  • Haab striae: Optical coherence tomographic analysis

    摘要: Purpose. — To study Haab striae in adults with primary congenital glaucoma and describe them by anterior segment optical coherence tomography, as well as analyze their evolution over the patient’s lifetime. Material and methods. — Three patients with primary congenital glaucoma were recruited, aged between 17 and 48 years old, with incidentally noted Haab striae on glaucoma consultation. They were studied by: slit lamp; anterior segment optical coherence tomography, Heidelberg Engineering Inc., Germany, (with anterior segment module) and Cirrus HD-OCT, Carl Zeiss Meditec, Dublin, California, USA (with 3D reconstruction); and specular biomicroscopy. The results obtained were contrasted with other studies performed in children. Results. — Rounded excrescences protruding towards the anterior chamber were found, as well as dense, irregular tissue between them. In addition, endothelial abnormalities (polymegathism and polymorphism) were found. These findings fit the evolution of those obtained in pediatric studies and remain stable in adult life. Conclusions. — Through noninvasive methods such as optical coherence tomography and specular biomicroscopy, it is possible to make the diagnosis and follow up these patients. The findings suggest posterior corneal topographic changes; additional studies on this subject may be necessary.

  • Quantitative and Morphological Corneal Endothelial Changes After Selective Laser Trabeculoplasty and Retinal Photocoagulation

    摘要: Selective laser trabeculoplasty (SLT) and retinal photocoagulation (RP) are two common laser procedures often performed at a wavelength of 532 nm, and may affect the corneal endothelium. This study used corneal specular microscopy to determine the impact of these procedures on the corneal endothelium. Design: Retrospective cohort study in a private practice. Methods: There were 249 eyes from 136 consecutive patients who underwent SLT for open-angle glaucoma and 132 eyes from 74 patients who underwent RP included. Corneal specular microscopy was performed immediately before and after each procedure and at 1-month postprocedure. Microscopy data included quantitative measures, such as cell density and central corneal thickness, and morphological measures, including percentage of hexagonal cells and coefficient of variation in cell area. Results: There was a small (just over 1%) reduction in corneal endothelial cell count from pre-SLT to post-SLT (P ? 0.008), and a statistically significant recovery at 1 month (P ? 0.04). Central corneal thickness also transiently increased from pre-SLT to post-SLT (P ? 0.03). Although polymegathism was unchanged, changes in pleomorphism were observed (P ? 0.03). The only change in the RP group was an increase in polymegathism between pre-RP and post-RP (P ? 0.001). Conclusions: SLT has measurable effects on both quantitative and morphological characteristics of the corneal endothelium, which seem to be transient. RP has fewer measurable effects, likely because, although the total laser energy is similar, it is delivered over a much longer time (3 ns versus 0.1 s). The changes observed in both procedures are minor and unlikely to be of clinical significance.

  • Corneal Endothelium Changes 6 Months after Laser Peripheral Iridotomy: Prospective Study

    摘要: A decrease in corneal endothelial cell density (ECD) after laser peripheral iridotomy (LPI) has been reported to varying degrees, whereas some investigations have demonstrated no change. Endothelial cell loss after LPI has been hypothesized to result from excessive heat production during the iris photovaporization (Argon) or photodisruption (neodymium:yttriumealuminumegarnet) processes. In our prospective study, we examined the impact of LPI on ECD, comparing ECD before LPI with that 6 months after LPI. Patients 30 years of age or older who were diagnosed as primary angle-closure suspect (PACS) or with primary angle closure (PAC) or primary angle-closure glaucoma (PACG) in at least 1 eye were recruited between September 2012 and December 2013 from glaucoma clinics at Aravind Eye Hospitals in Coimbatore and Pondicherry, South India. The Aravind Eye Hospitals Institutional Review Board approved the study protocol, and written, informed consent was obtained from all participants. The study adhered to the tenents of the Declaration of Helsinki. Exclusion criteria were (1) bilateral pseudophakia, (2) symptoms or signs of acute angle closure attack, or (3) prior iridotomy, iridoplasty, or incisional glaucoma surgery.

  • Predictive Value of Corneal Donor Demographics on Endothelial Cell Density

    摘要: Corneal endothelial cell density (ECD) is an important measure for determining suitability for transplantation. Although age has been correlated with ECD, the impact of sex and ethnicity is unclear. Methods: Corneal donor information from SightLife Eye Bank was collected between 2012 and 2016. Tests of association were adjusted for covariates using linear regression including age, race, and sex. “Unsuitable for transplantation” was a label assigned to specimens with extensive cell dropout, ECD ,2000 cells/mm2, and poor cell morphology. Repeated-measures analysis was used to account for the within-individual correlation between left and right eyes. Results: A total of 39,679 donor corneas were analyzed, with a mean ECD 2743.5 cells/mm2 and mean age of 58. Simple linear regression demonstrated an association between ECD and age (P , 0.001). Multiple regression showed no association between sex and ECD. Compared with whites, African American, and Asian ethnicities were predictors of increased ECD (mean +45.7 cells/mm2 (P , 0.001) and +90.3 cells/mm2 (P , 0.001), respectively); Hispanic ethnicity was a predictor of decreased ECD [mean 236.9 cells/mm2 (P = 0.002)]. A total of 233 (0.59%) corneas were unsuitable for transplantation, which was associated with age (P , 0.001) but not sex or ethnicity. Conclusions: Analysis of a large sample of donor corneas continues to show age but not sex as a predictor of decreased ECD. African American and Asian ethnicities tend to have slightly higher ECD than that of white donors, whereas Hispanic donors have slightly lower ECD; however, ethnicity was not a predictor of suitability for transplantation. Clinical signi?cance of these ?ndings is yet to be determined.

  • Effect of a single session of micropulse laser trabeculoplasty on corneal endothelial parameters

    摘要: Background: To evaluate the effect of a single session of micropulse laser trabeculoplasty on the cornea in eyes with primary open-angle glaucoma and pseudoexfoliation glaucoma. Methods: This single-centre, prospective, case series enrolled patients with primary open-angle and pseudoexfoliation glaucoma under treatment with glaucoma agents that required additional intraocular pressure reduction. Eyes underwent 360 degrees treatment with 532 nm micropulse laser trabeculoplasty. Central corneal thickness, endothelial cell count, hexagonal cell ratio and co-ef?cient of variation of endothelial cells were measured before micropulse laser trabeculoplasty and at one, three and six months after treatment. Results: Twenty eyes of 20 patients (mean age 67.6 (cid:1) 8.0 years) with primary open-angle glaucoma and 18 eyes of 18 patients (mean age 71.44 (cid:1) 6.43 years) with pseudoexfoliation glaucoma were included in the study. Treatment with micropulse laser trabeculoplasty resulted in statistically signi?cantly lower intraocular pressure compared to baseline in both primary open-angle and pseudoexfoliation glaucoma eyes (p < 0.0001 at both comparisons). Central corneal thickness, endothelial cell count, hexagonal cell ratio and co-ef?cient of variation of the endothelial cell size showed no signi?cant change between baseline and six months after micropulse laser trabeculoplasty in both primary open-angle and pseudoexfoliation glaucoma eyes. Conclusion: A single session of micropulse laser trabeculoplasty did not affect central corneal thickness and corneal endothelium parameters in eyes with primary open-angle and pseudoexfoliation glaucoma.
