Comparing Patient-Reported Outcomes of Laser In Situ Keratomileusis and Small-Incision Lenticule Extraction
摘要: Laser in situ keratomileusis (LASIK) and small-incision lenticule extraction (SMILE) are popular refractive surgeries. The objective refractive outcomes of LASIK and SMILE have been studied extensively; both procedures have comparable safety, efficacy, and predictability. However, owing to various psychosocial factors, refractive patients may report dissatisfaction despite good postoperative vision. Hence the importance of studies on subjective patient-reported outcomes. This review discusses the role of psychometric-technique-based validated questionnaires when evaluating subjective outcomes. It also summarizes the literature on patient-reported outcomes for LASIK and SMILE.

<p>Intraocular Pressure Calculation in Myopic Patients After Laser-Assisted In Situ Keratomileusis</p>
摘要: Background: Corneal refractive surgery is widely used worldwide. Myopia is the most common reason for laser-assisted corneal refractive surgery (LASIK) and one of the risk factors for glaucoma. Intraocular pressure (IOP) measurement becomes variable postoperatively as the results are affected by the decrease in corneal thickness and biomechanics. This prospective clinical case study attempted to establish a simple correction formula for the calculation of IOP in post-LASIK myopic patients. Methods: This study included 300 eyes of 150 patients with myopia and myopic astigmatism as a refractive error who underwent LASIK. IOP was measured preoperatively and 6 months postoperatively. Preoperative and postoperative corneal thickness as well as ablation depth were measured. Statistical analysis was performed to detect the relationship between ablation depth and change in IOP. An attempt was made to construct a correction formula for the calculation of post-LASIK IOP. Results: The age of the patients ranged between 18 and 50 (mean ± SD 34.78±8.8) years. The spherical equivalent of refractive error ranged between ?1.5 and ?10 diopters. The mean IOP decreased signi?cantly from 15.72±2.37 mmHg preoperatively to 11.71±2.24 mmHg postoperatively, with a mean difference of 4±1.75 mmHg (p?0.001). A positive correlation was detected between corneal thickness and IOP difference among patients both preoperatively and postoperatively (p?0.001). A positive correlation was identi?ed between ablation depth and IOP change (p?0.001). The correction formula for IOP was established: Real IOP=4+0.7(preoperative IOP)?0.3(ablation depth). Conclusion: IOP measurements change after corneal refractive surgery with LASIK. A corrected formula may be a good option for the proper calculation of post-LASIK IOP.

Laser Vision Correction for Regular Myopia and Supracor Presbyopia: A Comparison Study
摘要: A study to compare femto-presbyLASIK to standard myopia femto-LASIK refractive surgical correction with a total of 45 candidates was performed. The goal was to identify a more speci?c set of indications for presbyopia LASIK treatments. The results showed thoroughly good uncorrected visual acuity for myopia (decimal: 1.01 ± 0.15) as well as for presbyLASIK (decimal: 0.78 ± 0.17) corrections. Astigmatism was comparable in both groups and did not change signi?cantly from preoperative (0.98D ± 0.53 SD) to postoperative (1.01D ± 0.50 SD). Our study results suggest, that presbyLASIK treatment is as safe and e?ective as regular LASIK myopia correction and can hence be recommended to treat presbyopia.

Ocular Aberrations and Corneal Thickness-Intraocular Pressure Relationship One Year after Laser in situ Keratomileusis (LASIK) Enhancement with An Aspheric Ablation Profile
摘要: Objective: To evaluate the effect of an aspheric LASIK refractive enhancement performed by relifting the original flap on the higher order aberrations (HOA) of the eye and the relationship between central corneal thickness (CCT) and intra-ocular pressure (IOP). Methods: Consecutive case series of 30 eyes requiring refractive enhancement from 1804 uncomplicated primary LASIK procedures performed in 2012. Flap relift and wavefront optimized ablation (Wavelight Allegretto Eye Q 400 Hz excimer laser) were performed in all cases. Pre-and post-operative examination included Shack-Hartmann aberrometry for 3 mm and 5 mm pupil sizes, CCT and IOP. Postoperative results at 1 year were compared to baseline values. Results: Changes in average values of coma, spherical aberration (SA), trefoil, CCT and IOP were not significant. Linear regression revealed significant associations between change (y) in trefoil and pre-enhancement value of trefoil for 3 mm pupil (0.828x-0.045, r=0.722, p<0.001), coma and pre-enhancement value of coma for 5 mm pupil (y=0.281x-0.030, r=0.501, n=30, p=0.048), SA for 3 mm pupil compared with change in CT (y=0.0080-0.0009x, r=-0.378, p=0.0392), and SA for 5 mm pupil compared with change in CCT (y=0.0035x-0.0541, r=0.524, p=0.0029). Also there was a significant association between IOP and CCT both pre-(IOP=0.0313CT-3.3, r=0.740, p<0.0001) and post-enhancement (IOP=0.0243CT-0.018, r=0.675, p<0.0001). Conclusion: LASIK enhancement with an aspheric ablation profile did not significantly impact on the average values for HOAs, IOP or CCT. For individual cases, the likely shift in magnitude of coma, trefoil and SA can be estimated and, the change in SA is correlated with changes in CCT. The change in the abscissa value linking IOP with CCT may be an indication of changes in biomechanical properties of the cornea in the central region.

Evolution of visual acuity, flap thickness, and optical density after laser in situ keratomileusis performed with a femtosecond laser
摘要: To describe the postoperative evolution of visual acuity, flap morphology, and stromal optical density femtosecond laser-assisted laser in situ keratomileusis (FS-LASIK). The study comprised 44 consecutive eyes that had FS-LASIK performed with the WaveLight FS200 and the Allegretto excimer laser to correct myopia. Visual outcomes, flap thickness, and stromal optical density were assessed 1 day, 1 week, 1 month, and 3 months postoperatively. A statistically significant improvement in the mean uncorrected distance visual acuity at 1 day (0.94 ± 0.2) and 1 week (0.93 ± 0.2) to 1 month (1.04 ± 0.2) and 3 months (1.11 ± 0.1) postoperatively (P < .05). At 3 months postoperatively, the femtosecond laser had good outcomes in efficacy (0.98 ± 0.1), safety (0.98 ± 0.1), and predictability (100% of eyes were within ± 0.5 D of emmetropia). The femtosecond-created flaps were slightly thicker than intended, the mean SD intraflap was 7.1 μm, the range between the thickest and thinnest points in each flap was 25.4 μm, and the mean flap thickness homogeneity was 7.6 μm at 3 months postoperatively. A progressive decrease in the optical density of the flap stroma and the residual stromal bed was detected during follow-up. The femtosecond laser study appears to be a safe, effective, and predictable platform to obtain LASIK flaps. The flaps were planar and homogeneous but slightly thicker than intended. The optical density of the flap stroma was slightly higher at early follow-up and decreased over time.

Outcomes of Flap Lift Laser In Situ Keratomileusis Enhancements in a Large Patient Population
摘要: Purpose: To compare the incidence of corneal epithelial ingrowth (EIG) and the visual outcomes of enhancement with laser in situ keratomileusis (LASIK) by ?ap lift when a bandage soft contact lens (CL) was ?tted after the procedure and when it was not. Methods: This is a multicenter, retrospective, interventional case series in which all patients who underwent LASIK enhancement from January 2005 to January 2015 were included consecutively. Patients were grouped depending on whether a bandage CL was ?tted after re-treatment by ?ap lift (group 1) or not (group 2). Results: A total of 16,702 eyes from 12,485 patients were included. There were statistically signi?cant differences between the groups regarding corrected distance visual acuity (better in group 1) and safety index (better in group 2) although these were not clinically relevant. The incidence of EIG was 0.58% in group 1 and 0.55% in group 2 although, again, the difference was not statistically different. Conclusions: We found no relevant differences in LASIK enhancement between patients who received a bandage CL and those who did not regarding the incidence of EIG and visual outcomes.

Comparison of anterior segment changes after femtosecond laser LASIK and SMILE using a dual rotating Scheimpflug analyzer
摘要: Background: To compare the changes in the anterior segment after femtosecond laser in situ keratomileusis (FS-LASIK) and small incision lenticule extraction (SMILE) using a dual rotating Scheimpflug (DRS) analyzer (Galilei?; Ziemer Ophthalmology, Port, Switzerland). Methods: A total of 218 eyes of 109 patients who underwent FS-LASIK or SMILE for myopic correction were retrospectively studied. Ninety-eight eyes of 49 patients who underwent FS-LASIK were compared to 120 eyes of 60 patients treated with SMILE. A DRS analyzer was used for preoperative and 6-month postoperative anterior segment analyses. Measured variables included the central corneal thickness (CCT), anterior chamber depth (ACD), anterior and posterior keratometry (K), anterior and posterior best-fit sphere radius, and maximum posterior elevation (MPE). Results: After the procedure, the amount of CCT decrease was higher in the SMILE group than in the FS-LASIK group, but it was not statistically significant. The MPE was significantly increased after both procedures (p < 0.001 and p = 0.001 in the FS-LASIK and SMILE groups, respectively), with the amount of elevation being higher after FS-LASIK than after SMILE even though it was not statistically significant. And there was a significant change in the steep and average posterior K in the FS-LASIK group (p = 0.006 and 0.001, respectively), but not in the SMILE group. Conclusions: Regarding changes in the MPE and posterior K, changes in the posterior corneal surface were greater after FS-LASIK than after SMILE.

Comparison of Laser Platform Estimation and Objective Measurement of Maximum Ablation Depth Using Scheimpflug Pachymetry in Myopic Femtosecond Laser In Situ Keratomileusis
摘要: To compare the preoperative estimation of maximum ablation depth provided by the laser platform with objective measurement using Scheimpflug pachymetry in myopic femtosecond laser-assisted in situ keratomileusis (FS-LASIK). This is a prospective study carried out at the Rothschild Foundation, Paris, France. In 89 consecutive myopic eyes (45 patients) treated with FS-LASIK, the maximum ablation depth was calculated by measuring the difference between preoperative and 3-month postoperative measurements in central corneal thickness using Scheimpflug pachymetry (Pentacam HR; Oculus Optikger?te, Wetzlar, Germany). These values were compared with the preoperative estimation of the maximum ablation depth provided by the laser platform, without nomogram adjustment. We found a strong linear correlation between laser platform estimation of maximum ablation depth and Scheimpflug pachymetry (P , 0.00001). The mean ablation depth measured using Scheimpflug pachymetry was greater than that predicted by the laser platform, with a mean overall difference of 2.15 mm (P , 0.05). Subgroup analysis revealed a difference in ablation depth measurements of 7.1 mm in the high myopia subgroup, which was the only subgroup where the difference remained statistically significant. In myopic FS-LASIK, laser platform estimation of maximum ablation depth correlates well with measurement of maximum ablation depth using Scheimpflug pachymetry and therefore can be used safely.
