Prospective Study of Q Switched Alexandrite Laser 755a?ˉnm and V Beam Pulsed Dye Laser 595a?ˉnm in Dermatofibroma
摘要: Pulsed dye and carbon dioxide lasers have been applied in dermato?broma with clinical improvement. We treated 23 dermato?bromas two times at a 4-week interval with Q-Switched alexandrite laser 755 nm (7.5 J/cm2, 3 mm, 50 ms). V Beam pulsed dye laser with a wavelength of 595 nm was used for the residual erythema (10-11 J/cm2, 7 mm, 1.5 ms). A partial attenuation of brown colour was observed in 9 patients and complete disappearance of brown colour in 14 patients. Patient satisfaction was very high. Fifteen patients felt a decrease in hardening of dermato?broma after treatment. A pigment network in dermoscopy was observed in all patients before treatment and no one after treatment. A combined treatment using both V Beam pulsed dye laser and Q-Switched alexandrite laser may be a therapeutic option to reduce the aesthetic effect of dermato?broma with a high patient satisfaction and good cosmetic outcomes.

Alexandrite laser induced uveitis & pigment dispersion: A case report and review of the literature
摘要: To describe a complication of an upper eyelid alexandrite laser procedure. Observations: A 55-year-old woman presented with left eye blurred vision and photophobia after a left upper eyelid procedure with an alexandrite laser. She had elevated intraocular pressure (IOP), anterior chamber cell and pigment, posterior synechiae, and retroillumination defects in the left eye. She was treated with topical prednisolone and brimonidine. Six months later, although her anterior chamber had cleared and IOP had normalized, the patient reported decreased vision-related quality of life from persistent photophobia. Conclusions and Importance: Alexandrite lasers are commonly used for hair removal and skin depigmentation. When used periocularly without proper eye protection, they have the potential to create irreversible ocular complications. This case demonstrates the importance of proper eye protection with periocular laser procedures.

Insight into the mechanisms of type III minocycline-induced pigmentation removal: A case of repeated immediate pigment clearing with the Q-switched 755-nm alexandrite laser over a 13-year period
摘要: A 68-year-old man with rheumatoid arthritis presented for laser treatment of facial pigmentation. He had taken oral minocycline 100 mg 3 times weekly for 8 years (cumulative dose, 124.8 g) because of severe arthritic discomfort. Physical examination showed patchy slate blue/grey hyperpigmentation of the face, gums, teeth, and nail beds, consistent with type III minocycline-induced pigmentation (MP). Two different laser modalities were tested before treatment: Q-switched 755 nm (Qs755nm) alexandrite and Q-switched 1064 nm (Qs1064nm) neodymium:YAG. Qs755nm at 2.0 Hz/4-mm spot/3.4 J/cm2 showed the most improvement, with immediate pigment clearing, and was used for full facial treatment (3450 pulses). Pigment resolution was maintained at 13 months despite continued minocycline use, and the patient was highly satisfied. The patient continued taking minocycline and re-presented 3 times over the next 13 years with facial hyperpigmentation. Each time, immediate pigment clearing was achieved with the Qs755nm alexandrite laser. Histopathology showed intracellular and extracellular type III MP in the dermis, with notably no detectable change in the posttreatment biopsy. However, the extracellular pigment in the posttreatment sample was no longer highlighted by Fontana-Masson (FM) stain, but intracellular pigment continued to stain with FM. The change in melanin staining is consistent with the clinically observed reduction in pigment intensity.

The effects of 755 nm alexandrite laser on skin dryness and pruritus
摘要: Introduction: The alexandrite laser (AL) is a very safe and effective treatment used for unwanted hair removal with a reported success rate of 40% to 80% at 6 months and after several treatment sessions. Although a diffuse variety of side effects has been observed during laser treatment, changes in skin dryness and pruritus before and after AL epilation have not been reported yet to the best of our knowledge. Aim: To investigate the effects of 755 nm alexandrite laser on skin dryness and pruritus at the beginning and in the third and the sixth month after the treatment. Material and methods: Forty three patients with Fitzpatrick skin types of II–IV aged 18–45 with leg hair were included in this prospective study. Patients were treated with 755 nm alexandrite laser with 10–12 mm spot size. According to the skin phototype, the settings of the laser were as follows: 12–22 J/cm2 and pulse width of 3 ms. For self-assessment by the patient, the visual analogue scale (VAS) was used before, at the third and sixth month of the treatment as to skin dryness and pruritus. The patients were evaluated by the same dermatologist with the same VAS. The values were compared between before-at the third month, before-at the sixth month and at the third and at the sixth month of the treatment. Results: Pruritus scores were statistically lower at the third month when compared with the baseline scores (p < 0.01). However, there was no difference between the third and sixth month of the treatment as to pruritus scores (p > 0.05). There was a statistically significant difference between the scores before the treatment and the scores at the third month and at the sixth month as to skin dryness (p < 0.001). However, the difference was not prominent between the third and sixth month scores of skin dryness (p > 0.05). Conclusions: To the best of our knowledge, this is the first study researching the effects of AL on pruritus and skin dryness. Further studies with larger samples and longer follow-up periods will be able to better clarify the association.

Use of a Picosecond Alexandrite Laser for Treating Acquired Bilateral Nevus of Ota-Like Macules in Chinese Patients
摘要: Background and Objectives: Acquired bilateral nevus of Ota‐like macules (ABNOM) is common among the Chinese population. There have been few studies on the use of a picosecond alexandrite laser for the treatment of ABNOMs. The goal of this study was to evaluate the efficacy and safety of a 755 nm picosecond alexandrite laser for the treatment of ABNOMs. Study Design/Materials and Methods: A total of 225 subjects diagnosed with ABNOMs were included in this retrospective study. A 755 nm picosecond alexandrite laser system was used to treat all subjects. The safety and effectiveness of the laser were determined using a clinical photo assessment. The correlating factors of the patients, including the age, Fitzpatrick skin type, lesion location, lesion color, treatment procedures, melasma, and outcomes, were analyzed. Results: The patients underwent one to four treatment sessions with clinical clearance (>90%) rates of 8.89%, 30.99%, 56.65%, and 60.00%, respectively. A higher number of sessions was positively associated with a better treatment response. The hyperpigmentation incidence rate was 22.67%, and one case of hypopigmentation was observed. Conclusion: Our results suggest that a picosecond alexandrite laser is an effective and safe approach for treating ABNOMs in Chinese patients.

Choroidal neovascularization after an alexandrite laser burn
摘要: Lasers used for hair removal operate between 700 and 1000 nm to be absorbed by hair follicle melanin and destroyed by thermal injury. Due to the high concentration of melanin in the retinal pigment epithelium, the retina is particularly susceptible to laser-related accidents. The use of alexandrite laser for esthetic purposes has been growing in recent years as well as its use by unqualified personnel, which can result in eye-level accidents. We describe an alexandrite laser accident suffered by a beautician that resulted in choroidal neovascularization.

摘要: Des études préliminaires ont démontré l’efficacité du laser épilatoire, dans la prise en charge de l’hidradénite suppurée (HS) légère à modérée. Il diminuerait le nombre de follicules pilosébacés (FS) et préviendrait les microtraumatismes itératifs induits par les autres modes d’épilation. L’objectif de cette étude est d’évaluer l’efficacité et la tolérance du laser épilatoire Alexandrite dans l’HS.

Comparison of a picosecond alexandrite laser versus a Q-switched alexandrite laser for the treatment of nevus of Ota: A randomized, split-lesion, controlled trial
摘要: Background: Novel picosecond lasers have been available for various pigmentary disorders. However, there are limited data directly comparing picosecond lasers and Q-switched lasers for treatment of nevus of Ota. Objective: To compare the efficacy and safety of a picosecond alexandrite laser (PSAL) compared with a Q-switched alexandrite laser (QSAL) for the treatment of nevus of Ota. Methods: Each lesion of the 56 participants enrolled was split into two parts and randomly assigned to either PASL or QSAL treatment arm. Each lesion was treated up to 6 sessions at a 12-week interval. The efficacy and safety were determined based on blinded visual evaluation and self-report at each follow-up visit. Results: PSAL arm achieved a significantly better clearance (4.53 vs 4.0) with fewer sessions (5.26 vs 5.87) and less severe pain (5.61 vs 6.40). Patients were more satisfied with PSAL than QSAL (4.5 vs 4.0). Occurrences of post inflammatory hyperpigmentation (26% vs 34%) and hypopigmentation (21% vs 47%) were also lower in PSAL than QSAL arm. Limitations: Lack of objective assessments and outcome measures. Conclusions: PSAL demonstrated better clinical results and fewer adverse events than QSAL for the treatment of nevus of Ota.

Evaluation of the Safety and Efficacy of a Picosecond Alexandrite Laser With DLA for Acne Scars in Chinese Patients
摘要: Background and Objectives: Recently, picosecond laser treatment has been used as an effective treatment for acne scars. To evaluate the efficacy and safety of a picosecond alexandrite laser with a diffractive lens array in the treatment of acne scars in Chinese patients. Study Design/Materials and Methods: Patients with facial acne scars were treated with a picosecond alexandrite laser in three sessions at 4‐ to 6‐week intervals and followed up for 2 months. Primary outcomes were measured by physicians’ blinded evaluation of the acne scar using the ECCA (échelle d’éva physicians’luation clinique des cicatrices d’acné) grading scale. The secondary outcomes included the investigator global assessment (IGA) on the improvement of post‐inflammatory erythema (PIE), patients’ assessment of improvement on a 4‐point scale and of satisfaction on a 5‐point scale. Pain scores and adverse effects were also evaluated. Result: Twenty patients with Fitzpatrick skin types III and IV were enrolled in the study and completed all treatment and follow‐up visits. The mean ECCA scores fell from 197.75 ± 35.26 to 142.00 ± 35.92 (a 28% improvement), and the change was significant (P = 0.000). The mean IGA score of PIE improvement was 3.03 ± 0.75 (0 = no improvement and 4 = 76–100% improvement). On the basis of the patients’ self‐assessment, the average improvement scores were 2.30 ± 0.98 (0 indicating 0–25% improvement and 3 indicating >75% improvement). In total, 50% and 30% of the patients were “satisfied” and “very satisfied,” respectively, with the treatment. The mean pain score was 3.20 ± 0.50 (0 = no pain, 10 = maximum pain) with topical anesthesia. The adverse effects included transient and mild erythema, edema, and scabbing. Conclusions: Treatment with a picosecond alexandrite laser with a diffractive lens array is effective and safe for acne scars in Chinese patients. Lasers Surg. Med. ? 2019 Wiley Periodicals, Inc.

摘要: Red-diode-pumped Alexandrite lasers has proven to be an efficient method of generating light in the near infrared with broad tunability from 714 to 818 nm recently demonstrated [1]. Conventionally, tunable Alexandrite lasers have used wavelength selective elements such as birefringent filters or external gratings [1, 2]. In this work wavelength tuning is obtained by exploiting the birefringence of the crystal to use the crystal itself as the wavelength selective element, greatly simplifying the cavity and reducing innecessary loss components.
