  • Quantification of Inflammation Following Nd:YAG Laser Capsulotomy and Assessing the Anti-inflammatory Effects of Nepafenac 0.1% and Betamethasone 0.1%

    摘要: Purpose: To quantify the intraocular inflammation, measured using laser flare photometry (LFP), after Nd:YAG capsulotomy and compare the effects of different topical anti-inflammatory regimes. Methods: A single-center, prospective, randomized controlled study, which included 90 eyes of 90 patients with visually significant posterior capsular opacification. In all patients, Nd:YAG capsulotomy was done and was then randomized into three groups: (A) Topical betamethasone 0.1%; (B) Topical nepafenac 0.1%; and (C) Control group: did not receive any anti-inflammatory agent post capsulotomy. Results: There was a significant increase in the mean LFP values 2 h following laser capsulotomy (p < 0.001) and then declined gradually and came back to pre-laser values by 2 weeks in all three groups. The mean LFP values, central macular thickness, and best corrected visual acuity were comparable among groups. None developed central macular edema. Conclusion: Intraocular inflammation following Nd:YAG capsulotomies is minimal and transient. Routine use of prophylactic anti-inflammatory agents may be avoided.

  • Effect of different laser types on bonding strength of CAD/CAM-customized zirconia post to root canal dentin: an experimental study

    摘要: In clinical dentistry, the strength of bonding zirconia posts to root canal dentinal walls currently needs enhancement, and laser application can be an important contribution owing to its features that accommodate adjustable modifications on dental materials. Herein, the effect of different laser treatments applied to dentin surfaces on the strength of bonding zirconia posts to root canal dentinal walls is evaluated by using the pull-out bond strength test in a laboratory setting. A total of 40 single-rooted permanent mandibular premolar teeth that were freshly extracted were used here. The root canal preparation steps were performed using the crown-down technique. Custom-made zirconia posts were produced using CAD/CAM technology. Prior to the application of resin cement, the internal surfaces of the root canals were irradiated using Nd:YAG, Er:YAG, and KTP lasers. Pull-out tests were performed on each specimen by using a universal testing machine. One-way analysis of variance and Tukey post hoc tests were used to compare the pull-out bond strength data. The bond strengths of the laser-treated specimens were greater than those of the untreated controls (p < 0.05). While the value of the pull-out bond strength after Nd:YAG laser treatment was significantly higher than the values obtained after the applications of the Er:YAG and KTP lasers (p < 0.05), the pull-out bond strength after Er:YAG laser treatment was considerably greater than that after KTP laser treatment (p < 0.05). The bond strength between the root canal dentin and the CAD/CAM custom-made zirconia post was improved upon using all the laser modalities in current laboratory settings, among which, application of the Nd:YAG laser was the most successful.

  • Re: Erbium:YAG Laser Treatment of Female Stress Urinary Incontinence: Midterm Data

    摘要: The role of intravaginal laser therapy for the treatment of women with a clinical and urodynamic diagnosis of stress urinary incontinence (SUI) or mixed urinary incontinence was the focus of the recent study by Kuszka and colleagues. To evaluate subjective and objective outcomes of Er:YAG therapy with SMOOTH mode technology, the authors enrolled 59 women, of whom 32 had grade I (54%), 16 had grade II (27%), and 11 had grade III SUI (19%). Using validated questionnaires (ICIQ-UI SF, PISQ 12) and 1-h pad weight at baseline, after two, four, and five laser sessions, and at 6-mo and 2-yr follow-up, the authors found that intravaginal laser therapy led to cure/improvement for grades I and II but not for severe SUI. At least three laser sessions at intervals of 1 mo are required to obtain better results from laser therapy. Outcomes at the 6-mo and 2-yr follow-up visits were similar, indicating that laser treatment is suitable and does not need to be repeated within 2 yr.

  • Advanced Application of Holmium: YAG Laser Combined ALA-PDT for the Treatment of Refractory Condylomata Acuminata in Anal Canal

    摘要: Background: Anal canal condylomata acuminata is a disease caused by the human papilloma virus. Conventional cauterizing laser treatment cannot achieve a satisfactory result owing to a high recurrence rate, and the application of 5-aminolaevulinic acid-photodynamic therapy (ALA-PDT) also has limitations in the anal canal. Holmium yttrium aluminum garnet (Ho: YAG) laser with fiber is found to be effective at removing canal lesions, and may create suitable conditions for ALA-PDT. We aim to investigate the feasibility of Ho: YAG laser combined with ALA-PDT and to explore a more optimal therapy in refractory anal canal condylomata. Methods: Data of 37 patients with anal canal condylomata (number of warts≥ 10 lesions) from May 2017 to March 2019 were reviewed. In total 17 cases were treated with Ho: YAG laser plus ALA-PDT, and 20 patients treated with CO2 laser plus ALA-PDT were selected as control. Wart clearance and recurrence rates were evaluated as well as laser complications. Results: Most warts (88.23 %) were removed after a session of Ho: YAG laser pretreatment. The average number of laser sessions required to clear all warts was 1.94 in the Ho: YAG laser plus ALA-PDT group. Meanwhile, no wound infections or defecation disfunctions were found. Ho: YAG laser with ALA-PDT could significantly reduce wart recurrence rates (17.6 %) in comparison with CO2 laser with ALA-PDT (55 %). Conclusions: Ho: YAG laser combined with ALA-PDT raised the cure rate of refractory anal canal condylomata and has important guidance implications for clinical application.

  • Laser Vitreolysis for Symptomatic Floaters is Not Yet Ready for Widespread Adoption

    摘要: Vitreous floaters are common, related to age, myopia, genetic predisposition, and infiltration of the vitreous body. A subset of patients report symptoms impacting their quality of vision. Treatment with laser vitreolysis, the use of an Nd:YAG laser to vaporize the collagenous vitreous opacities, appears to be used more frequently; however, data regarding long-term safety and effectiveness are lacking. We present currently available data regarding efficacy and safety, as well as additional considerations. Laser vitreolysis of symptomatic floaters should not be routinely performed without additional studies documenting its safety and long-term efficacy. Ideally, the procedure would be effective in the majority of patients and be FDA-approved based on the results of an FDA registration trial before widespread adoption.

  • Tackling conservation challenges using erbium lasers: case studies at the British Museum

    摘要: Research was conducted at the British Museum to investigate the potential of Er:YAG lasers to address challenging conservation problems where traditional conservation methods had failed. The approach followed was to perform laser tests on model samples and unregistered objects to determine the laser alteration threshold fluence on each substrate and the ability of the laser to remove contaminants, such as inorganic crusts and organic coatings. The effects of the laser were assessed by means of optical microscopy, scanning electron microscopy and colour measurements. If tests were successful on the samples and unregistered objects, the laser was then used on artefacts from the collection of the British Museum. This article details the successful results obtained using Er:YAG lasers for the removal of corrosion from metals, remains of old adhesives from ceramics and shells, mould from polychromed limestone and terracotta, modern overpainting and varnish from a fresco painting.

  • Er:YAG laser cleaning of a??San Marziale in Gloriaa?? by Jacopo Tintoretto in the Church of San Marziale, Venice

    摘要: This article presents a parameter optimisation study for the use of an Er:YAG laser in combination with different solvent mixes for the cleaning of a large oil painting on canvas by Jacopo Tintoretto. The painting underwent restoration work between the 1700s and the mid-1900s in which it was lined and some areas were coated with a pig-mented protective varnish, which has oxidised over time and has completely obscured the work. Analysis was undertaken in order to study the condition of the work, the paint-ing technique used, and to monitor and verify the effectiveness of the cleaning treat-ments considered to remove the oxidised varnish. The article reports how the careful calibration and use of cleaning techniques involving the Er:YAG laser and solvent mixes have led to a full recovery of the vibrant chromatic intensity to the pictorial surface of the work.

  • Fabrication and AFM characterization of selenium recycled nano particles by pulse laser evaporation and thermal evaporation

    摘要: The feasibility of waste collection strikethrough in the traditional evaporation deposition system after preparing thin films was investigated. In this paper, first, the Selenium thin films of 0.5 μm thickness were prepared by a thermal vacuum evaporation system and an evaporation assisted laser Neodymium - Yag pulsed Nd:YAG laser system. Then, the waste materials that remain in the systems were collected and examined. The inspections include the particle size and other structural properties, taken before and after annealing at 300 °C for 30 min. Results show that the prepared thin film and particles are crystalline and the particle size of collected particles are in the range of about <50 nm, which makes this method very useful in the production of recycled nanoparticles of metals and materials that are costly or harmful, but can be an advantage and beneficial.

  • Control of the photoluminescence properties of Nd:YAG crystals by laser irradiation

    摘要: This paper presents luminescence properties of Nd:YAG crystal defects including oxygen vacancy, O-hole center, F and F+ centers. The Nd:YAG crystal defects have photoluminescence emission peaks in the ~ 350-500 nm range of UV-Vis spectral region. The results show that laser irradiation can control photoluminescence properties of the Nd:YAG crystal samples. Our results can be very important in solid state laser and surface engineering.
