  • Management of oral vascular anomalies with diode laser: report of 2 clinical cases

    摘要: Purpose This article reports the usefulness of a 980-nm diode laser (Simpler, Dr Smile) for the treatment of oral cavity vascular abnormalities. Introduction Vascular abnormalities are common vascular lesions characterized by elevated, usually, dome-shaped papules. These range in color from dark blue to dark purple and are seen more frequently in elderly patients. They often occur as single lesions on the lips, cheeks, and tongue. Although these lesions are usually asymptomatic, they can bleed if injured. Case report The first case is about a 42-year-old woman referred for evaluation of a facial and oral bluish lesion. Clinically, we diagnosed this extensive lesion as an acquired hemangioma. Under local anesthesia (Mepivacaine), we performed a photocoagulation of the lesion using a 980-nm diode laser (Dr Smile) in noncontact mode (2 mm of focal distance) with a 300-nm fiber, continues mode, 1.5 W power, during 1 min. The session was repeated 15 days later. After 6 weeks, wound healing was completed uneventfully. The second case is about a woman, aged 54, sent by a colleague for a purplish lesion on the inner side of the left cheek. Clinically, we diagnosed venous malformation. With the same diode (980 nm), we worked 2–3 mm away from the surface of the lesion, with a power of 4 W in continuous mode, under local anesthesia (Mepivacaine). The patient received only one session. The following controls at 10, 20, and 30 days showed the persistence of an ulceration which healed after 40 days. Conclusion The patients had no complications during the laser sessions, and their postoperative period was uneventful. We, therefore, concluded that the diode is an excellent therapeutic alternative in the treatment of oral cavity venous malformations.

  • Spontaneous and Dramatic Improvements in Open Bite Deformity Secondary to Large Maxillofacial Venous Malformation With Macroglossia After Sclerotherapy and Laser Therapy

    摘要: Large maxillofacial venous malformation (VM) lesions can affect the craniofacial skeleton, causing occlusal and craniofacial deformity. Few studies have discussed the management of these skeletal disorders. It is unclear whether orthodontic treatment and orthognathic surgery are necessary after such a VM lesion has been significantly reduced. Methods: A 13-year-old boy with a large, extensive maxillofacial VM lesion, severe facial asymmetry, macroglossia, and lower hypertrophy visited our department in 2010. He received more than 100 sclerotherapy treatments and 20 laser treatments in the past 8 years. Results: The patient’s cosmetic disfigurement greatly improved, and the VM lesion diminished by more than 80%. Changes in the bite and craniofacial skeleton progressed from ‘‘normal’’ to ‘‘open bite with skeletal deformity’’ and finally to ‘‘spontaneously close to normal’’. Conclusions: During the progression of VM, removal of the aggravation of open bite pathogenic factors can inhibit deformity and promote the spontaneous improvement, thereby circumventing the need for complicated osteotomy, orthodontic intervention and/or orthognathic surgery.

  • Management of a laryngeal venous malformation with Nd:YAG laser and bleomycin sclerotherapy

    摘要: Literature describing neodymium:yttrium-aluminum-garnet (Nd:YAG) photocoagulation and sclerotherapy for laryngeal venous malformations (VMs) is sparse. Here we present a case in which an extensive laryngeal VM in a 28-year-old female was managed through a combination of four serial Nd:YAG laser photocoagulation sessions and four bleomycin injections over the course of 2 years. The treatment plan resulted in noticeable lesion ablation, mucosalization, and significant improvement in symptoms. To our knowledge, this case is the first instance of bleomycin injection specifically into a laryngeal VM reported in the English medical literature.
