  • Nanosecond Laser Cataract Surgery in LOCS III Grade 4 and 5: A Case Series

    摘要: Purpose: a major obstacle to the widespread implementation of nanosecond laser cataract removal has been its limited efficacy in cases when the cataract is harder than lens opacities classification system iii (locs iii) grade 3. the latest technological modifications seem to have overcome this obstacle. Design: a case series. Methods: this study presents a case series of 17 consecutive patients with a cataract classified as harder than locs iii grade 3 who underwent nanolaser cataract surgery. the basic surgical procedure included administration of topical or local anesthesia, capsulorhexis, hydrodissection, cataract removal with a nanosecond laser, cortical cleanup, intraocular lens implantation, and clear corneal incision closure with physiological solution. Results: the nanosecond laser system effectively removed the cataract in all patients. this result was statistically significant (P < 0.000001) compared with an expected conversion rate to ultrasound phacoemulsification of 100%. Conclusions: Harder cataracts should no longer be considered a limitation for the use of nanosecond laser in cataract surgery.

  • Novel Triamcinolone Acetonide-Loaded Liposomal Topical Formulation Improves Contrast Sensitivity Outcome After Femtosecond Laser-Assisted Cataract Surgery

    摘要: Purpose: To assess visual results, macular modi?cations, and the incidence of clinically signi?cant macular edema (CSME) in patients using a topical triamcinolone acetonide-loaded liposomal formulation (TA-LF) after femtosecond laser-assisted cataract surgery (FLACS). Methods: Fifty-six eyes after FLACS were selected. Twenty-eight eyes in the combined therapy group (P + N) were treated with prednisolone 1% and nepafenac 0.1% for 21 days postoperatively, whereas 28 eyes in the TA-LF group received a liposomal formulation containing 2 mg/mL of TA (0.2%) for the same period of time. Follow-up visits at 1 day, 6 weeks, and 12 weeks after surgery consisted of visual acuity, contrast sensitivity (CS), central foveal thickness (CFT), total macular volume (TMV) measurements, and the detection of CSME. Results: CS improved in the TA-LF group (basal value: 1.087 – 0.339 vs. 1.276 – 0.147 at week 12, P = 0.0346), whereas in the P + N group, CS was not different from the baseline (basal value: 1.130 – 0.331 vs. 1.274 – 0.133 at week 12, P = 0.1276). There were similar increases in postoperative CFT and TMV in both groups. CFT and TMV signi?cantly correlate with CS only in the TA-LF group. The r2 for CFT and CS was 0.1963 (P = 0.0206), whereas the r2 for TMV and CS was 0.3615 (P = 0.0007) at 12 weeks. No difference was observed in the incidence of CSME between the groups. Conclusion: TA-LF is associated with better CS outcomes compared to combined therapy after FLACS.

  • Levels of Trace Elements in the Aqueous Humor of Cataract Patients Measured by the Inductively Coupled Plasma Optical Emission Spectrometry

    摘要: Trace elements play an important role in the pathogenesis of several serious ophthalmological disorders, such as glaucoma, age-related macular degeneration (AMD), diabetic retinopathy, cataract, etc. This study aimed to measure alterations of chemical elements’ (67) levels in the aqueous humor of patients undergoing cataract surgery. The pilot study included 115 patients, (age 74 ± 7.27, female 64.35%, male 35.65%). The aqueous levels of elements were measured by the use of the inductively coupled plasma optical emission spectrometry (ICP-OES), quality controlled with certified standards. The classification of elements based on their concentration was achieved by hierarchical cluster analysis. This is the first screening study that quantifies over 60 elements which are present in the fluid from the anterior chamber of the eye of cataract patients. The obtained results can be suitable for understanding and identifying the causes that may play a role in the initiation and progression of lens opacity.

  • Comparison of Nd:YAG laser (532 nm green) vs diode laser (810 nm) photocoagulation in the treatment of retinopathy of prematurity: an evaluation in terms of complications

    摘要: Purpose To compare the anterior and posterior segment complications of diode (810 nm) laser photocoagulation (LPC) and Nd:YAG (532 nm green) LPC in the treatment of retinopathy of prematurity (ROP). Patients and methods The 84 eyes of 43 patients treated with diode LPC (group 1) and 58 eyes of 31 patients treated with Nd:YAG LPC (group 2) for ROP in our clinic were enrolled in the study. Medical records of all patients were investigated retrospectively. The patients in each group were examined in terms of birth weights, gestational weeks, stage of retinopathy, number of lasers pots, laser parameters, and anterior and posterior complications of LPC. Results The mean birth weeks of group 1 patients were postmenstrual 27.7 ± 2.5 (23–33), while the mean birth weights were 1006.0 ± 334.5 (540–1980) grams. The mean birth weeks of group 2 patients were postmenstrual 27.4 ± 2.6 (23–33), while the mean birth weights were 1073.8 ± 329.2 (480–1720) grams. The mean numbers of laser spots were 1036.0 ± 515.2 (430–2410) in group 1 per eye, while the mean numbers of laser spots were 1085.4 ± 526.0 (445–2530) in group 2 per eye (p ≥ 0.05). Additional laser application was performed in four eyes (4.8%) in group 1 and four eyes (6.9%) in group 2. Four eyes (4.8%) treated with diode LPC and one eye (1.7%) treated with Nd:YAG laser developed retinal detachment. Two eyes of a patient (3.4%) applied Nd:YAG LPC developed cataract. Conclusion Cataract may develop when Nd:YAG laser is used; however, posterior segment complications may be more likely to appear with the use of diode laser in these cases.

  • PSS1 POOLED RESULTS OF THE TECNIS SYMFONY EXTENDED DEPTH OF FOCUS INTRAOCULAR LENS

    摘要: Several prospective studies have indicated that the TECNIS Symfony, an extended depth of focus (EDOF) intraocular lens (IOL), achieves excellent visual outcomes at all distances after cataract surgery. The purpose of this study was to combine the visual outcomes from multiple recent studies of the TECNIS Symfony ZXR00. We performed a systematic literature review and subsequent data pooling of studies looking at corrected visual outcomes for patients within 6 months (range: 3-6 months) of receiving bilateral implantation of TECNIS Symfony IOLs following cataract surgery. The primary visual quality outcome of interest was accommodation measured by defocus curves. Data from monocular and binocular defocus curves were extracted and then pooled using a weighted average approach. Separate pooled curves were estimated for monocular and binocular data. We identified 25 articles of potential interest in the database review; 7 of these included TECNIS Symfony defocus curves. Of these, 3 provided monocular defocus curves (total eyes = 130), and 6 provided binocular defocus curves (total eyes = 310). The pooled defocus curves showed best corrected visual acuity generally remained high (LogMar # 0.2) from 1.0 diopters (D) to -2.0 D in patients who received TECNIS Symfony IOLs. For the pooled monocular defocus curve, the corrected distance visual acuity (0 D) was LogMar-0.04, the corrected intermediate visual acuity (66cm; -1.5 D) was LogMar0.13, and the corrected near visual acuity (40cm; -2.5 D) was LogMar0.40. For the pooled binocular defocus curve, the corrected distance visual acuity was LogMar-0.02, the corrected intermediate visual acuity was LogMar0.11, and the corrected near visual acuity was LogMar0.34. The TECNIS Symfony EDOF IOL provides successful visual restoration across all distances after cataract surgery, with a minimal level of disturbing photic phenomena and high levels of patient satisfaction.

  • Rapid anterior capsule contraction after femtosecond laser–assisted cataract surgery in a patient with retinitis pigmentosa

    摘要: We report a case of rapid and extreme capsule contraction after uneventful femtosecond laser–assisted cataract surgery (FLACS) in one eye performed subsequent to uneventful standard phacoemulsification with manual continuous curvilinear capsulorhexis (CCC) creation in the fellow eye. In the setting of retinitis pigmentosa, the rapid and extreme anterior capsule contraction in the FLACS eye compared with the manual CCC eye might have been the result of release of inflammatory mediators from cell death during the laser capsulotomy combined with a structurally weaker CCC edge.

  • Femtosecond laser-assisted cataract surgery in a spherophakic lens

    摘要: Rationale: Spherophakia is a rare diagnosis which is often associated with a shallow anterior chamber, angle-closure glaucoma, lens subluxation, and lenticular myopia. When cataracts occur with subluxation of the lens, vision is often markedly affected. This often presents surgeons with a unique challenge of maintaining good visual outcomes while minimizing potential complications. Patient concerns: A 48-year-old female was referred for ophthalmological assessment due to decreased vision in the left eye. In the left eye, the best-corrected visual acuity at distance was 20/125 with manifest refraction of (cid:1)6.5D + ((cid:1)0.75) D (cid:3) 118°. The slit lamp examination showed iridodonesis and a signi?cant nuclear cataract (C3N3) with tremor in the left eye. After pupil dilation, a subluxated lens, weak zonules, and “fake golden ring” within the lens was noted. Diagnosis: Due to the patient’s symptoms, examination results, she was diagnosed with cataract, subluxation of the lens and spherophakia in left eye. Interventions: The patient underwent an uneventful femtosecond laser-assisted cataract surgery (Alcon Fort Worth, TX). The laser was able to perform a circular free-?oating anterior capsulotomy and easy lens fragmentation. Outcomes: There were no postoperative complications. At 3 months postoperatively, the uncorrected visual acuity was 20/25, and the manifest refraction was (cid:1)0.25 D (cid:1) 0.75 D (cid:3) 148° with the corrected distance visual acuity of 20/16. Lessons: Femtosecond laser-assisted cataract surgery is an effective approach for cataract surgery in patients with subluxated and spherophakic lenses, with the bene?ts of causing minimal further zonular damage and easy lens fragmentation.

  • Femtosecond Laser-Assisted Cataract Surgery, Technology, Outcome, Future Directions and Modern Applications

    摘要: Since its introduction in 2009, femtosecond laser?assisted cataract surgery (FLACS) has promised to revolutionize cataract surgery. Despite its promise, the assessment of FLACS’s perceived benefits has proven to be far more complicated than initially might have been thought. Most studies to date have not provided validation of FLACS technology as a clinically significant advancement on our current techniques. We review FLACS technology and outcomes, including detailed analysis of safety, efficacy, cost?effectiveness, and future prospects using data from the literature and our own published clinical experience.

  • Comparison of visual performance of toric versus non-toric intraocular lenses with same material

    摘要: We compared the visual performance of toric intraocular lenses (IOLs) and non-toric IOLs made of the same material. The subjects included patients implanted with either Acrysof IQ? toric IOLs (SN6AT3-9) or Acrysof IQ? IOLs (SN60WF) bilaterally. The toric group included 103 patients who were implanted with Acrysof IQ toric IOLs bilaterally. The non-toric group was a corneal astigmatism-matched control group and included 103 patients who were implanted with Acrysof IQ IOLs bilaterally. Results: The uncorrected distance visual acuity was significantly better in the toric group, whereas the uncorrected 50 cm visual acuity was better in the non-toric group. There was no significant difference in contrast sensitivity (with and without glare) between both the groups. The rate of spectacle dependency for distance vision was significantly lower in the toric group. There were no significant differences between the two groups in all items of the postoperative quality-of-vision questionnaire (25-item Visual Function Questionnaire). Conclusion: The toric IOLs used in this study reduced spectacle dependency more than the non-toric IOLs and did not compromise the subjective visual function, but the uncorrected 50 cm vision was worse in toric IOL implanted eyes.

  • The Association between Ocular Dominance and Refractive Errors in Chinese Myopic Subjects

    摘要: Background: To determine the association between ocular dominance and myopic anisometropia in Chinese myopic subjects. This relevance would facilitate to identify the ocular dominance in bilateral cataract patients. Design: Retrospectively case study. Participants: 1503 Chinese myopic subjects, mean age 27 years, who were candidates for corneal myopic refractive surgery were reviewed between 2011 and 2012. Methods: The ocular dominance was determined by the hole-in-the-card test. The associations between ocular the sphere, cylinder, spherical and astigmatic laterality and refractive characters, including dominance anisometropia, were analyzed. Main outcome measures: Ocular dominance, manifest refraction, cycloplegic refraction. Results: 992 (66%) subjects were right-eye dominant while 511 (34%) subjects were left-eye dominant. The dominant eyes had lower spherical equivalents (SE) and cylinders than the non-dominant eyes (-5.36 D vs. -5.48 D and -0.70 D vs. -0.76 D, respectively, P<0.001). The ocular dominance was significantly associated with the lower myopic eye in subjects with SE anisometropia >0.5 D (P<0.05). There was also significant association between the dominant eyes and the lower astigmatic eyes in the subjects with astigmatic anisometropia > 0.25 D (P<0.05). In unilateral astigmatic subjects, non-astigmatic eyes were noted to be the dominant eyes in 111 (57.51%) subjects and astigmatic eyes represented dominance in 82 (42.49%) subjects. The difference was not statistically significant (P=0.249). The ocular dominance was not associated with the sex. Conclusion: In Chinese myopic subjects, the dominant eye usually had lower myopic SE and lower astigmatism compared with the non-dominant eye, especially in subjects with high amount of anisometropia.
