ICG-Var3 pHLIP NIRF imaging demonstrated 100% sensitivity and specificity for detecting UTUC, superior to white light examination (78.9% sensitivity). It improved diagnosis by 21.2%, identifying lesions missed by white light. Normal and benign tissues showed no uptake. pHLIP-based imaging is promising for improving UTUC evaluation and management, but further studies are required for validation and clinical application.

1:Experimental Design and Method Selection:
The study used an ex-vivo design to evaluate the feasibility of ICG-Var3 pHLIP for imaging UTUC. The method involved irrigating specimens with the imaging agent and comparing NIRF imaging with white light examination and histopathology.
2:Sample Selection and Data Sources:
Twelve ex-vivo upper urinary tract specimens from patients undergoing surgical treatment for upper urinary tract malignancy (11 UTUC, 1 RCC as negative control) were prospectively enrolled.

4:Experimental Procedures and Operational Workflow:
Specimens were washed with saline, irrigated with ICG-Var3 pHLIP for 15 minutes, washed again, incised longitudinally, and imaged under white light and NIRF. Areas were marked and submitted for histopathology.
5:Data Analysis Methods:
Statistical analysis using Fisher's Exact Chi square test with IBM SPSS Statistics Version 20 to compare sensitivity and specificity of imaging modalities.
