P1774Effect of early pitavastatin therapy on coronary fibrous-cap thickness assessed by optical coherence tomography in patients with acute coronary syndrome: the ESCORT study
摘要: Coronary artery disease in young adults is rare, but it is in an increasing trend. However, the risk factors are largely unknown. Previous studies have shown that hyperuricemia is associated with coronary artery disease, but the relationship in young adults is unknown. In this article, we hope to explore the effect of hyperuricemia on coronary artery disease in young adults less than 35 years old. In this retrospective study,we consecutively included young adults (aged 18–35 years old) undergoing coronary angiography in our institution from January 2005 to December 2015. Patients with coronary angiography showed that luminal diameter stenosis≥50% in any of the major epicardial coronary arteries were considered as CAD. The severity of CAD was assessed by gensini score. We tested for interactions between uric acid (hyperuricemia or normouricemia) and other traditional risk factors on the coronary artery disease. The interaction effect on the presence of CAD in young adults was analyzed with multi-variate logistic regression analyses. The relationship between hyperuricemia and CAD in young adults was analyzed with multi-variate regression. 1203 patients (1129 male and 74 female) were included in this study. 790 of young adults were confirmed as having coronary artery disease. The hyperuricemia was present in 35.41% of patients. The multivariate analysis identified hyperuricemia was not independent for the presence of CAD. We furtherly found that the interaction between hyperuricemia and smoking achieved statistical significance for the present of CAD (p=0.0109).In non-smokers, hyperuricemia was an independent risk factor for the presence of CAD (odds ratio [OR]=1.85, 95% confidence interval [CI]=1.06–3.21, P=0.0293). Multivariate analysis identified hyperuricemia was an independent risk factor for the severity of CAD in young adults (odds ratio [OR]=8.45, 95% confidence interval [CI]=1.18–15.71, p=0.0231). In young adults (≤35 years old), hyperuricemia is significantly associated with the presence of CAD in non-smokers. Hyperuricemia has strong correlation with the severity of coronary artery disease in young adults (≤35 years old).

Quantitative assessment of dynamic <sup>18</sup> F-Fluoromethycholine PET and Dynamic Contrast Enhanced MRI in High Risk Prostate Cancer
摘要: Objectives: To describe dynamic 18F-Fluoromethycholine PET (dPET) and dynamic contrast enhancement MR (DCE MR) parameters in localized high-risk prostate cancer (PCa), and determine whether these differ from normal prostate. Furthermore, to determine whether a correlation exists between dPET and DCE MR parameters. Methods: Forty-one consenting patients who underwent prostate DCE MR and dPET were included in this institutionally approved study. Intraprostatic lesions on MR were assigned a PI-RADS v2 score, and focal lesions on PET were documented. All lesions were correlated with pathology. Quantitative and semiquantitative DCE MR and 2-tissue compartmental model dPET parameters were determined and tumor-to-normal gland ratios (T/N) for these parameters were calculated. Finally, dPET and DCE MR correlation was estimated using Spearman correlation coefficients. Results: There were 46 malignant lesions per standard of reference. On dPET, peripheral zone (PZ) tumors had higher K1 (p<0.001), and a T/N ratio ≥2 was significant (p<0.001). On DCE MR, the parameters in, kep, Ktrans and quantitative iAUC were higher for PZ and non-PZ tumors than corresponding normal tissue (p<.001); for PZ tumors, a T/N ratio ≥ 1.5 for Ktrans and pei was significant (p=0.0019 and p=0.0026, respectively). Moderate Spearman correlation (0.40<ρ<0.59) was found between dPET K1 and DCE MR Ktrans and pei. Conclusions: In patients with high-risk PCa, quantitative dPET and DCE-MR parameters in primary tumors differ from normal tissue. Only moderate correlation exists between K1 (dPET) and Ktrans and pei (DCE MR). The incremental value of any of these parameters to PI-RADS v2 warrants further investigation. Advances in knowledge: Unique quantitative and semiquantitative FCH PET/MR parameters in PCa differ from normal gland, and should be further investigated to determine their potential contribution to PI-RADS v2 in the detection of clinically significant PCa.

摘要: Loneliness is a common condition in older adults and is associated with increased morbidity and mortality, decreased sleep quality, and increased risk of cognitive decline. Assessing loneliness in older adults is challenging due to the negative desirability biases associated with being lonely. Thus, it is necessary to develop more objective techniques to assess loneliness in older adults. In this paper, we describe a system to measure loneliness by assessing in-home behavior using wireless motion and contact sensors, phone monitors, and computer software as well as algorithms developed to assess key behaviors of interest. We then present results showing the accuracy of the system in detecting loneliness in a longitudinal study of 16 older adults who agreed to have the sensor platform installed in their own homes for up to 8 months. We show that loneliness is significantly associated with both time out-of-home (β = ?0.88 and p < 0.01) and number of computer sessions (β = 0.78 and p < 0.05). R2 for the model was 0.35. We also show the model’s ability to predict out-of-sample loneliness, demonstrating that the correlation between true loneliness and predicted out-of-sample loneliness is 0.48. When compared with the University of California at Los Angeles loneliness score, the normalized mean absolute error of the predicted loneliness scores was 0.81 and the normalized root mean squared error was 0.91. These results represent first steps toward an unobtrusive, objective method for the prediction of loneliness among older adults, and mark the first time multiple objective behavioral measures that have been related to this key health outcome.

摘要: Aging well consists of the following components: management of chronic conditions, maintenance of physical health and cognitive health, and active social engagement. However, the increasing life expectancy, rising healthcare costs, increasing number of older adults, and decreasing number of care providers pose challenges to the maintenance of different components for aging well. To that end, technological innovation can help to augment human need and capability along the different components, and help overcome the potential barriers in aging well. In this paper, we summarize the published studies for different tablet-based applications designed specifically for older adults targeting different components of aging well. We discuss the strengths and weaknesses of the applications for each component, and identify the opportunities to develop a cohesive application for addressing the different components of aging well.

Micropulse laser for glaucoma in children
摘要: The optokinetic nystagmus (OKN) response (induction/suppression) correlates with subjective visual acuity (VA) in adults but has not been studied extensively in school-aged children. We investigated the correlation between subjective and objective VA as elicited with a new computerized OKN suppression test ("SpeedWheel") in adults and school-aged children. Fifteen children (6-12 years) and 27 adults with refractive errors, amblyopia, cataract, age-related macular degeneration and thyroid associated orbitopathy underwent testing of subjective VA with E- and Landolt C-symbols (Freiburg Acuity and Contrast Test [FrACT]) and objective VA (SpeedWheel) on a LCD screen. Statistical analysis: linear regression, Spearman correlation and Bland-Altman plots. Mean difference against the mean was 0.01 when SpeedWheel was compared to Landolt C, but 0.15 when compared to E-symbols. Overall, SpeedWheel correlated very strongly to FrACT ('E': r = 0.85; P < 0.001; Landolt C: r = 0.81; P < 0.001). This also held true in children ('E': r = 0.74; P < 0.003; Landolt C: r = 0.69; P < 0.005). In ocular disease, SpeedWheel appears to underestimate subjective VA. In 2 patients with loss of vision of unknown origin, subjective VA was lower than SpeedWheel VA. This is in agreement with the findings of Fukai et al. and could indicate aggravation or malingering. C? etinkaya et al found no correlation between OKN induction and subjective VA in 52 children. Thus OKN suppression rather than induction may be a better objective acuity test. SpeedWheel can assess visual acuity in in adults as well as in school aged children.

Clinical Analysis of Newly Diagnosed Diabetes Mellitus Patients by Abnormal Fundus Examination
摘要: Purpose: To investigate the clinical analysis of newly diagnosed diabetes mellitus (NDM) patients with abnormal fundus examination at the first visit. Methods: This retrospective study utilized the first visit medical records of 15 patients (30 eyes) who were diagnosed with NDM from February 2011 to October 2016. Results: Patients were divided into 3 groups: 1) diabetic retinopathy group including proliferative diabetic retinopathy (PDR) (3) and severe non-proliferative diabetic retinopathy (NPDR) (1); 2) retinal vascular disease group including central retinal vein occlusion (CRVO) (1), branch retinal vein occlusion (1), vitreous hemorrhage with CRVO (1) and macular edema (1); and 3) other retinal disease group including vitreous hemorrhage due to choroidal neovascular rupture (1), exudative age-related macular degeneration (3), central serous chorioretinopathy (2), and macular hole (1). All 3 PDR patients had latent autoimmune diabetes in adults (type 1.5 diabetes). The remaining 12 patients had type 2 diabetes. Three patients showed mild NPDR in the opposite eye and the other 9 patients did not have diabetic retinopathy in the opposite eye. Onset age, HbA1C and proteinuria were significantly different between the diabetic retinopathy group and the other retinal disease group (p = 0.006, p = 0.012 and p = 0.006, Mann-Whitney test). Conclusions: In patients with various retinal diseases, early detection of NDM could be achieved by performing fundoscopic imaging and systemic examination as well as basic ophthalmologic examination. In addition, patients with diabetic retinopathy should be treated promptly through ophthalmology and internal medicine consultation. For the retinal vascular disease and other retinal disease groups, not only treatment for ophthalmic diseases, but also education about diabetes treatment are important.
