  • Patient Factor Disparities in Imaging Follow-Up Rates After Incidental Abdominal Findings

    摘要: OBJECTIVE. The effect of demographics and societal determinants on imaging follow-up rates is not clear. The purpose of this study was to compare characteristics of patients with imaging findings representing possible cancer who undergo follow-up imaging versus those who do not to better understand factors that contribute to follow-up completion. MATERIALS AND METHODS. The records of 1588 patients with indeterminate abdominal imaging findings consecutively registered between July 1, 2013, and March 20, 2014, were reviewed. Several patient characteristics, including distance between patients’ home zip codes and the flagship hospital of the health system were compared between the groups who did and did not undergo follow-up imaging. Subgroup analyses based on the location of the index examination were also performed. RESULTS. Among the 1513 (36.62%) included patients, 554 did not undergo follow-up abdominal imaging within 1 year of the index examination. The same was true of 270 of 938 (28.78%) outpatients and 168 of 279 (60.21%) emergency department patients. Eighty-nine of 959 (9.28%) patients who underwent follow-up imaging were younger than 40 years, compared with 76 of 554 (13.72%) patients who did not undergo follow-up imaging (p = 0.005). Fifty-four of 959 (5.63%) patients who underwent follow-up imaging were older than 80 years, compared with 70 of 554 (12.64%) patients who did not undergo follow-up imaging (p < 0.001). More white patients (587 of 959 versus 301 of 554, p = 0.007) and fewer black patients (204 of 554 versus 270 of 959, p < 0.001) were found in the follow-up imaging group. Greater distance from the flagship hospital correlated with less follow-up in the outpatient subgroup only (p = 0.03). CONCLUSION. Emergency department patients and patients at the extremes of age are less likely to complete follow-up imaging. Insurance status and race and ethnicity may affect follow-up completion rates. The relationship between distance to hospital and follow-up completion requires further investigation.

  • Reflectance confocal microscopy-guided carbon dioxide laser ablation of low-risk basal cell carcinomas: A prospective study

    摘要: Background: Basal cell carcinoma (BCC) treatment modalities can be strati?ed by tumor subtype and recurrence risk. The main limitation of nonsurgical treatment modalities is the lack of histopathologic con?rmation. Re?ectance confocal microscopy (RCM) is a noninvasive imaging device that provides quasihistologic images. Objective: To evaluate the feasibility and ef?cacy of RCM-guided carbon dioxide (CO2) laser ablation of low-risk BCCs. Methods: Prospective study with biopsy specimeneproven low-risk BCCs imaged with RCM. RCM was performed on these sites before and after ablation. If residual tumor was found, a new series of laser passes were performed. The patients were then monitored for recurrence clinically and with RCM. Results: Twenty-two tumor sites in 9 patients (5 men, 4 women) were imaged and treated. Median age was 59 6 12.9 years (range, 30-74 years). Mean tumor size was 7.7 mm (range, 5-10 mm). Residual tumor was identi?ed in 5 of 22 cases (22.7%) under RCM on immediate ?rst-pass postablation sites, prompting additional laser passes. Median follow-up was 28.5 months (range, 22-32 months) with no recurrences found. Conclusions: Addition of RCM to laser ablation work?ow can detect subclinical persistent tumor after initial ablation and may serve as an aid to increase the ef?cacy of laser ablation.

    摘要: In order to solve the problems of low robustness and poor immunity of traditional Z-axis position servo control system for laser processing, a design scheme of Z-axis position servo control system for laser processing based on fuzzy PID control is proposed. The hardware configuration of the system improved and servo controller is added to improve the system performance. Combined with fuzzy PID control principle, the system software operation algorithm and flow are optimized to realize the design of Z-axis position follow-up control system for laser processing. The experimental prototype was built by high-precision laser sensor, and the follow-up control of the laser processing system was realized, so that the laser spot kept a constant distance from the surface to be processed. This laid a foundation for the next intelligent control technology of laser processing.

  • Laser Vision Correction for Regular Myopia and Supracor Presbyopia: A Comparison Study

    摘要: A study to compare femto-presbyLASIK to standard myopia femto-LASIK refractive surgical correction with a total of 45 candidates was performed. The goal was to identify a more speci?c set of indications for presbyopia LASIK treatments. The results showed thoroughly good uncorrected visual acuity for myopia (decimal: 1.01 ± 0.15) as well as for presbyLASIK (decimal: 0.78 ± 0.17) corrections. Astigmatism was comparable in both groups and did not change signi?cantly from preoperative (0.98D ± 0.53 SD) to postoperative (1.01D ± 0.50 SD). Our study results suggest, that presbyLASIK treatment is as safe and e?ective as regular LASIK myopia correction and can hence be recommended to treat presbyopia.

  • Confocal Microscopy and Lentigo Maligna: An in vivo Pilot Study for the Assessment of Response to Imiquimod Therapy

    摘要: Background: Reflectance confocal microscopy (RCM) is a noninvasive technique that provides real-time in vivo images of the epidermal layer. Imiquimod has been recommended as an alternative treatment in lentigo maligna (LM) when surgical excision is not the treatment of choice. In the present study we compare the results of in vivo RCM to the histopathological examination before and after treatment of LM with topical imiquimod. Methods: Thirty-four patients with confirmed LM were included. Imiquimod 5% was applied until a weeping erosion appeared in the LM-affected skin. Evaluation was performed by clinical examination, dermatoscopy, histopathology and RCM. Results: During the follow-up, 27 of 34 patients (79.42%) demonstrated a total tumor clearance by imiquimod treatment. In the treated area, a significant decrease of atypical cells was detected using RCM (p < 0.0001). Furthermore, a significant positive correlation in the detected atypical cells was shown using confocal microscopy and histology (p = 0.0001, r = 0.7335, respectively). Conclusion: In patients not suitable for surgical intervention imiquimod treatment is an appropriate treatment alternative. Thereby, in vivo RCM was demonstrated to be an excellent examining device, which not only allows diagnosis of LM, but also therapy and follow-up examinations. An important benefit of RCM, in contrast to conventional histopathology, is the simple handling with in vivo examination of epidermal skin without any pain for the patient.

    摘要: Treatment of intracranial ruptured and unruptured aneurysms can be performed with surgical (vascular clips) or endovascular procedures (coils, stents, flow diverters). There are several devices and techniques for endovascular treatment. Knowledge of this techniques allows physicians to choose and interpret accurately imaging modalities for non-invasive follow-up. Through a pictorial essay of imaging follow-up of treated intracranial aneurysms we show the differences in characterization with computed tomography angiography, magnetic resonance angiography without contrast (time of flight or “TOF” technique), contrast enhanced MR angiography (C+) and digital subtraction angiography (DSA). Additionally we recommend imaging modalities for each treatment device.
