSL use in cataract surgery. Poor dilation and posterior subcapsular cataracts are relative contraindications for FSL capsulotomy. The cost-effectiveness of FSL in cataract surgery is questioned due to equipment and maintenance costs.
