To determine the potential of tozuleristide to detect breast carcinoma in fresh pathology specimens and the feasibility of fluorescence-guided intraoperative pathology assessment of surgical margins.

Tumor targeting with tozuleristide allowed visual real-time distinction between pathologically confirmed breast carcinoma and normal tissue. Fluorescence could be used to identify the relationship of carcinoma to margins intraoperatively.

The cohort in the current study is small and although the results are very encouraging, the findings may not be statistically generalizable to all breast carcinoma types. Additional testing using a larger patient cohort with variation in histologic and molecular phenotypes will be required to fully characterize and confirm this promising breast carcinoma detection strategy.
1:Experimental Design and Method Selection:
Twenty-three patients received an intravenous bolus dose of 6 or 12 mg of tozuleristide at least 1 hour before surgery. Fifteen lumpectomy and 12 mastectomy specimens were evaluated for fluorescence by the site’s clinical pathology staff using the SIRIS, an investigational near-infrared imaging device.
2:Sample Selection and Data Sources:
Patients with biopsy-confirmed invasive ductal (IDC), invasive lobular (ILC), in situ ductal (DCIS), and in situ lobular (LCIS) carcinomas undergoing mastectomy or lumpectomy as standard-of-care treatment.

Synchronized Infrared Imaging System (SIRIS), an investigational near-infrared imaging device.
4:Experimental Procedures and Operational Workflow:
Specimens were placed under the SIRIS imaging head at a fixed focal distance of 35 to 40 cm and a black fabric covering was used to enclose the imaging head and platform to minimize interference from ambient light. Near-infrared (NIR), visible, and combined (NIR + visible) images were acquired.
5:Data Analysis Methods:
Images from sectioned tissue were analyzed for fluorescence both within grossly confirmed areas of tumor and within grossly benign breast tissue. The relative fluorescence intensity in tumor and surrounding benign tissue was assessed after images were exported into ImageJ image analysis software.
