Fractional laser ablation for the targeted cutaneous delivery of an anti-CD29 monoclonal antibody – OS2966
摘要: Monoclonal antibodies targeting cytokines are administered parenterally for the systemic treatment of severe psoriasis. However, systemic exposure to the biologic increases the risk of side-effects including immunosuppression, whereas only a small fraction of the active molecules actually reaches the target organ, the skin. This preclinical study examines the feasibility of delivering a humanized anti-CD29 monoclonal antibody (OS2966) topically to skin using minimally-invasive fractional laser ablation. This approach would enable the targeted use of a biologic for the treatment of recalcitrant psoriatic plaques in patients with less widespread disease while minimizing the risk of systemic exposure. First, the effect of a wide range of laser poration conditions on skin permeation and deposition of OS2966 was tested in vitro to determine optimal microporation parameters. Subsequently, confocal laser scanning microscopy was employed to visualize the distribution of fluorescently-labelled OS2966 in skin. The results demonstrated that delivery of OS2966 into and across skin was feasible. Above fluences of 35.1 J/cm2, skin deposition and permeation were statistically superior to passive delivery reaching values up to 3.7 ± 1.2 μg/cm2 at the most aggressive condition. Selective targeting of the skin was also possible since ≥70% of the OS2966 was delivered locally to the skin. Although nanogramme quantities were able to permeate across skin, these amounts were orders of magnitude lower than levels seen following subcutaneous or intravenous injection and would result in minimal systemic exposure in vivo. The diffusion of fluorescently-labelled OS2966 into the skin surrounding the pores was clearly higher than in intact skin and demonstrated the feasibility of delivering the antibody at least as deep as the dermo-epithelial junction, a critical border region where inflammatory cells cross to promote disease progression. These preliminary results confirm that fractional laser ablation can be used for the cutaneous delivery of OS2966 and now preclinical/clinical studies are required to demonstrate therapeutic efficacy.

摘要: Absorption of topical products through the epidermis is limited by the skin's barrier function. Numerous techniques and agents such as microneedling, dermabrasion, radiofrequency, and lasers have been used to increase penetration within an approach known as transdermal drug delivery. One of these techniques is laser-assisted drug delivery (LADD), which often uses ablative fractional lasers (CO2 or erbium:YAG lasers) because of their capacity to produce microscopic ablated channels. The parameters in LADD need to be adjusted to the patient, the skin condition and its location, and the drug. LADD has been used with various topical products, such as corticosteroids, photosensitizers, and immunotherapy agents (imiquimod or 5-fluorouracil) to treat numerous conditions, including scars, nonmelanoma skin cancer, and photodamage. LADD is a promising technique that enhances the absorption of topical molecules while adding the synergic effect of the laser.

Development of a combination therapy with silanols complexed with boron citrate and ablative-fractional laser for treatment of wrinkles and stretch marks
摘要: Objective: The 21st century's progress in medicine and cosmetology triggered the search for effective and safe new cosmetics and procedures to fight with such problems as wrinkles or stretch marks. The study aimed to use the synergy of silanols with boron compounds and to develop treatment methods supported by an fractional-ablative laser. Methods: Sixty-seven Caucasian people were enrolled in this study: 33 patients with facial and neck wrinkles and 34 patients with stretch marks. Preparations containing methylsilanetriol were pressed into the skin by means of oxygen infusion which were followed by the fractional-ablative laser treatments. Results: The effectiveness of removal of wrinkles was better if combination therapy was used in the form of transdermal delivery of methylsilanetriol combined with laser therapy. The effectiveness of stretch marks removal by combination therapy was comparable to a two laser treatments and more effective than one laser therapy. Moreover, the use of products based on methylsilanetriol stabilized with boric citrate resulted in shortening the period of regeneration after the treatment with fractional-ablative laser by 29-58%. Conclusion: The gel based on methylsilanetriol developed in this study can be successfully used after all laser treatments, but also those related to skin pricking to accelerate regeneration.

Clinical and histopathological comparison of microneedling combined with platelets rich plasma versus Fractional Erbium-Doped Yttrium Aluminium Garnet (Er: YAG) Laser 2940a??nm in treatment of atrophic post traumatic scar: A randomized controlled study.
摘要: Background: Different modalities have been tried for treating atrophic post traumatic scars. Objective: To compare the efficacy of microneedling combined with PRP with that of Fractional Er: YAG laser in treating post-traumatic atrophic scars. Methods: 45 patients with atrophic post traumatic scars were subdivided into three groups. Group I subjected to four sessions of skin microneedling followed by topical application of platelet rich plasma (PRP) immediately after each session. Group II subjected to the same protocol of treatment but for 6 sessions. Group III subjected to 4 sessions of Fractional Er: YAG laser at four weeks intervals for four sessions. Response to treatment was assessed clinically using the Quartile Grading Scale and histopathologicaly using H&E stain, trichrome and Orcein special stains. Results: All patients showed significant clinical improvement regardless of the treatment. There was a significant difference in clinical and histopathological improvement between group I and III, but there was no significant difference in improvement neither between group I and II nor between group II and III. Conclusion: microneedling could be as effective as fractional laser when combined with PRP and when used for more sessions.

Comparison of fractional neodymium-doped yttrium aluminum garnet (Nd:YAG) 1064-nm picosecond laser and fractional 1550-nm erbium fiber laser in facial acne scar treatment
摘要: To compare the safety and efficacy of fractional neodymium-doped yttrium aluminum garnet (Nd:YAG) 1064-nm picosecond laser and fractional 1550-nm erbium fiber laser in acne scar treatment and to assess the adverse effects and complications of the two devices. Thirty patients with cosmetically similar acne scars on both sides of the faces were enrolled and treated four times at 4-week intervals. Each side of the face was treated with either fractional 1064-nm Nd:YAG picosecond laser or fractional 1550-nm erbium fiber laser. Efficacy was evaluated using digital photography taken at baseline and 5 months by a blinded dermatologist using the ECCA grading scale (échelle d’évaluation clinique des cicatrices d’acné). Patient overall satisfaction and self-rated improvement scores were recorded at baseline and 5 months. Adverse effects were recorded at every visit. Twenty-seven patients completed the study. Both devices demonstrated significant median ECCA score improvement from baseline (P < 0.001). However, there was no significant difference between the two in terms of median ECCA score improvement, patients’ perception of scar improvement, and overall satisfaction. Considering the adverse effects, more pinpoint bleeding was significantly observed with the picosecond laser (P = 0.002), whereas more pain was noted with the erbium laser (P < 0.001). Both fractional 1064-nm Nd:YAG picosecond laser and fractional 1550-nm erbium fiber laser are safe and effective in the treatment of acne scars. Costs should be taken into consideration when deciding on which device to use to maximize treatment outcomes.

Fractional laser and laser assisted corticosteroid delivery for hypertrophic scars in thermal burns
摘要: The unprecedented survival of individuals who sustain acute burns and other trauma has increased the necessity for effective modalities in the treatment and rehabilitation of patients. Hypertrophic scars and contractures are common following thermal burns injury. Cutaneous scars can be complex and thus the approach to therapy is often multimodal with only moderate success. Intralesional corticosteroids have long been a main treatment of hypertrophic and restrictive scars. Recent advance in laser technology and applications now provide additional options for improvements in function, symptoms, and cosmesis. The ablative zones created by fractional ablative lasers may also be used in the immediate post-operative period to enhance delivery of drugs and other substances. This case report shows the successful use of fractional laser assisted corticosteroid to treat burn scars. It is need more extensive clinical trial to prove this method can be a therapeutic option for the extensive cutaneous scarring in burn patients.

Combination therapy using non-ablative fractional laser and intralesional triamcinolone injection for hypertrophic scars and keloids treatment
摘要: Combinations of various treatment modalities were shown to be more effective than monotherapy when treating hypertrophic scars and keloids. This study was conducted to assess the effectiveness of combination therapy with non-ablative fractional laser and intralesional steroid injection. From May 2015 to June 2017, a total of 38 patients with hypertrophic scars or keloids were evaluated. The control group of 21 patients received steroid injection alone, and 17 patients (the combined group) received 1550-nm erbium-glass fractional laser treatment and steroid injection simultaneously. The mean number of treatment sessions was statistically fewer in the combined group than in the control group (6.95 vs 5.47, P = .042). There was a significant difference in the patient's scale in the combined group (14.62 vs 22.82, P = .005); however, the observer's scale was not significantly different (17.92 vs 20.55, P = .549). The recurrence rate was 38.1% (8/21) in the control group and 35.3% (6/17) in the combined groups and showed no significant difference (P = .859). However, the mean remission period was statistically longer in the combined group (3.00 months vs 4.17 months, P = .042). Combination therapy with non-ablative fractional laser and intralesional steroid injection showed better results for the treatment of hypertrophic scars and keloids with fewer treatment sessions, better patient satisfaction, and longer remission periods.

Enhanced topical cutaneous delivery of indocyanine green after various pretreatment regimens: comparison of fractional CO2 laser, fractional Er:YAG laser, microneedling, and radiofrequency
摘要: Different devices have been used to enhance topical drug delivery. Aim of this study was to compare the efficacy of different skin pretreatment regimens in topical drug delivery. In six ex vivo human abdominal skin samples, test regions were pretreated with fractional CO2 and Er:YAG laser (both 70 and 300 μm ablation depth, density of 5%), microneedling (500 μm needle length), fractional radiofrequency (ablation depth of ± 80–90 μm), and no pretreatment. The fluorescent agent indocyanine green (ICG) was applied. After 3 h, fluorescence intensity was measured at several depths using fluorescence photography. Significantly higher surface fluorescence intensities were found for pretreatment with fractional Er:YAG and CO2 laser and for microneedling vs. no pretreatment (p < 0.05), but not for radiofrequency vs. no pretreatment (p = 0.173). Fluorescence intensity was highest for the Er:YAG laser with 300 μm ablation depth (mean 38.89 arbitrary units; AU), followed by microneedling (33.02 AU) and CO2 laser with 300 μm ablation depth (26.25 AU). Pretreatment with both lasers with 300 μm ablation depth gave higher fluorescence intensity than with 70 μm ablation depth (Er:YAG laser, 21.65; CO2 laser, 18.50 AU). Mean fluorescence intensity for radiofrequency was 15.27 AU. Results were comparable at 200 and 400 μm depth in the skin. Pretreatment of the skin with fractional CO2 laser, fractional Er:YAG laser, and microneedling is effective for topical ICG delivery, while fractional radiofrequency is not. Deeper laser ablation results in improved ICG delivery. These findings may be relevant for the delivery of other drugs with comparable molecular properties.

Hybrid Fractional Ablative and Nonablative Laser Resurfacing of Actinic Keratoses
摘要: Field treatment of actinic keratoses (AKs) is a therapeutic challenge due to lack of patient tolerability and compliance with traditional treatments. Actinic keratoses are a known risk factor for development of squamous cell carcinoma. Actinic keratoses may be treated with cryosurgery, topical immunomodulators, topical chemotherapy, electrodessication and curettage, and photodynamic therapy. Both cryosurgery and curettage can leave unsightly scars or permanent dyspigmentation. Topical immunomodulators and topical chemotherapy require patient compliance. Photodynamic therapy can be painful and requires strict sunlight avoidance. Laser resurfacing is emerging as a promising therapeutic option for AKs with low recurrence rate after treatment.1–4 We present a single treatment with a hybrid 1,470-nm diode/2,940-nm Er:YAG fractional laser (Halo; Sciton, Palo Alto, CA) as a novel method to treat AKs off the face.

Nonablative fractional laser therapy for treatment of actinic keratosis with 3a??months followa??up
摘要: Background: Actinic keratosis (AK) is a common skin disorder that is treated with different treatment modalities. Aims: The aim of this study was to assess the therapeutic effect of a 1540-nm non- ablative fractional laser on the AK. Methods: Ten patients with 31 AK were included in the study according to the inclu- sion and exclusion criteria. Treatment with nonablative fractional laser (1540 nm) 3 times at 4 weeks intervals in a noncontact mode was applied. Clinical severity was as- sessed at each session and 3 months after the last session. It is pertinent to mention that the patients were also involved in the assessment of improvement. Results: The results of present study showed that in comparison to the baseline, 3-session AK therapy with nonablative fractional laser significantly not only im- proved the AK severity by 79%, but also reduced the AK counts (from 31 to 17). On the other hand, the patients consented to the therapeutic effect. Conclusion: According to the obtained results and available studies, it seems that therapy with a 1540-nm nonablative fractional laser could be considered as a safe and effective alternative therapeutic option for the treatment of AK.
