  • Twelve commandments of radiology: guidance on the use of imaging in clinical practice

    摘要: The requesting of radiological investigations is frequently devolved to the ward-based doctors in training. In a National Health Service system that is regularly over capacity, radiological investigations are an expensive commodity often over-used or poorly targeted by doctors. By considering the following ‘Commandments of Radiology’; doctors could target imaging more appropriately, resulting in higher quality information and hence, better patent care and value for money.

  • Knowledge and practice of nursing personnnel regarding the care of neonates under phototherapy at Paropakar Shree Panch Indra Rajyalaxmi Devi Prasuti Griha, Thapathali, Kathmandu

    摘要: Background: High neonatal mortality is a re? ection of very poor neonatal care. Hence a specialist should be able to take up leadership role in neonatal care to prevent high mortality and morbidity rate. The study was carried out to determine the knowledge and practice of the nursing personnel regarding the care of neonates with phototherapy. Objectives: To assess the knowledge and skills/practice of nurses regarding the care of neonates with phototherapy. Methods: Descriptive and exploratory research design was adopted for the study from 15th to 30th of April, 2007 for two weeks at Paropakar Shree Panch Indra Rajyalaxmi Devi Prasuti Griha, Thapathali, Kathmandu. A semistructured questionnaire was designed to collect the data from 50 respondents working in different wards and they were selected by using Purposive sampling technique. The data collected were analysed and interpreted based on descriptive and inferential statistics. The level of knowledge score was converted into percentage and overall adequacy of knowledge was graded according to the following criteria:- If score > 75% highly knowledgeable If score 50 % to 75% moderate knowledge If score <50% inadequate knowledge1. Results: The ? ndings revealed that the majority of the respondents 28 (56%) were highly knowledgeable (the knowledge level of the respondents more than 75 %) and 22 (44%) respondents were with average knowledge (more than 50% and less than75%). No respondents were below 50% level (below 50%). Conclusion: The study attempts to determine the knowledge and practice of nursing staffs regarding the care of neonates with phototherapy. According to the data and information of the respondents the nursing personnel should be provided with in-service education training in relation to care of neonates with phototherapy. This study will be helpful for providing information about existing knowledge and practices of nursing personnel regarding phototherapy. It will help to provide knowledge about the strength and weakness of the services provided by the hospital regarding phototherapy for future researcher.

    摘要: Embedded software is at the core of current and future telecommunication, automotive, multimedia, and industrial automation systems. The success of practically any industrial application depends on the embedded software system’s dependability, and one method to verify the dependability of a system is testing its robustness. The motivation behind this paper is to provide a knowledge base of the state of the practice in robustness testing of embedded software systems and to compare this to the state of the art. We have gathered the information on the state of the practice in robustness testing from seven different industrial domains (telecommunication, automotive, multimedia, critical infrastructure, aerospace, consumer products, and banking) by conducting 13 semi-structured interviews. We investigate the different aspects of robustness testing, such as the general view of robustness, relation to requirements engineering and design, test execution, failures, and tools. We highlight knowledge from the state of the practice of robustness testing of embedded software systems. We found different robustness testing practices that have not been previously described. This paper shows that the state of the practice, when it comes to robustness testing, differs between organizations and is quite different from the state of the art described in the scienti?c literature. For example, methods commonly described in the literature (e.g., the fuzzy approach) are not used in the organizations we studied. Instead, the interviewees described several ad hoc approaches that take speci?c scenarios into account (e.g., power failure or overload). Other differences we found concern the classi?cation of robustness failures, the hypothesized root causes of robustness failures, and the types of tools used for robustness testing. This paper is a ?rst step in capturing the state of the practice of robustness testing of embedded software systems. The results can be used by both researchers and practitioners. Researchers can use our ?ndings to understand the gap between the state of the art and the state of the practice and develop their studies to ?ll this gap. Practitioners can also learn from this knowledge base regarding how they can improve their practice and acquire other practices.

  • A relational approach to understanding inhabitants’ engagement with Photovoltaic (PV) technology in homes

    摘要: Photovoltaic (PV) systems have been promoted in the UK housing sector as a key strategy for meeting carbon reduction commitments by offsetting the use of the non-renewable grid energy with renewable energy. However, inhabitants are not changing their routine energy consumption practices to take advantage of off-grid day time solar energy and, in some cases, even shifting practices away from the initial intentions underlying the technology. This means that the necessary energy savings from new housing are not being achieved. In this paper, this is attributed to the variation in the provisioning of PV technology in new homes, as well as inhabitants’ engagement with and know-how of PV technologies, subject to explicit rules and policies. The key contribution of this paper is to reveal how PV technologies and inhabitants interact within different socio-technological home contexts drawing on Practice theory and ethnographic methods applied to four housing developments in England.

  • Daylight-Mediated Photodynamic Therapy for Actinic Keratosis

    摘要: Actinic (solar) keratoses are premalignant lesions caused by prolonged exposure to ultraviolet radiation, with the potential to progress into squamous cell carcinoma. Hence, use of appropriate therapies is critical for effective treatment. Daylight-mediated photodynamic therapy with methyl aminolevulinate cream is a convenient new option for the safe and effective treatment of actinic (solar) keratosis lesions, offering similar efficacy to conventional photodynamic therapy, nearly painless treatment, and reduced in-clinic times. Use of appropriate therapies and optimization of clinical practice are critical for safe and effective treatment. Nurses play a key role in ensuring best clinical practice and effective treatment. This article explains the practicalities and principles of daylight-mediated photodynamic therapy to ensure best practice by treating nurses.
