Dental Imaging With Near-Infrared Transillumination Using Random Fiber Laser
摘要: Contrary to the conventional detection method like radiography, the near infrared light source has been demonstrated to be suitable for dental imaging due to different reflectivity among enamel, dentin, and caries lesion. In this paper, three light sources with different bandwidths based on a transillumination method are compared. The contrast among enamel, dentin, and caries lesion is calculated in different situations. The experimental results show that the random fiber laser has the best comprehensive quality in dental imaging due to its high spectral density, low coherence, and deep penetration. This work provides a guidance for light source selection in dental imaging.

Clinical performance of clinical-visual examination, digital bitewing radiography, laser fluorescence, and near-infrared light transillumination for detection of non-cavitated proximal enamel and dentin caries
摘要: The aim of this study was to compare the clinical performance of clinical-visual examination using the International Caries Detection and Assessment System (ICDAS) II, digital bitewing radiography, near-infrared light transillumination (NIR-LT), and laser fluorescence (LF) for the detection of non-cavitated proximal enamel and dentin caries. The study included 335 patients, aged 12–18 years, with no cavities in the posterior teeth. Clinical-visual inspections of 335 non-cavitated proximal caries were performed by two examiners. For enamel caries, clinical validation included a combination of clinical-visual and digital bitewing radiography assessments. For dentin caries, the clinical validation was opening the cavity. The accuracy rate, sensitivity, specificity, predictive values, and areas under receiver operating characteristic curves were determined. The agreement between the examiners’ measurements was calculated using the kappa coefficient. The sensitivity, specificity, and accuracy of the methods were compared using the McNemar test. The significance level was set at p < 0.05. Digital bitewing radiography had the highest sensitivity (0.96) and accuracy (0.96), and LF had the lowest sensitivity (0.38) and accuracy (0.39). After separation of the lesions into enamel and dentin caries, clinical-visual examination had the highest sensitivity (0.98) and accuracy (0.98) for enamel caries, while digital bitewing radiography had the highest sensitivity (0.97) and accuracy (0.97) for dentin caries. The NIR-LT method had a higher sensitivity for enamel caries (0.86). Each method also differed significantly from the others (p < 0.001). Digital bitewing radiography gave the best prediction of proximal enamel and dentin caries. NIR-LT showed good potential for detection of proximal caries.

Use of a?laser fluorescence device for the in vitro activity assessment of incipient caries lesions; Einsatz eines Laserfluoreszenzverfahrens zur Erfassung der Aktivit?t von initialen kari?sen L?sionen in vitro;
摘要: Purpose To evaluate the progression of demineralization during orthodontic treatment, the assessment of lesion activity is important in addition to the detection of lesion extent. This study aimed to evaluate the ability of laser ?uorescence measurements to differentiate between active and inactive initial lesions in vitro. Methods In all, 63 extracted, permanent human teeth were available for the study. On each occlusal investigation site, surface extent and activity of caries lesions were classi?ed visually using ICDAS/ICCMS (International Caries Detection and Assessment System/International Caries Classi?cation and Management System) criteria, following laser ?uorescence measurements. Teeth were hemisected and assessed for lesion depth. Methyl red dye was applied to validate lesion activity on the sections. A Mann–Whitney U-test was performed to evaluate differences between laser ?uorescence measurements for active and inactive lesions. For visual and laser ?uorescence ?ndings, diagnostic accuracy (Az, sensitivity and speci?city) were calculated. Results A total of 19 sound surfaces and 44 surfaces with ICDAS codes 1/2 were classi?ed visually. The differences in laser ?uorescence measurements between active and inactive lesions were not signi?cant for the initial lesions (p > 0.05). Sensitivity/speci?city for lesion depth was 90.5/92.3% (Az = 0.894), respectively, for visual assessment and 69.1/76.9% (Az = 0.745), respectively, for laser ?uorescence. Sensitivity/speci?city for activity assessment was 68.6/85.0% (Az = 0.768) for the visual method. For laser ?uorescence measurements, no diagnostic accuracy values could be calculated for caries activity assessment. Conclusion The visual detection and activity assessment of initial lesions was more suitable than the laser ?uorescence method. Based on the results of this in vitro study, use of laser ?uorescence cannot be recommended for distinguishing between active and inactive initial caries lesions on occlusal surfaces.

摘要: The size or depth of the dental caries could be underestimated by conventional dental caries detection method, this paper, backscattering tooth imaging method based on near infrared random fiber laser has been investigated. We demonstrate that the random fiber laser is ideally suitable for dental caries diagnosis owing to its near infrared wavelength, high spectral density and low coherence.

An <i>ex</i> <i>vivo</i> comparative study of occlusal and proximal caries using terahertz and X-ray imaging
摘要: Objectives: To examine the effectiveness of terahertz (THz) pulsed imaging (TPI) in comparison to intraoral photostimulable phosphor late (PSP) and cone beam CT (CBCT) for the detection of dental caries ex vivo. Methods: Newly extracted 32 human permanent teeth surfaces (16 with caries and 16 without caries) were serially sectioned mesiodistally and imaged by using four image sets as follows: (1) CBCT; (2) PSP; (3) THz movie video; and (4) THz static images. All images were evaluated twice separately by two calibrated observers for the presence/absence of caries using a 5-grade rating/confidence scale. Weighted κ coefficients were calculated. Different image sets were compared with the histological gold-standard using the receiver operating characteristic and area under curves for each image set were compared using χ2 tests, with a significance level of α = 0.05. Results: Intra- and interobserverκ-values for all image sets were almost excellent ranging between 0.777 and 0.988. Mean Az values of observers and readings were 0.898 for CBCT, 0.888 for PSP images, 0.853 for THz static images and 0.781 for THz video movie. No statistically significant differences (p > 0.05) were found between Az values for the different image sets. When observer scores were evaluated according to caries location again no statistically significant differences (p > 0.05) were found between Az values for the occlusal and proximal caries for the four image sets. conclusions: Terahertz pulsed imaging was found to be successful for the detection of dental caries ex vivo.
