研究目的
To identify variables with a potential impact on the SVG plaque type as assessed by optical coherence tomography (OCT) imaging in non-significant lesions.
研究成果
Clinical and laboratory findings impact plaque composition in non-significant SVG lesions, with associations to risk factors like smoking, impaired renal function, age, HDL levels, and platelet count, indicating that atherosclerosis in SVGs is not limited to significant stenoses and may influence prognosis.
研究不足
The study did not correlate OCT findings with clinical endpoints, had a relatively small sample size from a single center, and OCT is invasive with potential for iatrogenic vessel damage.
1:Experimental Design and Method Selection:
The study used a cross-sectional design with optical coherence tomography (OCT) imaging to assess plaque morphology in saphenous vein grafts (SVGs). Statistical analysis involved uni- and multivariable logistic regression to identify associations between clinical/laboratory variables and plaque types.
2:Sample Selection and Data Sources:
Twenty-nine patients with 43 non-significant SVG lesions were enrolled from the OCTOPUS registry. Inclusion criteria included prior CABG with SVG, acute coronary syndrome or evidence of ischemia, and exclusion criteria included lack of consent, STEMI, age under 18, severe valvular insufficiency, contrast allergy, or unsafe lesion location.
3:List of Experimental Equipment and Materials:
St Jude Ilumien Optis Medical system for OCT imaging, OCT Dragonfly catheter, guiding catheter,
4:014' guidewire, 6 Fr radial or femoral approach, contrast flush, heparin for anticoagulation, CAAS Intravascular 0 software for image analysis. Experimental Procedures and Operational Workflow:
OCT catheter advanced into SVG, positioned 5 mm distal to lesion, automatic pullback with contrast injection, images acquired and analyzed by core laboratory for plaque characteristics (e.g., lipid-rich, calcified), with measurements of lumen dimensions and plaque features.
5:Data Analysis Methods:
Categorical variables analyzed with chi-square test, continuous variables with t-test or Mann-Whitney U test, logistic regression for odds ratios, using Statistica 10 software.
