  • Enhancing cutaneous delivery with laser technology: almost there, but not yet

    摘要: Preclinical research has shown the potential of different laser-based strategies (direct ablation, photothermolysis and mechanical waves) to overcome the stratum corneum and facilitate the cutaneous delivery of drugs. However, specific protocols for the routine use of these strategies have not been stablished yet. The aim of this review has been to provide the readers with a view of the translational prospects of the different laser technologies with regards to their utility for enhancing the penetration of drugs.

  • A Randomized, Splita??Face, Doublea??Blind Comparison Trial Between Fractionated Frequencya??Doubled 1064/532a??nm Picosecond Nd:YAG Laser and Fractionated 1927a??nm Thulium Fiber Laser for Facial Photorejuvenation

    摘要: Fractionated lasers are a popular therapeutic option for facial photorejuvenation. In this study, we compare the safety, tolerability, and efficacy of a fractionated frequency‐doubled 1064/532 nm picosecond Nd:YAG fractionated picosecond laser (FPL) versus a fractionated 1927 nm thulium fiber laser (TFL) for facial rejuvenation. This was a double‐blind, randomized, split‐face comparison study involving 20 subjects. Facial halves were randomized to receive either FPL or TFL treatment. Three treatments were delivered at 1‐month intervals. Subjects were followed up for 1, 3, and 6 months post‐final treatment session and evaluated by blinded, non‐treating investigators for dyspigmentation, erythema, keratosis, texture, and rhytids on a standardized scale. Subjects also recorded a quantitative daily diary rating healing progress for 14 days after every treatment session. Statistically significant improvements in elastosis, erythema, keratosis, dyschromia, and skin texture were noted in both treatment groups. There were no significant differences detected in clinical efficacy between the two groups. Subject daily diaries revealed statistically significant differences in tolerability during the immediate 14‐day post‐operative recovery period. The facial half treated with FPL displayed significantly less redness on days 3 and 4; significantly less swelling on day 5; significantly less crusting on days 1 through 9; significantly less peeling on days 3 through 9; and significantly less itch on days 4 and 7. There were no unexpected adverse effects observed. Both FPL and TFL are safe and effective treatment options for facial rejuvenation. FPL may be associated with significantly less downtime.

    摘要: This review demonstrates the progress, current status and problems of photodynamic therapy and laser photothermolysis for various skin diseases and rejuvenation. These new minimally invasive technologies are detailed from the points of view of their advantages and disadvantages. Particular attention is paid to complications, associated with prolonged dermal phototoxicity after insufficient intravenous intradermal penetration of photosensibilisers after local application to skin. Finding a compromise between phototoxicity and insufficiency of local photosensibilisation we've developed a technical approach employed to overcome of limitations associated with targeted delivery of photosensibilisers into deep dermal layers. Intradermal delivery of photosensibilisers through so-called microthermal zones or dermal micropores, formed by focused pulse-periodic radiation of fiber laser, previously never used for photodynamic therapy of dermal disorders.

  • Harper's Textbook of Pediatric Dermatology || Laser Therapy

    摘要: The pulsed dye laser has revolutionized the treatment of vascular anomalies. More recently, the multiplex laser, a combination of pulsed dye and Nd:YAG, has been shown to be beneficial for recalcitrant port wine stains and other conditions. In addition to their use in treating port wine stains, lasers have also an important role in treating a number of childhood pigmentary disorders. These are primarily congenital naevi such as café‐au‐lait macules, naevus of Ito and some blue lesions. Laser treatment is based on selective thermal destruction or damage to a targeted chromophore located in the skin, either that of blood, melanin or tissue water. Lasers have been shown to successfully treat vascular and pigmented lesions and also remove unwanted hair. They can also be used to remove verrucous lesions such as angiofibromas and epidermal naevi through vaporization.

  • Fractional Ablative Laser Therapy is an Effective Treatment for Hypertrophic Burn Scars

    摘要: Objective: The aim of this study is to determine objective and subjective changes in mature hypertrophic burn scars treated with a fractional ablative carbon dioxide (CO2) laser. Background: Fractional CO2 laser treatment has been reported to improve burn scars, with increasing clinical use despite a paucity of controlled, prospective clinical studies using objective measures of improvement. Methods: A multicenter, site-controlled, prospective open-label study was conducted from 2013 to 2016. Objective and patient-reported outcome measures were documented at baseline, at each monthly laser treatment, and 6 months after treatment. Objective measurements employed were: mechanical skin torque to measure viscoelastic properties; ultrasonic imaging to measure scar thickness; and reflectometry to measure erythema and pigmentation. Subjective measures included health-related quality of life, patient and investigator scar assessment scales, and blinded scoring of before and after photographs. Subjects aged 11 years or older with hypertrophic burn scars were recruited. Each subject received 3 monthly treatment sessions with an ablative fractionated CO2 laser. Results: Twenty-nine subjects were enrolled, of whom 26 received at least 1 fractional CO2 laser treatment and 22 received 3 treatments. Mean age of those completing all 3 treatments was 28 years. Statistically significant objective improvements in elastic stretch (P < 0.01), elastic recovery (P < 0.01), extensibility (P < 0.01), and thickness (P < 0.01) were noted. Patient- and physician-reported scar appearance and pain/pruritus were significantly improved (P < 0.01). There was no regression of improvement for at least 6 months after treatment. Conclusions: Fractional ablative laser treatment provides significant, sustained improvement of elasticity, thickness, appearance, and symptoms of mature hypertrophic burn scars.

  • An insight into laser tattoo removal: reasoning, methods and limitations

    摘要: This article aims to review the possible reasons a patient may wish to remove their tattoo using lasers, as well as providing an overview of the tattooing process within the skin, where the tattoo ink particles lay within the skin’s structures and how lasers specifically target unwanted ink particles within the skin. An exploration into various tattoo removal methods, with a more in-depth focus on laser tattoo removal with its associated risks and limitations will also be addressed.
