Laser hair removal following forehead flap for nasal reconstruction
摘要: The forehead flap is a dependable option for nasal reconstruction owing to its reliability and anatomic likeness to nasal skin. For patients with low hairlines, the vertical design of the paramedian forehead flap can intrude into the scalp, thus incorporating hair into the nasal reconstruction. The inadequate length of the forehead flap or shift to an oblique design may result in eyebrow elevation and asymmetry. Therefore, laser hair removal (epilation) on the forehead flap has been proposed to improve esthetic results. An alexandrite laser (755 nm, 10 to 20 ms, 18-mm spot size) with a Dynamic Cooling Device? (DCD?) cooling system was used for hair removal in 22 patients (16 male and 6 female patients) after nasal reconstructions using forehead flaps from December 2011 to September 2016. All patients received cryogen spray cooling laser treatment (CSC-LT). The mean follow-up period was 24 months, with a range between 18 and 50 months. The average duration of treatment was 1.8 months (range, 1–5 months). The energy density ranged from 14 to 18 J/cm2 with an average of 17.2 J/cm2. The number of treatments ranged from 2 to 4 (mean 2.8). Patients had satisfactory esthetic results over 11.1 months (range, 8–18 months). Residual white hairs were observed in 3 patients, and 4 patients had tiny black residual hairs without deteriorating cosmesis. Using an alexandrite laser to remove hair on the forehead is safe and reliable in nasal reconstruction with superior recipient site cosmesis.

800??nm diode laser does not display longa??term benefit for hair removal in Becker's nevus: A retrospective analysis of 24 cases
摘要: Becker’s nevus is manifested by either ipsilateral or bilateral pigmented patch with or without hypertrichosis. Although other regimens, including oral antiandrogen and topical glycolic acid, are available, lasers are the most widely used therapeutic regimen. The clinical outcomes of laser therapy for Becker’s nevus vary greatly with studies, skin color, body site and number of treatment session. Previous studies showed that a diode laser reduced hair density by 47-80% in Fitzpatrick skin type V-dominated cohort, whether diode laser can effectively remove hair in Becker’s nevus in Chinese is unknown. Moreover, whether diode laser exhibits long-term benefit for hair removal in Becker’s nevus also remains to be determined. Here, we retrospectively analyze the efficacy and safety of 800 nm diode laser for hair removal in Backer’s nevus in Chinese.

The effects of 755 nm alexandrite laser on skin dryness and pruritus
摘要: Introduction: The alexandrite laser (AL) is a very safe and effective treatment used for unwanted hair removal with a reported success rate of 40% to 80% at 6 months and after several treatment sessions. Although a diffuse variety of side effects has been observed during laser treatment, changes in skin dryness and pruritus before and after AL epilation have not been reported yet to the best of our knowledge. Aim: To investigate the effects of 755 nm alexandrite laser on skin dryness and pruritus at the beginning and in the third and the sixth month after the treatment. Material and methods: Forty three patients with Fitzpatrick skin types of II–IV aged 18–45 with leg hair were included in this prospective study. Patients were treated with 755 nm alexandrite laser with 10–12 mm spot size. According to the skin phototype, the settings of the laser were as follows: 12–22 J/cm2 and pulse width of 3 ms. For self-assessment by the patient, the visual analogue scale (VAS) was used before, at the third and sixth month of the treatment as to skin dryness and pruritus. The patients were evaluated by the same dermatologist with the same VAS. The values were compared between before-at the third month, before-at the sixth month and at the third and at the sixth month of the treatment. Results: Pruritus scores were statistically lower at the third month when compared with the baseline scores (p < 0.01). However, there was no difference between the third and sixth month of the treatment as to pruritus scores (p > 0.05). There was a statistically significant difference between the scores before the treatment and the scores at the third month and at the sixth month as to skin dryness (p < 0.001). However, the difference was not prominent between the third and sixth month scores of skin dryness (p > 0.05). Conclusions: To the best of our knowledge, this is the first study researching the effects of AL on pruritus and skin dryness. Further studies with larger samples and longer follow-up periods will be able to better clarify the association.

Laser depilation as adjuvant therapy in prevention of recurrence of pilonidal sinus disease: initial experience of a district general hospital in the UK
摘要: INTRODUCTION Pilonidal disease is a chronic condition involving the sacrococcygeal area. It can have a significant impact on quality of life, social activities and occupation. Surgery is the mainstay of treatment. However, laser depilation has been proposed as an adjunct to surgery and has shown improved results in some studies. We present our experience of laser depilation in the treatment of pilonidal disease in a district general hospital setting in the UK. MATERIALS AND METHODS A prospectively maintained database of all consecutive patients who received laser depilation after elective surgery for pilonidal disease was analysed. Patients were offered a minimum of two sessions of laser depilation. The primary outcome measure was disease recurrence. Evidence of new symptoms or signs of pilonidal disease after one year from the latest surgical intervention was defined as recurrent disease. Data are presented as medians with interquartile ranges. RESULTS A total of 64 patients underwent laser depilation after elective surgery between 1 June 2013 and 1 June 2018; 57 were eligible for final analysis. Sixty-five per cent of patients received six or more sessions of laser treatment. There were no short- or long-term complications related to laser depilation. Patients who had more than two sessions of laser depilation showed an improved recurrence rate. Overall, recurrence rate in our series was 12% at a median follow-up of 172 weeks. CONCLUSION Laser depilation is a safe and effective adjunct to surgery in minimising the recurrence of pilonidal disease. Patients with primary pilonidal disease and those who are undergoing minimally invasive surgery may also benefit from adjuvant laser depilation. Further high-quality control trials are required to assess its efficacy and safety.

The qualification and training of laser/intense pulse light hair removal operators within South Africa
摘要: Background: Lasers and intense pulse light (IPL) sources are powerful devices that can cause skin burns, pigmentary changes, and scarring if used incorrectly. Adequate training is essential, and regulations are required to limit complications. Aims: The purpose was to investigate the qualifications and training obtained by laser hair removal operators in South Africa. Methods: Questionnaires were distributed and information gathered from owners/managers of laser clinics, suppliers of laser devices in South Africa, individuals in the workplace performing laser hair removal procedures, and accredited tertiary institutions. Results: A majority of clinic owners/managers (94.45%) felt that more emphasis should be placed on laser hair removal training at a tertiary level, and 66.67% outsource additional training provided by the manufacturer of laser devices. Based on the survey to manufacturers, 50% did not require any formal qualification as a minimum requirement, while 33.33% indicated laser hair removal training is incorporated at a NQF level 4 (National/Senior Certificate). The majority of individuals (68.89%) received training from tertiary institutions; however, they did not receive any practical training, and 60.87% felt the amount of training was insufficient. According to the survey sent to tertiary institutions in South Africa, only 27.78% offer laser hair removal training, and of these, 20% offer no practical training and 50% indicated that no practical examination is provided. Conclusions: There is a clear lack of training in laser hair removal in South Africa. The industry should have standard requirements in terms of minimum practical and theoretical hours with regards to the therapy.

摘要: All lasers are not the same. They are tools or instruments that come in a variety of different forms and are designed to perform different tasks. Some lasers vaporize the outer layers of sun-damaged or aging skin. Some can also pass through the outer layers of the skin and destroy deeper birthmarks (port-wine stains) or tiny blood vessels (spider veins.) As is the case with most technology, the use of lasers is an art form that improves with training and experience. Lasers, like any technology, must be used for the right reasons. The right reason is that, in the facial surgeon's opinion, it is superior to other forms of treatment, in terms of safety and outcomes. For some conditions, lasers exceed other forms of treatment. For others, I have found laser therapies to be inferior. So, if training in dermabrasion and chemical peeling was not offered during residency or fellowship, it is recommended that the surgeon arrange to spend enough time with a colleague who performs these procedures to feel comfortable offering and performing them.

摘要: The pulsed dye laser has revolutionized the treatment of vascular anomalies. More recently, the multiplex laser, a combination of pulsed dye and Nd:YAG, has been shown to be beneficial for recalcitrant port wine stains and other conditions. In addition to their use in treating port wine stains, lasers have also an important role in treating a number of childhood pigmentary disorders. These are primarily congenital naevi such as café‐au‐lait macules, naevus of Ito and some blue lesions. Laser treatment is based on selective thermal destruction or damage to a targeted chromophore located in the skin, either that of blood, melanin or tissue water. Lasers have been shown to successfully treat vascular and pigmented lesions and also remove unwanted hair. They can also be used to remove verrucous lesions such as angiofibromas and epidermal naevi through vaporization.

Successful Treatment of Keratosis Follicularis Spinulosa Decalvans With an 800-nm Diode Laser
摘要: Keratosis follicularis spinulosa decalvans (KFSD) is a rare genodermatosis characterized by widespread keratosis pilaris and progressive cicatricial alopecia of the scalp, eyebrows, and eyelashes; photophobia; and corneal dystrophy. We report a man with KFSD who was successfully treated with laser hair removal on the scalp. A 23-year-old man presented with an 8-year history of hair loss, which had started as a discrete 2.5-cm bald patch on the vertex of the scalp. He gradually developed more patches of alopecia, which expanded and coalesced, and in which the hair never regrew. He complained of itching and tenderness in the affected areas. Furthermore, he had dry skin, with rough-feeling bumps on the upper arms and thighs. As a child, he suffered with photophobia requiring the use of tinted spectacles, but this subsided during his teenage years. He had no other medical history of note and did not take any regular medication. He was of skin phototype II. There was a strong history of male pattern hair loss on the paternal side of the family.

Hair removal treatment using 1,064 nm long-pulsed Nd:YAG laser in auricular post reconstruction of microtia patient: two case reports
摘要: Microtia is a congenital anomaly of the external and middle ear with various degrees of severity. The hair growth on the reconstructed auricle causes aesthetic problems. We describe two cases about the successful epilation of unwanted hair in postauricular reconstruction of microtia patients using 1,064 nm long-pulsed neodymium: ytrium-alumunium-garnet (LP Nd:YAG) laser in Fitzpatrick skin type IV patients, using 6x6 mm tip, 35 J/cm2 fluence, 10 ms pulse, and 1 Hz frequency with burnt of hair as an endpoint. On the follow up day 11 of case one and day 7 of case two, the hair reductions on treated auricles were 46% and 58%, respectively, with no adverse effects. Minimal 6 repeated sessions are recommended for optimal result. In these case reports, LP Nd:YAG laser treatment is considered effective for the unwanted hair growth on the reconstructed auricle in dark skin type patients with minimal risk of adverse effect.

Safe and Effective Laser Hair Removal Treatments
摘要: Objective: The aim of this study was to review the current literature regarding safe and effective laser hair removal (LHR) treatments. Methods: Significant and current studies (2008–2017) were retrieved through a series of searches conducted in CINAHL, Cochrane, and MEDLINE databases using Boolean terms/phrases. Pertinent results are explained in narrative form with supporting tables and figures. Results: Today, there is a vast amount of current literature on the need for LHR treatments and on its side effects and comparative studies using various lasers on the market. There is an ongoing concern associated with the lack of standards and guidelines. Conclusion: Practitioners interested in offering LHR within the primary care setting must verify that operating a Class 4 medical device is within their scope of practice. Despite the increased popularity of LHR treatments and the advancement of technology, there are limited up-to-date information on safe and effective treatment parameters and minimal information regarding the importance of patient education. More research is necessary to determine the efficacy of LHR treatments in both men and women of all ages and skin types. With no LHR guidelines in place, the best practice for LHR treatments is contained within this literature review using the most recent research available to date.
