  • New technology for coagulation of dilated vessels using the combined effects of several modes of generation and wavelengths in one laser pulse for the treatment of pediatric hemangiomas: Open prospective study

    摘要: An open, prospective, nonrandomized study of 122 children with infantile hemangiomas aged 1 to 24 months was conducted to evaluate the effectiveness and safety of treatment with multiline laser equipment using the Nd:YAP Q-Sw/KTP emitters with the combined use of two wavelengths of 1079/540 nm. The average age of the children was (6.3 ± 0.3) months, 22 of them were boys (18.0%) and 100 were girls (82.0%). An erythometry and ultrasound examination were performed to determine the depth of the neoplasm, to assess its blood flow, and the presence and diameter of the supply vessels. Studies of these indicators were conducted for patients before and after the treatment. Laser treatment was performed on 109 patients with 119 hemangiomas. A total of 81 superficial hemangiomas underwent a short course of laser therapy (2-5 procedures) and remaining patients with 38 combined hemangiomas received a long course of laser treatment, consisting of 6 to 10 procedures. Restoration of normal color, skin relief, and the absence of scars were noted in post-treatment sample on evaluation. After the course of treatment, erythometry readings corresponded to the values of normal skin and decreased to 110 to 80 cu. Ultrasound examination showed vascular formation and feed vessels were not visualized.

  • Effect of different laser types on bonding strength of CAD/CAM-customized zirconia post to root canal dentin: an experimental study

    摘要: In clinical dentistry, the strength of bonding zirconia posts to root canal dentinal walls currently needs enhancement, and laser application can be an important contribution owing to its features that accommodate adjustable modifications on dental materials. Herein, the effect of different laser treatments applied to dentin surfaces on the strength of bonding zirconia posts to root canal dentinal walls is evaluated by using the pull-out bond strength test in a laboratory setting. A total of 40 single-rooted permanent mandibular premolar teeth that were freshly extracted were used here. The root canal preparation steps were performed using the crown-down technique. Custom-made zirconia posts were produced using CAD/CAM technology. Prior to the application of resin cement, the internal surfaces of the root canals were irradiated using Nd:YAG, Er:YAG, and KTP lasers. Pull-out tests were performed on each specimen by using a universal testing machine. One-way analysis of variance and Tukey post hoc tests were used to compare the pull-out bond strength data. The bond strengths of the laser-treated specimens were greater than those of the untreated controls (p < 0.05). While the value of the pull-out bond strength after Nd:YAG laser treatment was significantly higher than the values obtained after the applications of the Er:YAG and KTP lasers (p < 0.05), the pull-out bond strength after Er:YAG laser treatment was considerably greater than that after KTP laser treatment (p < 0.05). The bond strength between the root canal dentin and the CAD/CAM custom-made zirconia post was improved upon using all the laser modalities in current laboratory settings, among which, application of the Nd:YAG laser was the most successful.

    摘要: The treatment of superficial vascular lesions on skin are among the most common sought-after dermatological laser procedures. The treatment method is based on the absorption of laser light by haemoglobin in blood, which results in the transfer of heat and wanted vessel wall damage. The yellow wavelength range is considered ideal for this purpose, because of the high absorption peak of haemoglobin around 577 nm. However, the yellow spectral range is challenging for any laser technology to produce, which is why there are only a few options on the market, notably the pulsed dye laser (PDL), resulting in limited and expensive treatments. We report on a clinical study that assessed the efficacy of a compact high-power yellow laser system designed for the treatment of cutaneous vascular lesions, such as telangiectasia. The laser system is based on frequency-doubled vertical-external-cavity surface-emitting lasers (VECSELs); a novel technology, which has emerged as an attractive alternative to solid-state, and dye lasers due to the enhanced functionality and broad wavelength coverage [1,2]. The system includes a handheld scanner for an easy and fast delivery of laser light onto skin and is capable of emitting 8 W (continuous wave) at the challenging 590 nm wavelength range (see Fig. 1 Left). The first head-to-head clinical study (ClinicalTrials.gov Identifier: NCT03472859) compared the efficacy of the VECSEL-based yellow laser system and a traditional green KTP laser in the treatment of facial telangiectasia [3]. Altogether 24 subjects participated and completed the clinical study. The subjects were randomized to receive KTP treatment on one side of the face and yellow laser treatment on the other side. Each subject received 1–2 treatments with both lasers. The efficacy of the treatments was assessed by blinded investigators using before and after images taken with a Visia imaging system (see Fig. 1 Right). There was no statistically significant difference between the efficacies of the two lasers; 83% of the subjects had at least 50% improvement after the first treatment with the yellow laser system and similar results were observed for the KTP laser. Treatments with the yellow laser system were 4.7-fold faster than with the KTP but subjects experienced more pain, which limited the use of higher fluences. The yellow laser system settings were 5.6-8.1 J/cm2, whereas as green KTP settings were 20–30 J/cm2. In the next study, we will add a cooling device to the yellow laser to enable the use higher fluences, which is expected to lead to better results, as demonstrated by Kapiciogly et al [4].

  • Successful treatment of multinucleate cell angiohistiocytoma in an adult male patient with intralesional corticosteroid and KTP laser

    摘要: Multinucleate cell angiohistiocytoma (MCA) is a rare, benign, idiopathic, vascular and fibrohistiocytic proliferation characterized by strong female-predominance and predilection for the extremities. Most often asymptomatic, this condition follows an indolent, yet progressive course, with spontaneous remission being uncommon. There are no standard first-line therapies. Treatments with varying success include surgical excision, cryotherapy, laser (argon; carbon dioxide; pulsed dye), and intense pulsed light. We present a case of MCA treated with a novel combination of intralesional corticosteroid (ILC) injections and potassium-titanyl-phosphate (KTP) laser. A 67-year-old male presented with one-year history of multiple, discrete, well-circumscribed, smooth, somewhat flat-topped, erythematous to violaceous papules on his bilateral dorsal hands. Past medical history was significant for gout, hypothyroidism, hypertension, and dyslipidemia. Skin biopsy revealed an increased number of vascular channels and large angulated multinucleate cells in the upper and mid dermis, consistent with a diagnosis of MCA. The patient requested treatment for cosmetic purposes. Initial treatment with ICS injections (0.5 mL of 10 mg/mL triamcinolone acetonide) did not provide significant benefit. An additional two sessions, each 8 weeks apart, of ILC injections (0.3 mL of 5.0 mg/mL triamcinolone acetonide) combined with KTP laser (wavelength: 585-nm; fluence: 12.0 J/cm2; spot size: 5.0 mm; pulse duration: 15 ms) resulted in near complete clearance with improvement in both induration and discoloration. This report highlights a rare case of MCA in an adult male patient successfully treated with ICS injections and KTP laser. This combination approach can be added to the therapeutic armamentarium for MCA, which is often treatment-refractory.
