  • Reporting of light irradiation conditions in 300 laboratory studies of resin-composites

    摘要: Objective. To evaluate how the light delivered to resin-composites was described in recent articles. Method. PubMed was searched for 300 articles published between January 2017 and May 2018 with keywords relating to photocuring of dental materials. The articles examined a wide range of resin-composite properties and performance. For each article, the information provided about the light curing unit (LCU), the light curing conditions and the characteristics and quantity of the light used in the study were recorded. Specifically, the type of LCU used; the irradiance; how the irradiance was measured; the exposure times; whether the light energy (radiant exposure) received by the specimen was determined, or if only the light output at the LCU tip was measured; whether the distance between the tip of the LCU and the specimen was reported; and whether the emission spectrum from the LCU was reported. Where possible, the resin manufacturer’s minimum energy requirement (MER: the product of the recommended minimum exposure time and irradiance) was compared to the radiant exposure delivered to the specimen. Results. Of the 300 articles examined, 217 were published in 2017 and 83 in 2018. Of these articles, 130 (43%) were found in open access journals, and 170 (57%) were in subscription-based journals. The name of the LCU used was not provided in 31 articles, 14 articles did not provide the exposure time, and 227 articles did not report the distance to the specimen. An irradiance value was reported in 231 articles, but this was the irradiance received by the specimen in only 48 instances. The emission spectrum from the LCU was reported in 15 articles. There was a large range in the radiant exposures from below 10 J/cm2 to greater than 100 J/cm2. Significance. The majority of articles from 2017 and early 2018 did not include sufficient description of the characteristics and quantity of the light received by the resin-composite specimens to allow the study to be replicated. It is recommended that future articles should report: (1) the identity of the LCU used; (2) the radiant exposure received by the specimen (J/cm2); and (3) appropriate reference to the emission spectrum from the LCU.

  • Effect of phototherapy on dentin bond strength and microleakage when bonded to resin with different conditioning regimes

    摘要: Aim To assess the efficacy of phototherapy on bond strength and microleakage of bulk fill composites in comparison to conventional dentin surface treatments. Materials and Methods Ninety human third molars were divided into 3 groups (n=30 each) according to surface conditioning treatment. Group 1 (Etch and Bond) EB, group 2 was treated with Er,Cr: YSGG (ECL) and group 3 was treated with a diode laser (DL). Based on the type of bulk fill resin composite, samples were divided into six sub-groups. In sub-group 1, 2 and 3 (n=15) build-up was done using ZirconCore (ZC) and these subgroups were named as EB-ZC, ECL-ZC and DL-ZC. In subgroup 4,5,6 (n=15) MulticCore Flow (MC) was used for core build-up and these subgroups were named as EB-MC, ECL-MC, DL-MC. For shear bond strength (SBS) all the samples were exposed to loads using a universal testing machine. Five samples each from all subgroups were immersed in methylene blue dye for 24hours prior to microleakage testing. Data were assessed using analysis of variance and Tukey multiple comparisons test. Results: Among all the groups the lowest bond strength was achieved in DL-ZC [10.45(0.459)]. Similarly, the highest bond strength was attained in EB-MC[17.84(0.925)]. The highest microleakage scores amongst different group was exhibited in DL-ZC [82.45(39.459)] whereas, the lowest microleakage scores were displayed in EB-MC [31.21(15.92)]. Conclusion: Phototherapy in the form of Er,Cr:YSGG laser showed comparable adhesive bond outcomes to conventional etch and bond dentin conditioning techniques. Moreover, Multicore bulk fill material showed better bond strength and microleakage scores than zirconium particle infiltrated bulk fill composite in the presence of dentin phototherapy.

  • Efficacy of phototherapy with different conventional surface treatments on adhesive quality of lithium disilicate ceramics

    摘要: Aim: The aim of the present study was to evaluate shear bond strength (SBS) of LDC and resin composite in combination with phototherapy and different ceramic surface treatments. Materials and Method: Forty Lithium Disilicate Glass Ceramic (LDC) disks measuring (4x4x9mm) were fabricated. The specimens were randomly divided into four groups (n=10 each) according to the surface treatment. Group 1 HF+ Silane (Control); Group 2 HF+ Ultrasonic bath + Silane; Group 3 SECP (Self etch ceramic primer) and Group 4 phototherapy (Er, Cr: YSGG) + Silane. On each ceramic disk a resin build-up was done. For SBS the specimens were subjected to increasing load with a transversal velocity of 1ml/min on a universal testing machine on a ceramic resin interface. Failure mode was evaluated using digital microscope. The failure modes were divided into adhesive, cohesive and admixed interface. Data through bond strength testing was tabulated using statistical program for social science (SPSS). Means and standard deviations were compared using analysis of variance and Tukey's post hoc test (p<0.05) Results: The maximum and minimum bond strength was observed in group 2 [19.58(1.011)] and group 1 [17.14(1.122)] respectively. The bond strength among experimental group 1 [17.14 (1.122)] and group 4 [17.48(1.145)] were found to be comparable. Specimens in Group 2 displayed significantly higher bond strength among all experimental groups. Commonly adhesive failure mode was observed in the present study, with an incidence of 60%, 100% and 70% in groups 1,2 and 3 correspondingly Conclusion: Phototherapy using laser at frequency 30Hz and 4.5 W can be used as a surface conditioner for LDC alternate to HF acid. Conditioning of LDC using Self-etch ceramic primer showed better SBS outcomes as compared to phototherapy (Er, Cr: YSGG laser).

  • Influence of phototherapy on adhesive strength and microleakage of bleached enamel bonded to orthodontic brackets: An Invitro study

    摘要: Aim: The aim of the present in-vitro study was to explore bleached enamel reversal with phototherapy (Er,Cr:YSGG) when using two dissimilar bonding system to bond brackets on microleakage and shear bond strength. Material and Methods: Amongst one twenty samples, ninety samples were bleached using 35% hydrogen peroxide. After the bleaching procedure, the teeth were randomly divided into three groups according to surface conditioning treatment (n=30 each). Group 1: treated with Er,Cr:YSGG (ECL) Group 2: 10% sodium ascorbate (SA) and Group 3: Bleached only (BO). Based on the type of adhesive, specimens were further classified into eight subgroups. In subgroup 1,2,3 and no treatment (NT) (n=15) adhesive Transbond XT was applied on already etched surface and these subgroups were named as NT-TEP, BO-TEP, ECL-TEP and SA-TEP. Similarly, in subgroup 5,6,7 and NT specimens (n=15) were conditioned with Transbond Plus self-etching primer and these subgroups were named as NT-SEP, BO-SEP, ECL-SEP and SA-SEP. Samples from each subgroup were exposed to increasing load for evaluation of shear bond strength. Microleakage among subgroups were tested by immersing samples in Methylene blue prior one day. Analysis of variance and Tukey multiple comparisons test was used to assess data. Results: The maximum and minimum bond strength was observed in group NT-TEP [17.14(2.54)] and BO-SEP [6.14(0.215)] respectively. Samples conditioned with phototherapy ECL-TEP [16.14(1.231)] exhibited comparable bond strength to specimens conditioned with sodium ascorbate (SA-TEP) [16.72(1.298)]. Similarly, bond strength values of ECL-SEP [13.43(0.921)] and SA-SEP were comparable. The highest microleakage score was demonstrated in BO-SEP [83.21(38.21)] whereas, the lowest microleakage scores were displayed in NT-TEP [23.73(13.45)]. Conclusion: Er,Cr:YSGG phototherapy reversal on bleached enamel with etch and rinse adhesives has a potential to be used in clinical settings alternate to conventional sodium ascorbate when bonding orthodontic brackets.

  • Photodynamic activation of irrigation (using different laser prototypes) on push out bond strength of fiber posts

    摘要: Background: To evaluate the influence of photodynamic therapy (PDT) and light activated irrigation (LAI) using different laser prototypes Er,Cr:YSGG (ECYL), Er:Yag (EYL), Nd-Yag (NYL) on pushout bond strength of Pre-fabricated fiber reinforced composite (PFRC) post to radicular dentin. Material and method: Fifty maxillary and mandibular teeth were collected, cleaned and decoronated. Canals were instrumented using crown-down technique. Flaring of canals was performed using protaper S1, SX files. F1, F2 and F3 files were utilized for finishing of canal preparations. Prepared root canals were dried and obturated with gutta percha. Teeth were placed vertically within the section of polyvinyl pipes using acrylic resin. Post space preparation was completed using peso-reamers. Based on the PDT and laser activated irrigation (LAI) with different laser prototypes, samples were divided into five groups of 10 specimens each. Group 1- PDT, group 2–5.25 % NaOCl (sodium hypochlorite) and 17 % EDTA (ethylene diamine tetra acetic acid), group 3–5.25 % NaOCl with 17 % EDTA and NYL, group 4–5.25 % NaOCl with 17 % EDTA and EYL ; and group 5–5.25 % NaOCl +17 % EDTA and ECYL. Fiber post were luted in root canals with self-etch dual cure cement and teeth were sectioned at three levels (apical, middle and coronal). Push-out test was performed by placing the specimens in universal testing machine. Failure analysis of debonded surfaces was evaluated using stereomicroscope at 40× magnification. Means and standard deviations of push out bond strength were assessed by one-way analysis of variance (ANOVA). Comparison between means of push out bond strength was evaluated using Tukey multiple comparison tests (p = 0.05). Result: The highest push out bond strength was observed in group 5 at all three levels, coronal (10.08 ± 0.74 MPa), middle (8.95 ± 0.94 MPa) and apical (6.00 ± 0.88 MPa). The lowest push out bond strength was demonstrated by group 2 at all levels, coronal (6.15 ± 1.22 MPa), middle (5.65 ± 0.95 MPa) and apical (2.25 ± 0.55 MPa). Intra-group comparison showed decrease in push out bond strength in the coronal to apical direction among all investigated groups. Inter-group comparison exhibited comparable push out bond strength at all three levels of root for group 1 and group 2 specimens (p > 0.05). Conclusion: LAI with different laser prototypes improved push out bond values of PFRC post to root dentin as an adjunct to NaOCl and EDTA treatment. PDT improved push out strength compared to conventional canal cleaning regime.

    摘要: This study was done to evaluate whether the irradiation of dentine with ultrashort laser pulses prior to adhesive procedures contributes to increase the adhesive resistance to microtraction essays. Twenty-four human teeth (third molars) were used, divided into 4 groups (n=6): standard adhesive procedures with etch-rinse adhesive were used in the control group, and in the experimental groups, the dentin was irradiated by 25 fs pulses prior to the adhesive procedures, with varying fluences. One tooth from each group was used to evaluate its surface roughness. After 24h, the teeth were sectioned with perpendicular cuts, producing toothpicks that were submitted to the microtraction test. The results showed that the groups irradiated with fluences under 4 J/cm2 presented similar results among themselves, comparable to the control group, while the group irradiated with 8 J/cm2 showed lower adhesive strength. We propose that the adhesive strength and surface roughness reduction resulting from the fluence increase is probably related to the shielding arising from an electron plasma formation during the ablation, decreasing the material removal efficiency. Nevertheless, the lower energy densities did not affect the adhesiveness, maintaining values similar to the control.

  • Laser-Milled Microslits Improve the Bonding Strength of Acrylic Resin to Zirconia Ceramics

    摘要: Heightened aesthetic considerations in modern dentistry have generated increased interest in metal-free “zirconia-supported dentures.” The lifespan of the denture is largely determined by the strength of adhesion between zirconia and the acrylic resin. Thus, the effect on shear bond strength (SBS) was investigated by using an acrylic resin on two types of zirconia ceramics with differently sized microslits. Micromechanical reticular retention was created on the zirconia surface as the novel treatment (microslits (MS)), and air-abrasion was used as the control (CON). All samples were primed prior to acrylic resin polymerization. After the resin was cured, the SBS was tested. The obtained data were analyzed by using multivariate analysis of variance(α = 0.05). After the SBS test, the interface failure modes were observed by scanning electron microscopy. The MS exhibited significantly higher bond strength after thermal cycles (p < 0.05) than the CON. Nevertheless, statistically comparisons resulted in no significant effect of the differently sized microslits on SBS (p > 0.05). Additionally, MS (before thermal cycles: 34.8 ± 3.6 to 35.7 ± 4.0 MPa; after thermal cycles: 26.9 ± 3.1 to 32.6 ± 3.3 MPa) demonstrated greater SBS and bonding durability than that of CON (before thermal cycles: 17.3 ± 4.7 to 17.9 ± 5.8 MPa; after thermal cycles: 1.0 ± 0.3 to 1.7 ± 1.1 MPa), confirming that the micromechanical retention with laser-milled microslits was effective at enhancing the bonding strength and durability of the acrylic resin and zirconia. Polycrystalline zirconia-based ceramics are a newly accessible material for improving removable prosthodontic treatment, as the bond strength with acrylic resin can be greatly enhanced by laser milling.

  • Effect of different laser types on bonding strength of CAD/CAM-customized zirconia post to root canal dentin: an experimental study

    摘要: In clinical dentistry, the strength of bonding zirconia posts to root canal dentinal walls currently needs enhancement, and laser application can be an important contribution owing to its features that accommodate adjustable modifications on dental materials. Herein, the effect of different laser treatments applied to dentin surfaces on the strength of bonding zirconia posts to root canal dentinal walls is evaluated by using the pull-out bond strength test in a laboratory setting. A total of 40 single-rooted permanent mandibular premolar teeth that were freshly extracted were used here. The root canal preparation steps were performed using the crown-down technique. Custom-made zirconia posts were produced using CAD/CAM technology. Prior to the application of resin cement, the internal surfaces of the root canals were irradiated using Nd:YAG, Er:YAG, and KTP lasers. Pull-out tests were performed on each specimen by using a universal testing machine. One-way analysis of variance and Tukey post hoc tests were used to compare the pull-out bond strength data. The bond strengths of the laser-treated specimens were greater than those of the untreated controls (p < 0.05). While the value of the pull-out bond strength after Nd:YAG laser treatment was significantly higher than the values obtained after the applications of the Er:YAG and KTP lasers (p < 0.05), the pull-out bond strength after Er:YAG laser treatment was considerably greater than that after KTP laser treatment (p < 0.05). The bond strength between the root canal dentin and the CAD/CAM custom-made zirconia post was improved upon using all the laser modalities in current laboratory settings, among which, application of the Nd:YAG laser was the most successful.

  • Effect of Er:YAG laser on debonding strength of laminate veneers

    摘要: Objective: The purpose of this study was to evaluate the debonding strength of laminate veneers after using erbium?doped: yttrium aluminium garnet (Er:YAG) laser. Materials and Methods: A total of 60 bovine mandibular incisor teeth were divided into two groups (n = 30). Cylindrical specimens (0.7 mm × 5 mm) were fabricated from Empress II ceramic. Then, they were cemented to incisors using dual?cured resin cement (Variolink II). In the first group, no laser application was performed. The Er:YAG laser was applied on each laminate veneer at the test group for 9 s by using the scanning method. Shear force to remove the laminate veneers were applied with universal testing machine at a crosshead speed of 1 mm/min. Results: Results of this study exhibited significant differences between the control (27.28 ± 2.24 MPa) and test group (3.44 ± 0.69 MPa) (P < 0.05). Conclusion: This study shows that application of Er:YAG laser decreased the bond strength of laminate veneers.

  • The effects of Er:YAG, Nd:YAG, and Diode (940nm) Lasers irradiation on Microtensile bond strength of two steps self-etch adhesives

    摘要: Nowadays, studies show the effective laser irradiation role on the adhesion of bonding agents to the dentin. Therefore, the present study was set to find an appropriate protocol for the use of Nd:YAG, Er:YAG, and Diode lasers during the application of two steps self-etch adhesives in direct dental restorations in order to achieve higher microtensile strength.
