Haab striae: Optical coherence tomographic analysis
摘要: Purpose. — To study Haab striae in adults with primary congenital glaucoma and describe them by anterior segment optical coherence tomography, as well as analyze their evolution over the patient’s lifetime. Material and methods. — Three patients with primary congenital glaucoma were recruited, aged between 17 and 48 years old, with incidentally noted Haab striae on glaucoma consultation. They were studied by: slit lamp; anterior segment optical coherence tomography, Heidelberg Engineering Inc., Germany, (with anterior segment module) and Cirrus HD-OCT, Carl Zeiss Meditec, Dublin, California, USA (with 3D reconstruction); and specular biomicroscopy. The results obtained were contrasted with other studies performed in children. Results. — Rounded excrescences protruding towards the anterior chamber were found, as well as dense, irregular tissue between them. In addition, endothelial abnormalities (polymegathism and polymorphism) were found. These findings fit the evolution of those obtained in pediatric studies and remain stable in adult life. Conclusions. — Through noninvasive methods such as optical coherence tomography and specular biomicroscopy, it is possible to make the diagnosis and follow up these patients. The findings suggest posterior corneal topographic changes; additional studies on this subject may be necessary.

Flap Sliding Technique for Managing Flap Striae following Laser In Situ Keratomileusis
摘要: Purpose. To assess the e?cacy and safety of a simple, noninvasive, “?ap-sliding” technique for managing ?ap striae following laser in situ keratomileusis (LASIK). Methods. This prospective, interventional study included eyes with post-LASIK ?ap striae. All eyes underwent ?ap sliding 1-2 days after surgery. Following ?ap edge epithelialisation, a cellulose sponge was used to gently slide the ?ap perpendicular to the striae direction. This technique allows for ?ap striae treatment without ?ap lifting, avoiding any associated lifting complications. Uncorrected distance visual acuity (UDVA), corrected distance visual acuity (CDVA), and refractive error were monitored one day after the ?ap-sliding procedure. Results. Fifteen eyes (15 patients) with post-LASIK ?ap striae were managed using the ?ap-sliding technique. The procedure did not successfully relocate the ?ap striae in 1 eye, and ?ap elevation and ?oating (using a balanced salt solution) were required. Therefore, 14 eyes were included in post-?ap-sliding analyses. The UDVA improved in all patients the ?rst day after the ?ap-sliding procedure was performed, with 11 of 14 eyes (78.57%) reaching an UDVA of 20/25 or better. Complications following ?ap sliding occurred in 2 eyes (14.29%). One eye had intra- operative epithelial abrasion, and 1 eye had residual postoperative striae outside of the optical zone. Conclusion. The ?ap-sliding technique is a simple, noninvasive, e?cient, and safe technique for managing post-LASIK ?ap striae that develop after epithelial healing in the early post-LASIK period. In vivo slit scanning confocal microscopic observation in a patient with moderate and severe keratoconus: a case report
摘要: A 22-year-old Indian female was referred to Sg Buloh hospital with the diagnosis of bilateral keratoconus. On examination, slit lamp biomicroscopy and corneal topography revealed stage 3 keratoconus in the right eye and stage 2 keratoconus in the left eye. Corneal cell morphology in both eyes was evaluated using confocal microscope. In qualitative observation, almost all corneal layers in right eye except endothelium were partially or completely obscured by haze. Additionally, morphological alterations, such as elongation of keratocyte nuclei and cluster of cells, and dark bands in the anterior stroma were observed in right eye. In the left eye, the amount of haze was less, allowing better visibility of the corneal layers compared with the right eye. The dark bands were evident in the posterior stroma. Quantitative analysis showed that anterior and posterior stromal keratocyte density and endothelium cell density were relatively low in the right eye (834.0, 700.5, and 2,133 cells/mm2, respectively) compared with the left eye (934.1, 750.6, and 2,361 cells/mm2, respectively). In this case, the right eye, exhibiting stage 3 keratoconus, showed more morphological alteration, particularly in the anterior stroma compared with the left eye with stage 2 keratoconus. Increased severity of the disease can explain these differences in corneal cell morphology.

Use of 3-Dimensional Imaging in Submental Fat Reduction Following Hyperthermic Laser Lipolysis
摘要: In a randomized controlled study, patients with a wide variety of hypopigmented scars, and striae alba had mean final pigment correction rates of approximately 100% by colorimetric analysis after 9 treatments of 308-nm excimer laser biweekly. However, maintenance treatment every 1 to 4 months is suggested because the pigment gradually declined toward baseline values during a 6-month follow-up. Although the authors performed 308-nm excimer light only once weekly for 6 times in our patient, she had a good response and rapid repigmentation within 3 months, and no recurrence of hypopigmentation was found at a 10-month follow-up. A possible hypothesis is that she had no genetic predisposition for vitiligo, and she had a darker Fitzpatrick skin type. Patients with Fitzpatrick skin Type III and above generally respond better to an excimer laser than those with lower skin types. To the best of our knowledge, this is the first case of corticosteroid-induced perilymphatic linear hypopigmentation treated with 308-nm excimer light.
