Evaluating the Laser Output of the Micropulse P3 Laser Delivery Device Following Repeated Use
摘要: Repeated use of the Micropulse P3 (MP3) probe during micropulse transscleral cyclophotocoagulation is associated with an increase in laser output over time. The purpose of this study was to examine the laser efficacy of the MP3 probe following repeated use. This was an observational study carried out using Cyclo G6 Glaucoma Laser System with 6 MP3 laser delivery probes. Each probe was fired for 100 seconds, every 10 minutes until the probe was deactivated. The laser output was measured using a laser power meter. Maximum observed laser power output was also noted. All probes were deactivated after 90 minutes (9 cycles) of use. Mean laser output of all 6 probes was determined, and results suggested an increase in output with time. When examining the differences in total laser output for each cycle, no significant differences were observed for the first 4 cycles, but not for the remaining 5 cycles, wherein the increased laser outputs were found to be significantly different from baseline (cycle 1). Findings suggest a possible increase in laser output with repeated use, especially after 4 cycles, and clinicians need to be cautious if they intend to use the probes repeatedly.

Subthreshold micropulse laser photocoagulation therapy in a case of bilateral retinal astrocytic hamartomas with tuberous sclerosis complex
摘要: Rationale: Report a case of bilateral multiple retinal hamartomas (RAHs) in a patient with tuberous sclerosis complex (TSC) and introduced a new method (subthreshold micropulse laser photocoagulation) for the treatment of RAHs. Patient concerns: A 20-year-old man with TSC complained of decreased vision and metamorphosia in both eyes for 2 months. At presentation, visual acuity (VA) was 20/32 in the right eye and 20/40 in the left eye. Fundus photographs, optical coherence tomography, fundus ?uorescein angiography (FFA), and indocyanine green angiography indicated multiple RAHs in both eyes. Diagnoses: Bilateral retinal astrocytic hamartomas. Interventions: In the right eye, 577 nm photocoagulation was adopted to treat the RAHs with obvious ?uorescein leakage in FFA. The paramacular RAHs were treated by subthreshold micropulse mode to minimize the damage to macula. Photocoagulation therapy was administrated in the left eye after 1 dose of intravitreal ranibizumab treatment. Outcomes: After photocoagulation therapy (including subthreshold micropulse laser photocoagulation for the paramacular RAHs in both eyes), the VA improved to 20/25 OD and 20/32 OS with no recurrence of exudation. Lessons: About 577 nm photocoagulation for photocoagulation for RAHs in the macular zone is a good option for multiple RAHs in patients with TSC.

Effect of a single session of micropulse laser trabeculoplasty on corneal endothelial parameters
摘要: Background: To evaluate the effect of a single session of micropulse laser trabeculoplasty on the cornea in eyes with primary open-angle glaucoma and pseudoexfoliation glaucoma. Methods: This single-centre, prospective, case series enrolled patients with primary open-angle and pseudoexfoliation glaucoma under treatment with glaucoma agents that required additional intraocular pressure reduction. Eyes underwent 360 degrees treatment with 532 nm micropulse laser trabeculoplasty. Central corneal thickness, endothelial cell count, hexagonal cell ratio and co-ef?cient of variation of endothelial cells were measured before micropulse laser trabeculoplasty and at one, three and six months after treatment. Results: Twenty eyes of 20 patients (mean age 67.6 (cid:1) 8.0 years) with primary open-angle glaucoma and 18 eyes of 18 patients (mean age 71.44 (cid:1) 6.43 years) with pseudoexfoliation glaucoma were included in the study. Treatment with micropulse laser trabeculoplasty resulted in statistically signi?cantly lower intraocular pressure compared to baseline in both primary open-angle and pseudoexfoliation glaucoma eyes (p < 0.0001 at both comparisons). Central corneal thickness, endothelial cell count, hexagonal cell ratio and co-ef?cient of variation of the endothelial cell size showed no signi?cant change between baseline and six months after micropulse laser trabeculoplasty in both primary open-angle and pseudoexfoliation glaucoma eyes. Conclusion: A single session of micropulse laser trabeculoplasty did not affect central corneal thickness and corneal endothelium parameters in eyes with primary open-angle and pseudoexfoliation glaucoma.

Transfoveal Micropulse Laser Treatment of Central Serous Chorioretinopathy within Six Months of Disease Onset
摘要: Background: It has been recommended that any invasive treatment performed in patients with central serous chorioretinopathy (CSCR) not be initiated earlier than four months after disease onset due to the potential for spontaneous remission of symptoms. The goal of this study was to examine the outcome of transfoveal subthreshold micropulse laser treatment (SMPLT) of CSCR performed at six months or less after disease onset. Materials and methods: The study included 32 cases of CSCR lasting between three weeks and six months (mean: 3.4 ± 2.3 months). All patients had transfoveal SMPLT applied and were followed for at least three months after each session of SMPLT. Two sessions of SMPLT in total were planned in case of an insu?cient response to the ?rst instance of treatment. Evaluation parameters included any change in best-corrected visual acuity (BCVA) and retinal morphology. Results: Total resolution of subretinal ?uid (SRF) was noted in 26 cases (81.25%). Final BCVA improved signi?cantly from 0.37 ± 0.22 logMAR to 0.22 ± 0.20 logMAR after treatment. Overall, early SMPLT correlated with better ?nal BCVA (p = 0.0005, Spearman rank correlation). For eyes achieving a total resolution of SRF, BCVA improved from 0.33 ± 0.21 logMAR to 0.17 ± 0.14 logMAR (p = 0.004, Spearman rank correlation). The analysis of SMPLT nonresponders revealed a tendency for poorer baseline visual acuity. Conclusions: Patients with CSCR lasting six months or less treated with transfoveal SMPLT achieve better functional results with early application of this procedure. As baseline BCVA predicts ?nal visual acuity, earlier treatment, permitted by the safety of SMPLT, may improve ?nal visual outcomes.

Efficacy and safety of subthreshold micropulse laser compared with threshold conventional laser in central serous chorioretinopathy
摘要: Purpose To compare the efficacy and safety of subthreshold micropulse laser (SML) with threshold conventional laser (TCL) in central serous chorioretinopathy (CSC). Methods Prospective, randomized, double-masked, non-inferiority, 12-week clinical trial. Patients were randomly assigned 1:1 to SML group or TCL group. Patients in the SML group were treated with 577 nm micropulse laser. The spot size was 160 μm, the duty cycle was 5% and exposure time was 0.2 s. The power was 50% threshold tested. Patients in the TCL group were treated with 577 nm continuous laser. The power was 100% threshold tested. The primary outcome was the mean change in best-corrected visual acuity (BCVA) at week 12, with a non-inferiority limit of five letters on the Early Treatment Diabetic Retinopathy Study (ETDRS) visual acuity charts. Results Eighty-eight patients were enroled. Seventy-seven patients were male. Forty-four patients were in SML group and 44 in TCL group. At week 12, SML was equivalent to TCL with a gain of 6.23 ± 8.59 and 6.61 ± 6.35 letters, respectively, (SML–TCL difference: ?0.38 letters; 95% confidence interval (CI):?3.58–2.81; Pnon-inferiority = 0.0026). There was no statistically significant difference between the two groups (t = 0.240, P = 0.811). At week 12, the proportion of patients whose SRF had been totally absorbed was 63.63 and 81.82% respectively for SML and TCL groups. There was no statistically significant difference between the two groups (χ2 = 3.67, P = 0.056). Conclusions Both SML and TCL can improve visual acuity in CSC. SML was non-inferior to TCL in the improvement of BCVA.

Outcomes of Micropulse Transscleral Cyclophotocoagulation in Uncontrolled Glaucoma Patients
摘要: This study aimed at evaluating the intermediate-term efficacy and safety of micropulse transscleral cyclophotocoagulation (MP-TSCPC) in cases of uncontrolled glaucoma. Patients with moderate to advanced glaucoma and uncontrolled intraocular pressure (IOP) despite maximally tolerable antiglaucoma medications were selected to undergo MP-TSCPC using the MP3 handpiece with the Iridex Cyclo G6 (IRIDEX Laser Systems). Follow-up examinations took place on a regular basis until 15 months postoperatively. Seventy-five eyes of 69 patients (53.6% male patients) were included. Mean age was 55.5 ± 22.9 years. Primary open-angle glaucoma was the most common diagnosis. Corrected distance visual acuity at baseline ranged between 0 and 2.1 logMAR. Mean prelaser IOP was 26.0 ± 7.91 mm Hg. This was reduced significantly to 13.8 ± 5.6 mm Hg (44.0% reduction, P < 0.001) at week 1, and to 18.0 ± 7.7, 18.4 ± 7.1, 16.7 ± 6.2, 15.1 ± 4.1, 15.7 ± 5.32, and 14.8 ± 5.50 mm Hg at months 1, 3, 6, 9, 12, and 15, respectively. The mean number of antiglaucoma drops decreased significantly up to 12 months of follow-up (P = 0.008) and that of oral acetazolamide tablets decreased significantly up to 15 months (P < 0.001). The success rate decreased progressively with time, reaching 81.4% at 6 months and 73.3% at 1 year. No major postoperative complications were encountered, and no eye lost vision completely. MP-TSCPC is an efficient noninvasive glaucoma treatment that achieves sustained IOP reduction and reduced need for ocular antihypertensive medications for up to 15 months. The optimal laser parameters to achieve the best success rate with the least side effects still need to be determined.

Evaluation of Chronic Central Serous Chorioretinopathy after Subthreshold Yellow Micropulse Diode Laser Photostimulation at a Wavelength of 577 nm
摘要: Objective: This study evaluated the efficacy and safety of subthreshold yellow diode-laser micropulse 577 nm (SDM) in the treatment of chronic central serous chorioretinopathy (CSCR). Methods: 40 eyes of 40 patients with non-resolving CSCR of >12 months duration were treated with SDM yellow laser (577 nm). Best corrected visual acuity (BCVA) and intraocular pressure (IOP) were measured. Amsler grid screening, ophthalmoscopy, fundus autofluorescence (FAF) and SD-OCT were performed. Follow up measurements were recorded from 3 to 12 months. Results: Restoration of normal macular anatomy was obtained in 85.7% of cases with a significant reduction in mean central foveal thickness (CFT). Mean visual acuity gain was 14.95 ETDRS letters, subjective or objective deterioration after treatment was not recorded in any case. Conclusion: The possibility of favouring reabsorption of subretinal liquid by stimulating the retinal pigment epithelium, even at the fovea, without damaging retinal tissue, opens up new frontiers for the therapy of chronic CSCR.

Clinical Outcomes of Micropulsed Transcleral Cyclophotocoagulation in Moderate to Severe Glaucoma
摘要: Purpose: The purpose of this study is to evaluate the safety and effectiveness of a new micropulsed transcleral continuous wave diode laser in a clinical setting of patients diagnosed with open angle glaucoma, ranging from mild glaucoma to severe. Methods: Twenty-six eyes of thirteen patients were treated by a single surgeon at a single private practice center. Results were reviewed retrospectively. Each subject was treated with preset MP3 setting according to recommended treatment guidelines. Results: At baseline, patients were on an average of 3.3 topical glaucoma medications with an average IOP of 25.6 mmHg. At POD1 IOP was reduced by 20%, by 34% at POD7, 8% at POD28 and decreased 30% from baseline at the final postop visit 6-12 months after the procedure. Average medication use decreased from 3.3 to 1.8 from baseline to final post op visit. There were no serious adverse events. Conclusion: Micropulsed transcleral diode laser is a safe and effective alternative for glaucoma therapy for patients with open angle glaucoma. Patients can expect significant IOP lowering along with reduction in number of topical glaucoma medications required for IOP control.
