  • Safety and Efficacy of Diode Laser Transscleral Cyclophotocoagulation in Eyes With Good Visual Acuity

    摘要: The main aim of this study was to evaluate the safety and efficacy of diode laser transscleral cyclophotocoagulation (TSCPC) in eyes with good visual acuity. A retrospective chart review identified patients who underwent TSCPC for uncontrolled intraocular pressure (IOP) from 2014 to 2016. Enrollment criteria included pretreatment best-corrected visual acuity (BCVA) of 20 of 40 or better, and a minimum of 6 months of postoperative follow-up. Thirty-three eyes of 33 patients(mean age, 72.1 ± 16.0 y) were enrolled (mean follow-up 12.6 ± 6.1 mo). The mean (SD) IOP was reduced 48.3% from 27.1 mm Hg (7.8) at baseline to 13.1 mm Hg (4.2) at last follow-up. Reduction of ≥ 1 glaucoma medications was achieved in 17 eyes (51.5%) at last follow-up. The cumulative probability of complete success (BCVA loss <2 Snelling lines, 20% reduction IOP, no reoperation for glaucoma, no IOP < 5 mm Hg) was 78.8% and 50% at months 6 and 12, respectively. The cumulative probability of qualified success (BCVA ≥ 2 lines with ≥ 20% reduction IOP, no reoperation for glaucoma, no IOP < 5 mm Hg) was 90.1% and 81.3% at month 6 and 1 year, respectively. Significant vision loss, defined as BCVA ≥ 2 lines, occurred in 33% of patients. The most common complications were postoperative iritic (56.3%) and cystoid macular edema (12.5%). TSCPC demonstrates a strong reduction in IOP and glaucoma medication use. However, with significant vision loss in 33% of patients, future prospective studies with a comparison group receiving traditional glaucoma surgery are needed to determine comparative safety and efficacy.

  • Determination of Infrared Laser Energy Dose for Cancer Cells Inactivation as a Candidate of Photodynamic Therapy

    摘要: The aim of this research is to find out the effect of GaAlAs diode laser 808 nm exposure on MCF-7 breast cancer cells in vitro with and without the addition of a photosensitizer. Methylene blue (C16H18N3SCl) with concentration of 2 μM is used as the photosensitizer based on the preliminary results of cytotoxicity assay. Energy dose is set in the range of 23,043 to 322,062 J/cm2. Laser exposure with the addition of the photosensitizer generates the highest percentage of cell death of 20.80% at energy dose of 184,344 J/cm2 and cell death begin to decrease at energy dose above this value. This behavior is likely caused by photodegradation and photobleaching effect of the photosensitizer due to the longer exposure time. However, the percentage of cell death without the addition of photosensitizer is increased proportionally to the increase of energy dose and achieved 32,45% cell mortality at 299,559 J/cm2. This study not only shows that the exposure of infrared laser can be used to inactivate cancer cells but also determines its optimum energy dose, makes it a possible candidate for photodynamic therapy in the future.

  • Laparoscopic Partial Nephrectomy by Diode Laser with Highly Selective Clamping of Segmental Renal Arterial

    摘要: The treatment of small renal masses has shifted from radical nephrectomy to partial nephrectomy, in particular, laparoscopic partial nephrectomy. Renal artery clamping is often necessary to minimize hemorrhage during resection in cases of partial nephrectomy. However, renal artery clamping may lead to greater incidence of renal malfunction after partial nephrectomy. Therefore, novel techniques which could both avoid renal artery occlusion and achieve adequate hemostasis are urgently required. Selective renal segmental artery clamping was one of several common methods that could achieve “zero ischemia.” At the same time, a number of reports about laser?assisted partial nephrectomy on humans using multiple kinds of outstanding coagulative specialties of lasers were published. Among facilities with clinical use of lasers, diode laser emitting a wavelength of 980 nm has been recently utilized in the human body operations. The purpose of our clinical research was to evaluate the effectiveness and safety of application of diode laser (980 nm wavelength) in laparoscopic partial nephrectomy with highly selective clamping of the segmental renal arterial.

  • Antimicrobial Efficacy of Photodynamic Therapy, Diode Laser, and Sodium Hypochlorite and Their Combinations on Endodontic Pathogens

    摘要: Background: Sterilization of the root canal system is of prime importance for a successful root canal therapy. Lasers and photodynamic therapy (PDT) have become the latest choice to eradicate microorganisms in the root canal. Objective(s): To evaluate and compare the antimicrobial activity of diode laser, photodynamic therapy, and sodium hypochlorite along with their combinations on endodontic pathogens: Enterococcus faecalis and Streptococcus mutans. Methods: A total of 120 uniradicular teeth were stored in 5.2% NaOCl solution to remove organic residues and the crowns were sectioned at the cemento-enamel junction (CEJ) to obtain root canal length of 15 mm. Canals were shaped using step-back technique up to #40 K-file and were autoclaved. The selected teeth (specimen) were randomly divided into two equal groups with sixty root canals inoculated with E. faecalis (Group E) and remaining sixty teeth with S. mutans (Group S). Further, the groups were subdivided according to the disinfection technique used. Ten teeth from each subgroup were disinfected with a diode laser, photo activated disinfection (PAD), sodium hypochlorite, a combination of sodium hypochlorite and diode laser, a combination of sodium hypochlorite and PAD respectively. Ten teeth in each group served as control without any disinfection. The treated specimens were transferred to test tubes containing 5ml sterile Luria Bertani broth, incubated and the bacterial count, optical density in each root specimen was calculated and compared. The pairwise comparison of colonies across the subgroups was done by the Kruskal-Wallis test and within the subgroups was carried out using the Mann-Whitney U test. The statistical implication was tested at 5% and the study was performed using SPSS 18.0 ver. (SPSS Inc.). Results: A significant reduction (98%) in the E. faecalis count was observed when the NaOCl was used in combination with the diode laser or PAD. PAD along with 3% NaOCl presents the advantage of utilizing a lower wavelength laser beam. Hence, PAD in combination with NaOCl can be an alternative and better option for root canal disinfection for both the endodontic pathogens, E. faecalis and S. mutans.

  • Malpositioned canine treatment with autotransplantation and laser

    摘要: Children and young adults often have tooth loss due to congenital tooth deficiency, trauma, or caries. Autotransplantation has many benefits. The transplanted tooth can be moved orthodontically, maintains alveolar bone growth potential during eruption, and functional periodontal ligament function also permits tooth eruption, allows the defected areas to be filled with the bones; gingival contour is much more successful than the one obtained with prosthesis. In this paper, treatment steps and follow?up results of autotransplantation case supported with biostimulation are mentioned. A 14?year?old female patient was admitted to the clinic with a complaint of decayed tooth 53 and malposed tooth 13. Mobile primary tooth was pulled out, and the socket was shaped with surgical drills. By performing transplantation of ectopic canine, splint was applied with steel wire and composite. Diode laser was used to provide deep disinfection of canals. The patient underwent low-dose laser therapy for biostimulation immediately after these procedures. We did not encounter any ankylosis, root resorption, periodontal, or functional problems in our evaluation with computed tomography after 3 years follow-up of the patient.

  • Use of high-power diode laser at 1060?nm for the treatment of vascular lesions

    摘要: The use of 1060-nm wavelength light emitted from a high-power diode laser is a novel method for the removal of vascular lesions. Two Caucasian women with Fitzpatrick skin phototype II, who had various vascular lesions, were treated with a 1060-nm high-power diode laser, applying a fluence of 120 J/cm2 and a single pulse duration of 74 milliseconds. Immediate results were that the cherry angiomas became dark and a scab was formed (that remitted 2 weeks after treatment); the telangiectasias and venulectasias disappeared immediately. This technology has reported effective and safe results for removing different types of vascular lesions in the two patients treated. Side effects were those expected for this technique.

    摘要: Nowadays, studies show the effective laser irradiation role on the adhesion of bonding agents to the dentin. Therefore, the present study was set to find an appropriate protocol for the use of Nd:YAG, Er:YAG, and Diode lasers during the application of two steps self-etch adhesives in direct dental restorations in order to achieve higher microtensile strength.

  • Laser‐assisted diagnosis of symptomatic cracks in teeth with cracked tooth: A 4‐year in‐vivo follow‐up study

    摘要: The purpose of this study was to determine whether a near-infrared diode laser could be used as an adjunct to help identify and localise symptomatic cracks in patients with cracked tooth. An 810-nm diode laser at 1 W continuous wave energy with a 200 micro-?bre tip was used to scan the offending tooth. Crack identi?ed during the laser scan was removed, restored and reviewed after four weeks and after 4 years. The laser was able to signi?cantly (P = 0.0001) identify suspected cracked teeth from non-cracked control teeth. Of the twelve patients examined, ten participants experienced discomfort as the laser passed over a crack line. At 4-week and 4-year recall, a majority of the ten treated patients responded positively to the management of crack. This study showed that diode laser could be an adjunctive tool for early detection and management of symptomatic cracks with the possibility of improving long-term survival.

  • Laser Bowel Anastomosis for Dogs and Cats; 犬猫に対するレーザーを用いた腸管吻合術;

    摘要: For the purpose of development of the laser technique in veterinary medicine, the new bowel welding method using a diode laser were devised. A special intestinal clamp was developed for these studies, and it was named laser welding intestinal clamp (LW clamp). In fundamental researches, laser welding of bowel in dogs and cats was confirmed to be possible using the LW clamp and the contact probes under a diode laser. Therefore, we expected that this method is applicable to clinical cases. Laser bowel anastomoses were performed for cats with megacolon and toy-breed dogs with severe bowel obstruction. As short- and long-term clinical results, any severe postoperative complications were not found, and all patients revealed favorable recovery.

  • Improving weld bead homogeneity in short arc GMAW processes applying low power diode lasers

    摘要: Using laser and arc power in equal parts, laser-arc hybrid welding processes are successfully adapted by the industry. This paper shows the stabilizing effects of diode lasers on short arc GMAW (gas metal arc welding) which deliver just a fraction of the supplied arc power. In a comprehensive comparison the stabilized and the sole GMAW processes are confronted and differences regarding process behavior and weld bead properties are pointed out. The tests were carried out with DC01 (1.0330) plates of 1 mm thickness deploying lasers using a mean laser intensity of 1,1 x104 W/cm2. Adding this low amount of intensity provides stability to the weld process and results in weld beads of higher homogeneity. The process behavior is characterized by the transient current and voltage curves which are analyzed in regards to consistency and uniformity. Metallographic analysis is employed to compare the weld bead dimensions at different energy ratios.
