  • Does the combination of photobiomodulation therapy (PBMT) and static magnetic fields (sMF) potentiate the effects of aerobic endurance training and decrease the loss of performance during detraining? A randomised, triple-blinded,??placebo-controlled trial

    摘要: Background: Photobiomodulation (PBMT) is a therapy that uses non-ionising forms of light, including low-level lasers and light-emitting diodes (LEDs) that may be capable of modulating cellular activity. Some biological processes may also interact with static magnetic fields (sMF), leading to modulatory effects on cells. Previous studies have verified that the combination of PBMT and sMF (PBMT/sMF) enhances the performance of individuals during aerobic training programs. The detraining period can cause losses in aerobic capacity. However, there is no evidence of the existence of any recourse that can decrease the effects of detraining. We aimed to investigate the effects of PBMT/sMF application during training and detraining to assess the effectiveness of this treatment in reducing the effects of detraining.

  • A Comparative Assessment of Pain Caused by the Placement of Banded Orthodontic Appliances with and without Low-Level Laser Therapy: A Randomized Controlled Prospective Study

    摘要: Patients still refuse or discontinue orthodontic treatment due to related pain and discomfort. In this study, we investigate if low-level laser therapy (LLLT) can reduce pain caused by orthodontic bands. Sixty subjects who needed bands placed on the upper permanent first molars were assigned randomly to the LLLT group, placebo, and control groups. Inclusion criteria were: age range 10–14 years, fully erupted upper first molars in healthy condition, presence of tight mesial proximal contact. Exclusion criteria were: systemic or metabolic diseases, chronic pain or neurological or psychiatric disorders, use of pharmacological agents interfering with pain perception, previous orthodontic treatment or the simultaneous presence of other devices in the patient’s mouth. The assessment of pain was performed by using a numeric rating scale (NRS) considering different time intervals, i.e., immediately after bands placement, 6 h, 24 h, and from day 2 to day 5. Differences in the maximum pain and in pain experienced at each time-point, among the three groups, was assessed by using the Kruskal–Wallis H. The final sample included 56 patients, 29 males, and 27 females, with a mean age of 12.03 ± 1.3 years. Patients were randomly allocated into three groups (tested, control, and placebo group) with each group consisting respectively of 19, 20, and 17 individuals. Subjects in the LLLT experienced less pain at each time interval as well as the maximum pain score being lower in the LLLT compared to control and placebo groups. These findings were all statistically significant (p < 0.05). LLLT can alleviate the intensity of pain after the placement of orthodontic bands.

  • Is Low-Level Laser Therapy Effective for Pain Control After the Surgical Removal of Unerupted Third Molars? A Randomized Trial

    摘要: One of the main challenges after extraction of unerupted third molars is pain control, and one of the treatments for pain control is low-level laser therapy (LLLT). Thus, this study aimed to assess the effectiveness of LLLT for pain control after extraction of lower third molars. Materials and Methods: This randomized, double-blind, split-mouth study included patients who required bilateral extraction of unerupted lower third molars. Patients received LLLT on 1 side (laser group) but not on the opposite side (control group). On the experimental side, each patient received a laser application at 5 intraoral points for 30 seconds per point. This procedure was simulated on the control side. The pain control response was assessed with a modified pain visual analog scale at the following times: immediately after surgery (T0), after laser application (T1), 24 hours after surgery (T2), 48 hours after surgery (T3), and 72 hours after surgery (T4). Results: Thirty-two patients, 56.25% of whom were women, underwent extraction; the average age was 22.2 years. At the times analyzed, the laser group presented better results than the control group. As for the times, there were differences in pain scores between T0 (8.03 ± 14.87) and T3 (2.66 ± 4.23), as well as T4 (3.36 ± 7.83), in the laser group and differences between T0 (19.76 ± 26.66) and T3 (7.11 ± 10.76), as well as T4 (6.26 ± 13.14), in the control group. Conclusion: LLLT was effective in reducing pain after surgical removal of unerupted third molars. At T3 and T4, pain reduction in relation to T0 was noted in both groups.

  • Evaluation of the proper level of specific absorption rate of human blood for 532a??nm laser in blood low-level laser therapy

    摘要: Low-level laser has been used for a variety of clinical applications as a practical non-medicinal treatment, due to its ability to modulate blood rheology and improve biostimulation. This article research aim was to evaluate the proper level of specific absorption rate (SAR) of human blood during low-level laser treatment. Measurements were conducted on human blood in vitro, at different laser powers (wavelength 532 nm at powers of 50, 60, 70 and 80 mW) and exposure duration. Dielectric parameters were observed as a function of frequency from 40 Hz–30 MHz by utilizing an Agilent 4294A impedance analyzer at average room temperature of 25 °C. The SAR increased steadily with increasing frequency until it attained saturation peak, and thereafter exhibited a decrease trend. The SAR values range from 0.173–1.417 W kg?1 for blood irradiated using laser power of 50 mW and range from 0.178–0.754 W kg?1 for 60 mW. These values of SAR within 5–10 min of radiation present better stimulation results. Using laser powers of 70 and 80 mW for irradiations, the SAR values within the range from 0.003–0.791 W kg?1 and 0.130–0.491 W kg?1, respectively, were computed. The SAR values here portend high risk associated to blood than its stimulation mechanism because the blood already attained a plateau and became saturated. This causes imbalance within the blood molecules resulting in a decrease in SAR as the frequency increases. This is due to a phase lag that develops between the electric field and induced dipole alignment creating a significant drop in the SAR of blood. The rate of inhibition increases rather than stimulation since the thermal radiation becomes exceedingly high resulting in crenation and hemolyzation of the blood. Therefore, we recommend using a laser at an output of power 50 mW for 5–10 min to reach the maximum capacity of SAR for more absorption and optimal stimulation.

  • Comparison between fractional Co2 laser-triamcinolone injection combination therapy and triamcinolone injection monotherapy for keloid

    摘要: The histopathologic view of keloid shows dense fibroblasts and bundles of collagen throughout dermis. Treatment that completely cure keloid still not exist, although there are many treatment options. The monotherapy fractional CO2 laser shows good results, but is still as an adjuvant therapy. Fractional CO2 laser affects fibroblast and its functions in producing collagen. The combination therapy will combine the selective phototermolysis effect of CO2 lasers with antimitotic and antiinflammatory effects of corticosteroids. This study was an open trial with parallel design that compared fractional CO2 laser-intralesional triamcinolone acetonide combination therapy (treatment group) and intralesional triamcinolone acetonide (control group) in keloid patients with collagen density as evaluation parameter. The dose of triamcinolone was 10 mg/ml of 0.05-0.1 ml/cm2 of keloid. Fractional CO2 laser energy setting was 10-20 mJ. The decrease of collagen density in control and treatment group was significant (p=0.008 and p=0.001), although the decrease difference between control and treatment group was not significant (p=0.328). The collagen density that decrease in a shorter time shows that fractional CO2 laser could be a good combination therapy.

  • Pulsed Nd:YAG laser: effects on pain, postural stability, and weight-bearing pattern in children with hemophilic ankle arthropathy

    摘要: Hemophilic ankle arthropathy features joint pain, impaired postural control, and abnormal weight-bearing pattern. This study was designed to investigate the effects of pulsed Nd:YAG laser on pain, postural stability, and weight-bearing pattern in children with hemophilic ankle arthropathy. Forty children diagnosed with hemophilia type A (age, 8–16 years) were randomly allocated to either the treatment group (n = 20) who received a three-phase active therapy with pulsed Nd:YAG laser thrice/week (total energy was 1500 J) plus a physical exercise program for four consecutive weeks, or the placebo group (n = 20) who received placebo laser plus the physical exercise program. Pain, postural control (i.e., directional control [DC], endpoint excursion [EE], center-of-gravity movement velocity [CoG-MV], and maximum excursion [MXE]), and weight-bearing pattern (i.e., pressure peaks of the hindfoot [PP-HF] and forefoot [PP-FF], and the foot contact area [FCA]) were assessed pre- and post-treatment. The treatment group showed greater improvement in pain (P = .004), DC (P = .02), EE (P = .003), and CoG-MV (P = .003) compared to the placebo group. However, there was no statistically significant difference between both groups regarding the MXE (P = .15). In addition, the treatment group achieved favorable improvements in PP-HF (P = .003), PP-FF (P = .027), and FCA (P = .002) relative to the placebo group. Pulsed Nd:YAG laser is a potentially effective therapy for pain relief, postural control enhancement, and weight-bearing pattern adjustment in children with hemophilic ankle arthropathy.

  • Pulse Dye Laser Therapy Successful for Elastosis Perforans Serpiginosa

    摘要: Background. We describe a case of elastosis perforans serpiginosa and its successful management with PDL laser. Case Presentation. A 15-year-old male presented with a history of itchy, raised, red and unsightly lesions on the back of his neck. He was diagnosed with Elastosis Perforans Serpiginosa on tissue biopsy and underwent pulse dye laser therapy over four years with excellent results. Conclusions. Our results show that pulse dye laser therapy is a safe and effective treatment for elastosis perforans serpiginosa.

  • Low-level laser therapy prevents muscle apoptosis induced by a high-intensity resistance exercise in a dose-dependent manner

    摘要: Apoptosis is a highly regulated form of cell death that is characterized by definite morphological, biochemical, and molecular events. Data indicate that acute exercise affects apoptotic signals in skeletal muscle and that the apoptotic response is strongly connected to strenuous exercise. This idea is supported by clinical and experimental studies showing that apoptosis occurs after the variety of exercise types as evidenced in running, swimming, and resistance exercise (RE). Although apoptosis induced by exercise may be a normal process used to remove partially damaged cells, it is possible that excessive exercise (e.g., high effort load) causes extensive cell death. Therefore, therapeutic interventions could be tested to attenuate this process in skeletal muscle. In this regard, photobiomodulation (PBM) may be suggested as a useful tool to counteract undesirable effects of exercise. This is based, for example, on studies showing that the application of low-level laser therapy (LLLT) before RE can delay fatigue and attenuate muscle necrosis, inflammation, and oxidative stress. Further evidence in favor of the PBM comes from an important study by Sussai et al., in which the application of LLLT immediately after swimming to exhaustion attenuated the number of apoptotic muscle cells. To our knowledge, only the study by Sussai et al. has examined the repercussion of LLLT on aerobic exercise–induced apoptosis and there is no information on whether a similar response could be obtained upon RE. Moreover, it is unclear how different doses of LLLT affect exercise-induced apoptosis. With this perspective in mind, we investigated whether LLLT, performed with three differences pre-exercise doses, would affect RE-induced muscle apoptosis in rats.

  • Low-level laser therapy (LLLT) in sites grafted with osteoconductive bone substitutes improves osseointegration

    摘要: The aim of this study was to evaluate the osseointegration of implants placed in areas grafted with different osteoconductive bone substitutes irradiated with infrared low-level laser therapy (LLLT). Fifty-six rats were randomly allocated into 4 groups: DBB, bone defects filled with deproteinized bovine bone graft (DBB); HA/TCP, bone defects filled with biphasic ceramic made of hydroxyapatite and β-tricalcium phosphate (HA/TCP); DBB-L, bone defects filled with DBB and treated by LLLT; HA/TCP-L, bone defects filled with HA/TCP and treated by LLLT. Bone defects were performed in the tibia of each animal and filled with the different biomaterials. The grafted areas were treated with LLLT (λ 808 nm, 100 mW, ? ~ 0.60 mm) in 7 sessions with 48 h between the irradiations. After the 60-day period, the implants were placed, and the animals were euthanized after 15 and 45 days. The osseointegration and bone repair in the grafted area were evaluated by biomechanical, microtomographic and histometric analyses, and the expression of some bone biomarkers was evaluated by immunohistochemistry analysis. LLLT induced higher degree of osseointegration, which was associated with the greater expression of BMP2 and OCN. LLLT performed in areas grafted with osteoconductive bone substitutes prior to implant placement improves osseointegration.

  • Subthreshold laser therapy for macular oedema from branch retinal vein occlusion: focused review

    摘要: Retinal vein occlusion is the second- leading cause of vision loss by retinal vascular disease. Subthreshold micropulse laser therapy (SLT) is safer than conventional laser photocoagulation (CLP), yet existing reviews of its use for branch retinal vein occlusion (BRVO) are limited in scope. A literature search of PubMed, Google Scholar, Embase, Cochrane Library and ClinicalTrials.gov databases was conducted in August 2019 without restriction on language or publication date. Outcomes included changes in macular oedema (ME) and visual acuity (VA), and rates of complications or retreatments. Fourteen studies involving 315–405 eyes diagnosed with BRVO were evaluated. Treatment with SLT is associated with significant and durable reduction of ME and VA as early as 1 month. SLT performs comparably with conventional photocoagulation and intravitreal injections (IVIs) of ranibizumab. Subthreshold laser therapy is safer and as effective as CLP for the treatment of ME associated with BRVO. SLT may be used in combination with anti- VEGF IVIs to enhance improvement in VA and ME resolution.
