  • Novel Use of Nona??Invasive Devices and Microbiopsies to Assess Facial Skin Rejuvenation Following Laser Treatment

    摘要: Patient‐reported outcomes and blinded observer rating scales are subjective and unreliable but are extensively utilized to evaluate cosmetic office‐based treatments, including ablative lasers. This study aims to assess the practicality of using non‐invasive and minimally invasive methods for objective skin assessment following a skin rejuvenation treatment. Twelve patients received a single 1,470/2,940 nm laser treatment for facial rejuvenation. Assessments were performed before treatment, and 7 days, 3 weeks, and 3 months post‐treatment. Images were taken with the VISIA Skin Analysis System to measure wrinkles, textures, pores, ultraviolet (UV) spots, brown spots, red areas, and porphyrins. Other non‐invasive skin measurements—high‐resolution ultrasonography, optical coherence tomography, transepidermal water loss and BTC 2000—were used to measure epidermal/dermal thickness, blood flow, surface roughness, wrinkle depth, attenuation coefficient, elasticity, viscoelasticity. Microbiopsies (0.33 mm in diameter or the equivalent of a 23‐gauge needle) were collected for histology and gene expression of tissue rejuvenation. Significant improvement in facial skin aesthetics after laser treatment was recorded in UV spots, brown spots and pores after 3 weeks and in UV spots and brown spots after 3 months. The dermal attenuation coefficient decreased significantly at 3 weeks, while blood flow 0.5 to 0.7 mm below the skin surface increased significantly between 5 days and 3 weeks following treatment. Epidermal hyaluronic acid expression assessed by immunostaining and expression of inflammatory genes were elevated at 7 days post‐treatment compared with untreated or 3 months post‐treatment. There were no statistically significant changes in collagen or elastin‐related genes between groups at the studied parameters. Non‐invasive devices can be effectively used to provide objective measurements of skin structure, pigmentation, blood flow, and elasticity to assess the efficacy of facial skin rejuvenation treatments. Furthermore, microbiopsies can objectively evaluate facial skin rejuvenation without scarring. Using non‐invasive skin imaging, a single treatment with the 1,470/2,940 nm laser was observed to be effective in improving skin appearance after 3 months, namely in reducing UV spots and brown spots, without significant changes in the tissue at the molecular level, as assessed by microbiopsy.

  • Face and neck rejuvenation using an improved non-ablative fractional high power 1064-nm Q-switched Nd:YAG Laser: clinical results in 16 women

    摘要: Objective: To evaluate the safety and efficacy of a specific treatment protocol using a new and improved non-ablative fractional high-power 1064-nm Q-switched Nd:YAG laser for face and neck rejuvenation. Methods: Sixteen women, aged 30 to 60 years old, were selected to undergo three consecutive treatment sessions with this new laser at maximum energy (2,400 mJ/pulse, Clear Lift laser?-Harmony XL-Pro, Alma Lasers Ltd.). Face and neck were treated in eight patients, respectively. Each treatment used the same protocol. The efficacy was evaluated by the therapist (TS), the patient (PS), and two separate independent experts who were blind to the study (E1 and E2). Results: According to E1 and E2, the mean reduction in signs of skin aging on a Global Esthetic Improvement Scale was 30-40%. Using a 0–10 points scale, TS and PS mean (range) satisfaction rates were 9.0 (8–10); 9.2 (6–10) for the face and 8.7 (8–10); 8.0 (3–10), for the neck, respectively. The procedure was practically painless, no significant adverse effects were observed, and the patients returned to their daily and work activities without downtime. Conclusion: This laser was safe and effective for face and neck minimally invasive rejuvenation, with excellent results using our usual routine use conditions.

  • Fractional 1,927?nm Thulium Laser Plus Photodynamic Therapy Compared and Combined for Photodamaged Décolleté Skin: A Side‐by‐Side Randomized Controlled Trial

    摘要: Background and Objectives: Décolleté photodamage is a common condition typically treated with light and energy‐based devices. This study investigated the efficacy and safety of a fractional 1,927 nm thulium laser (TL) alone and combined with photodynamic therapy (PDT). Study Design/Materials and Methods: In a 12‐week follow‐up study, participant décolletés were divided into four treatment areas and randomized to receive a single treatment with field‐directed TL, PDT, combination TL‐PDT, or lesion‐directed curettage control. All actinic keratoses (AKs) underwent lesion‐directed curettage before randomization. TL was delivered at 20 mJ/mb, 500 mJ/cm2 fluence, 5 W, and 8 (n = 6 pts.) or 16 (n = 6 pts.) passes. PDT was performed with 16% methyl aminolevulinate (MAL) creme incubated for 3 h, followed by red light‐emitting diode light at 37 J/cm2. Outcome measures included clinical assessment of overall photodamage and specific subcomponents, assisted by optical coherence tomography (OCT) imaging. Results: Twelve women with moderate to severe photodamage on the décolleté and a cumulative total of 184 thin grade I AKs were included. Field‐directed treatments TL and combination TL‐PDT equally improved the overall photodamage, mottled pigmentation, and rhytides compared with lesion‐directed control (P < 0.05). The skin texture improved by TL alone and was further improved by combining TL and PDT (P < 0.05). Median AK complete responses were similar for field‐directed interventions TL‐PDT (100%), TL (90%), PDT (82%), and lesion‐directed curettage control (52%) (P = 0.464). Patients presented with mild local skin responses, slightly more pronounced when combining TL with PDT versus individual treatments (P < 0.05). No scarring or adverse events were observed. Conclusions: The 1,927 nm fractional thulium laser is an effective, tolerable, and safe field‐directed treatment for décolleté photodamage. Provided alone, TL proved to be as effective as combined TL‐PDT for overall photodamage, while a greater improvement in skin texture was achieved using TL and PDT in combination.

  • Topical Nanofat Biocrème Improves Aesthetic Outcomes of Nonablative Fractionated Laser Treatment: A Preliminary Report

    摘要: Background: Improvements in skin erythema and elasticity have been seen with topical application of platelet-rich plasma after fractional laser treatment. Nanofat, when injected in small needles into the dermis has shown improvements in tissue thickness, discoloration and wrinkle depth. Objective: To evaluate improvements in skin following a nonablative fractional laser (FXD) treatment combined with the application of a novel topical nanofat biocrème, called neo-U. Methods: Fifty patients were treated with a nonablative FXD followed by application of a topical nanofat biocrème. Harvested fat was processed into nanofat, which was compounded with a transdermal liposomal delivery vector to produce a topical biocrème. In two patients, post-auricular skin punch biopsies were performed pre/post-treatment and examined for histological changes. Photos of a historical group treated with only the FXD laser were compared to photos of the patients treated with topical nanofat biocrème and a FXD laser. Results: Findings from postauricular skin biopsies show the skin exposed to FXD with nanofat biocrème had more elastin fibers and a slight increase in the thickness of the epidermis. Patients having FXD plus nanofat biocrème had a statistically significant improvement in the degree of wrinkles, nasolabial fold depth and texture compared to historical controls. Conclusions: Transdermal delivery of nanofat topical biocrème applied after fractional laser treatment can serve as a delivery system to improve fine lines, depth of nasolabial folds and overall texture of the tissue to a greater degree than laser resurfacing alone.

  • Reflections on the Er:YAG Laser Used in “Nonthermal Mode” to Rejuvenate Skin

    摘要: CO2 and Er:YAG lasers are both actively used for laser skin rejuvenation, both ablative and microablative. Despite seeming similarities in efficiency of their radiation’s absorption by soft tissues, these lasers may have completely different effects on skin: tissue vaporization with CO2 laser radiation always assumes existence of areas with irreversible thermal effects such as coagulation, dehydration, carbonization, and vaporization. If Er:YAG and vaporization is used for the same purpose, same thermal damage areas may be obtained but smaller in size. However, in case of sufficiently high energy density and short duration of pulses, an ablation mode may be implemented, in the case in which areas of irreversible thermal changes are contained within vaporization zone. Thus, underlying layers of irradiated tissue are not thermally damaged.

  • Improved cosmeceutical phototherapy using microneedle

    摘要: Skin rejuvenation is the most popular topic in cosmetic industry. Recently, skin phototherapy is spotlighted among a lot of skin therapies by its advantageous effects and ease of use. Phototherapy has been widely used for cosmeceutical purposes such as anti-aging or anti-bacterial effects. By the optical properties of light, however, the visible lights cannot reach deep inside the skin layer by re?ection and scattering at the outermost boundary of the skin, the stratum corneum and epidermis. This paper describes new method to improve the skin phototherapy using microneedles. We used microneedle as the light transporter like optic ?ber to transport light inside the skin by total internal re?ection. The total internal re?ection inside microneedles was geometrically analyzed and was measured microscopically. The light transmittance with or without a microneedle array was measured using an optical microscope system. Three different ranges of lights (red, green, blue) were exposed on the backside of the microneedle and the light pass through the microneedles. As the results, using microneedles, light transmissivity was dramatically improved even the shorter wavelength of the light (Blue light). Collaboration of phototherapy and microneedle will be a powerful cosmetic and medical tool for skin therapy.
