  • Corneal topography and angle parameters after laser iridotomy combined with iridoplasty assessed by dual Scheimpflug analyzer

    摘要: Purpose To investigate the changes in corneal topography including parameters such as corneal curvature and corneal aberrations, along with anterior chamber angle (ACA) after laser iridotomy (LI) combined with peripheral iridoplasty (PI) using dual Scheimpflug analyzer. Methods In this prospective observational study, dual Scheimpflug analyzer images were acquired before and 1 week after LI plus PI. Corneal curvature of both axial and instantaneous maps from anterior and posterior surface, respectively, and total corneal power (TCP) were acquired. These corneal parameters from three zones (central, middle, and peripheral) and total corneal wavefront aberration, trefoil, and coma were obtained. The ACA from four quadrants, anterior chamber depth (ACD), anterior chamber volume (ACV), and intraocular pressure (IOP) were also inspected. Results ACD increased from 2.15 ± 0.25 to 2.18 ± 0.24 mm (P = 0.002). ACV and ACA from all four quadrants increased significantly after the laser treatment (all P < 0.05). IOP decreased from 16.9 ± 3.1 to 14.7 ± 2.9 mmHg following LI plus PI (P = 0.000). No significant changes were detected in corneal axial and instantaneous curvature from three zones on the anterior and posterior corneal surface after LI plus PI (all P > 0.05). The TCP, total corneal wavefront aberration, trefoil, and coma also revealed no significant changes after the laser procedure (all P > 0.05). Conclusions Treatment with LI combined with PI did not affect the corneal topographic parameters from both anterior and posterior surfaces. However, LI plus PI significantly and effectively improved ACA parameters.

  • Intra-operative ASOCT determined changes in angle recess in plateau iris syndrome post phaco alone and post phaco-endocycloplasty

    摘要: Cataract surgery alone is known to be beneficial in angle closure glaucoma with more than modest intraocular pressure (IOP) reduction and widening of the anterior chamber angle (ACA). However, this may not be true in all mechanisms of angle closure and persistence of plateau is known to occur in plateau iris syndrome (PIS). Of the currently available surgical modalities, only the combination of endocycloplasty with phaco (phaco-ECPL) is capable of delivering an anatomic change in the angle in PIS. We propose to demonstrate change in angle recess via intra-operative anterior segment optical coherence tomography (iASOCT) during phaco-endocycloplasty, post phaco alone and subsequently post phaco-ECPL. Until recently, iASOCT was synonymous with lamellar keratoplasty and iOCT with retinal surgery, but its use in the anterior segment is constantly evolving.

  • Comparison of Anterior Segment Measurements between Scheimpflug Camera and New Module of Optical Coherence Tomography

    摘要: Purpose: We compared the anterior segment measurements using a Scheimpflug camera (Pentacam? [Oculus Inc., Wetzlar, Germany]) and a new anterior segment module of optical coherence tomography (Cirrus HD-OCT 5000? [Carl Zeiss Meditec, Dublin, CA, USA]). Methods: Anterior segment measurements were evaluated in 47 eyes of 26 patients who visited for the purpose of cataract surgery. Measurements of the central corneal thickness (CCT), anterior chamber depth (ACD), anterior chamber angle (ACA; nasal and temporal side), and angle to angle distance (ATA) were compared between the Pentacam? and the new anterior segment module of the Cirrus HD-OCT 5000?. Results: The mean CCTs measured by the Pentacam? and the new anterior segment module of the Cirrus HD-OCT 5000? were 561.68 ± 31.35 μm and 559.04 ± 36.43 μm, respectively. There was no statistically significant difference (p = 0.387) and high correlations in the results using the two methods (r = 0.824, p < 0.001). The mean ACD measurements using the Pentacam? and Cirrus HD-OCT 5000? were 2.70 ± 0.44 mm and 2.68 ± 0.42 mm, respectively. The measurements were strongly correlated (r = 0.981, p < 0.001) without a statistically significant difference (p = 0.091). The measurements of ACA (nasal and temporal side) and ATA did not differ significantly different between the two methods (p = 0.109, p = 0.153, p = 0.322, respectively). High correlations were observed between the two methods (r = 0.944, r = 0.951, r = 0.955, respectively; p < 0.001 in all groups). Conclusions: There was no significant difference between the CCT, ACD, ACA, and ATA measured by the Pentacam? and the new anterior segment module of the Cirrus HD-OCT 5000? in adult patients. However, there was a low degree of agreement for the CCT. The measurements of ACD, ACA, and ATA can therefore be considered interchangeable when used clinically.
