    摘要: The study purpose is to construct a reflective Oxygen Saturation (SpO2) and Heart Rate (HR) four-limb detective device, improve transmission SpO2 device only to measure finger, toe and earlobe. Using Silicon Labs Si1143, Arduino Nano, self-calibrated SpO2 algorithm and mobile application software constitute a mobile reflective SpO2 and HR detective device. Static four parts and post-exercise experiment had been carried out to test our device, the result can achieve comparable performance to a clinical device with no significant difference revealed by paired t-tests. This study has applied for clinical trial permission, the schedule will improve the calibration curve and compensation factor of unverifiable body parts by blood draw comparison. Provide clinical staff for immediate local SpO2 and HR information, evaluate the effectiveness of vascular suture and hyperbaric oxygen therapy.

  • Oxygen delivery therapy with Epiflo reduceswound hyperperfusionin patient with chronic leg ulcers: a laser speckle contrast analysis.

    摘要: BACKGROUND: Transdermal continuous oxygen therapy (TCOT)is today employed in wound careto promote healing by improving local hypoxia and preventing infection, and it has been described to reduce local inflammation over 1 month of administration. The present study aims to investigate the effects of this treatment on wound microcirculation through laser speckle contrast analysis. (LASCA) METHODS: 20 adult patients (mean age: 76 ± 11.5) were prospectively enrolled. Inclusion criteria were: presence of venous or mixed lower limb ulcers from three or more months without dimension reduction, without indication to surgery, weekly treated by our outpatient clinic with silver dressings. Subjects underwent 1 month of TCOT (Epiflo?)in addition to foam dressing. The primary endpoint was the comparisonof ulcer and healthy skin perfusion through LASCA, performed before and after the treatment period. Secondary considered endpoints were wound area, WASI and PUSH Tools 3.0 ulcer severity scales and pain assessment (NRS). RESULTS:Before treatment,the wound area was significantly more perfused than healthy skin(+45%p=.005), at the end of the study this difference was not significant anymore (+20,5% p=.11). Ulcer perfusion decreased, (- 12.5%, p .047), while healthy skin perfusion did not vary significantly. A reduction of the wound dimension (median difference: 2cm , p=.009). and pain (median difference:2 NRS point, p<.001) after therapy were assessed. CONCLUSION: Laser speckle contrast analysis shows that 1 month of transdermal continuous oxygen therapy can help to reduce hyperperfusion of ulcer bed in patients with chronic lower limb ulcers, strengthening the hypothesis that this treatment effectively contrast inflammation. This could correlate with the area and pain reduction assessed, however the absence of control group in this study does not allow a generalization of thishypothesis. Larger, controlled trials are needed to properly assess the relationshipbetween TCOT effects on wound microenvironment and effective healing process.

  • Solar powered oxygen systems in remote health centers in Papua New Guinea: a large scale implementation effectiveness trial

    摘要: Background Pneumonia is the largest cause of child deaths in Papua New Guinea (PNG), and hypoxaemia is the major complication causing death in childhood pneumonia, and hypoxaemia is a major factor in deaths from many other common conditions, including bronchiolitis, asthma, sepsis, malaria, trauma, perinatal problems, and obstetric emergencies. A reliable source of oxygen therapy can reduce mortality from pneumonia by up to 35%. However, in low and middle income countries throughout the world, improved oxygen systems have not been implemented at large scale in remote, difficult to access health care settings, and oxygen is often unavailable at smaller rural hospitals or district health centers which serve as the first point of referral for childhood illnesses. These hospitals are hampered by lack of reliable power, staff training and other basic services. Methods We report the methodology of a large implementation effectiveness trial involving sustainable and renewable oxygen and power systems in 36 health facilities in remote rural areas of PNG. The methodology is a before–and after evaluation involving continuous quality improvement, and a health systems approach. We describe this model of implementation as the considerations and steps involved have wider implications in health systems in other countries. Results The implementation steps include: defining the criteria for where such an intervention is appropriate, assessment of power supplies and power requirements, the optimal design of a solar power system, specifications for oxygen concentrators and other oxygen equipment that will function in remote environments, installation logistics in remote settings, the role of oxygen analyzers in monitoring oxygen concentrator performance, the engineering capacity required to sustain a program at scale, clinical guidelines and training on oxygen equipment and the treatment of children with severe respiratory infection and other critical illnesses, program costs, and measurement of processes and outcomes to support continuous quality improvement. Conclusions This study will evaluate the feasibility and sustainability issues in improving oxygen systems and providing reliable power on a large scale in remote rural settings in PNG, and the impact of this on child mortality from pneumonia over 3 years post–intervention. Taking a continuous quality improvement approach can be transformational for remote health services.
