  • Long-Wave Medical Infrared Thermography: A Clinical Biomarker of Inflammation in Hidradenitis Suppurativa/Acne Inversa

    摘要: Background: A more reliable classification of skin inflammation and severity of active disease results from ultrasound sonography and the new hidradenitis suppurativa/acne inversa (HS) classification system IHS4. However, an objective assessment of skin inflammation in a continuous mode is still the ultimate goal. Long-wave medical infrared thermography (MIT) may offer a blood flow and temperature differential assessment in inflammatory conditions. Objective: To evaluate the application of MIT in HS. Methods: Standardized photography of the areas involved or previous candidates for HS involvement was performed and MIT pictures were taken simultaneously and superimposed on the photographs of 18 patients (11 female, 7 male, median age 38.75 years [95% confidence interval 28.5–51 years], Hurley score I 5.6%, Hurley score II 38.9%, and Hurley score III 55.5%). A modification of the Otsu method facilitated the automatic lesion segmentation from the background, depicting the inflammation area. Moreover, MIT was administered in real-time mode during radical HS surgery. Results: A 1°C temperature difference from a corresponding symmetric body region was indicative of inflammation. MIT figures detected a gradual increase of skin temperature from 33.0°C in healthy skin on average to 35.0–36.6°C at the center of inflammatory lesions in the axilla and to 35.4–36.9°C at the center of inflammation in the groin area. Real-time MIT assessment enabled the definition of the margins and depth of the surgical intervention during the procedure. Conclusion: MIT is a promising tool for the detection of inflammation severity in HS lesions and can be used as a clinical biomarker in evaluation studies of medical and surgical HS treatment.

  • Laser and Light-Based Treatment Modalities for the Management of Hidradenitis Suppurativa

    摘要: Hidradenitis suppurativa is a chronic inflammatory skin disease characterized by inflammatory nodules, abscesses, and sinus tracts in intertriginous areas that has a significant impact on patient quality of life. Treatments range from topical, intralesional, systemic, and surgical. Furthermore, laser and light-based treatment options have become increasingly popular because of the limitations of adverse effects associated with systemic therapy and decreased down-time associated with laser and light therapy. These modalities are thought to work through a variety of mechanisms including decreasing inflammation, destroying hair follicles, targeting sebaceous glands, killing bacteria, and debulking lesions through ablation. This review explores these laser and light-based treatment modalities for the treatment of hidradenitis suppurativa.

  • Performance evaluation of solar absorption-compression cascade refrigeration system with an integrated air-cooled compression cycle

    摘要: Hidradenitis suppurativa (HS), also known as acne inversa, is a chronic inflammatory skin disorder affecting apocrine gland-bearing areas of the body. Although the precise pathogenesis of HS is unclear, genetic mutation may one important factor. Recently, loss-of-function mutations of nicastrin (NCSTN), a subunit of γ-secretase, are detected in some familial HS patients. In 2016, we reported that the expression of both AKT and its activated form phosphorylated-AKT (p-AKT) is up-regulated in NCSTN-silencing HaCaT cells. But how defective NCSTN affected AKT is still unknown. MicroRNAs (miRNAs) are small noncoding RNAs functioning as biological regulators in some skin diseases. In previous study, we found that miR-100-5p was down-regulated in both familial HS and keratinocyte specific-NCSTN-knockout (NcstnΔKC) mice. As AKT-related pathway is one target of miR-100-5p, whether decreased miR-100-5p expression could affect skin cells through this pathway in familial HS with NCSTN mutation remains to be proved. Here, we investigated the effect of miR-100-5p on AKT to influence skin keratinocyte proliferation in the presence of NCSTN mutation.
