Impact of light conditions on reading ability following multifocal pseudophakic corrections
摘要: Purpose: To examine the impact of light intensity and temperature on reading performance following bilateral pseudophakic multifocal presbyopic correction. Patients and methods: This is a prospective clinic-based trial conducted at the Department of Ophthalmology in the University Hospital of Alexandroupolis, Greece. Three groups of patients were formed (G1: patients with bilateral bifocal implantation, G2: patients with bilateral trifocal implantation, and control group: patients with bilateral pseudophakic monofocal implantation). Reading ability was quantified with the Greek version of MNREAD chart with minimal reading speed at 80 words/min for the following light intensities (25, 50, and 75 Foot-Candles [FC]) and temperatures (3,000, 4,000, and 6,000 K). Preferred light conditions for reading were assessed, as well. ClinicalTrials.gov Identifier: NCT03226561. Results: Control group demonstrated significantly lower reading ability at all light combinations with maximal ability at 75 FC and 6,000 K (0.58±0.18 logMAR). Bifocal group presented a light-dependent reading ability that ranged from 0.45±0.08 logMAR (25 FC and 3,000 K) to 0.40±0.11 logMAR (75 FC and 4,000 or 6,000 K). Trifocal participants presented the best reading ability that was light intensity-independent; however, their performance was reduced at 6,000 K. G1 and G2 preferred primarily intermediate light temperature, while control participants preferred cold light temperature. Conclusion: Multifocal pseudophakic corrections improve reading ability; however, they present variable efficacy according to the light conditions.

摘要: Purpose: To investigate visual outcomes, spectacle independence, and patient satisfaction after trifocal intraocular lens implantation. Methods: A retrospective study conducted on 5,186 patients who underwent phacoemulsification and were implanted with a diffractive trifocal intraocular lens (FineVision Micro F, PhysIOL SA, Liège, Belgium). Results: The mean pre-operative keratometry was 43.61 ± 1.55 D, and mean keratometric cylinder was -0.86 ± 0.66 D. At 3 months post-operatively, values changed to 43.59 ± 1.56 and -0.71 ± 0.46 D, respectively. Mean sphere varied from 2.04 ± 2.60 to 0.14 ± 0.38 D, and mean spherical equivalent from 1.64 ± 2.61 to -0.05 ± 0.36 D. More than 60% of the patients achieved better corrected distance visual acuity. The majority had a final residual refractive error (predictability) lower than 1 D (99.1%), with 14.3% needing optical adjustment (bioptics). Posterior capsulotomy was performed in 2.2%, and the lens was replaced in 5 patients. A high percentage of patients were satisfied in terms of vision and spectacle independence at all distances. Although 2% referred to worsening of night vision quality, only 2% were not very satisfied and 20 patients were dissatisfied. The causes were: blurred vision (7 cases), spectacle dependence (10), dysphotopsia (7), and dry eye (4). Conclusions: The implantation of this model provided good visual performance in all distances, as well as high levels of spectacle independence and satisfaction.

The Latest in IOLs: Light Adjustable and Trifocal IOLs—Experience from Aboard
摘要: Purpose of Review To investigate the latest findings in the field of intraocular lenses and their future development. Recent Findings The idea of intraocular lenses (IOLs) has changed the clinical practice of cataract surgery over the last 50 years. Technology and scientific studies have created IOLs of different designs, with ultraviolet filters for UV protection, different types of haptics to ensure stability, multifocal designs for distant, and near vision. IOLs’ development is fast and it is time to explore the new designs and their usefulness. Summary The newest data from the IOL field are on light adjustable and trifocal IOLs, which are presented herein and are expected to change our perspective and lead the way in even more complicated and useful IOLs.

Long-term results of a diffractive trifocal intraocular lens: Visual, aberrometric and patient satisfaction results
摘要: Purpose: To evaluate the long-term objective and subjective results of a trifocal intraocular lens. Methods: Prospective observational study enrolling 24 eyes that underwent uneventful microincisional cataract surgery with bilateral implantation of a diffractive trifocal intraocular lens (FineVision Micro F, PhysIOL, Belgium). After a mean follow-up period of 60 ± 2 months, patients answered the 10-item Near Activity Vision Questionnaire (NAVQ-10) and were assessed for uncorrected and corrected distance, intermediate and near visual acuity, defocus curve, light-distortion analysis, and total ocular aberrometry with a pyramidal wavefront sensor (Osiris, CSO, Italy). Results: There was significant improvement of uncorrected visual acuity at all distances in all eyes (p < 0.01). The distorsion index and best-fit circle radius were significantly lower in binocular conditions (p = 0.02). Mean Rasch score at NAVQ-10 was 16.29 ± 11.57. Patients reported to be completely, very, and moderately satisfied with their uncorrected near vision in 67%, 25%, and 8% of cases, respectively. We found no significant correlation between patient satisfaction scores at near vision satisfaction questionnaire and other variables such as light-distortion analysis or wavefront aberrometry parameters. Conclusion: The studied diffractive trifocal intraocular lens provides good and stable long-term results for distance, intermediate, and near vision, with high levels of patient satisfaction at long-term follow-up. The intraocular lens’ light-distortion effect is less disturbing in binocular conditions and may have its perception down-modulated by neuroadaptation over time.

Comparison of the Visual Outcomes after Cataract Surgery with Implantation of a Bifocal and Trifocal Diffractive Intraocular Lens
摘要: Purpose: To evaluate and compare visual outcomes and optical quality after implantation of a bifocal (Acrysof ReSTOR? SN6AD1) or trifocal (AT LISA? tri 839MP) diffractive intraocular lens (IOL). Methods: Fifty-one eyes of 43 patients undergoing cataract surgery were enrolled and assigned to one of two groups: the trifocal group, comprising 24 eyes implanted with the trifocal diffractive IOL (AT LISA? tri 839MP), and the bifocal group, comprising 27 eyes implanted with the bifocal diffractive IOL (Acrysof ReSTOR? SN6AD1). Visual acuity (distant, intermediate, and near vision) and refractive postoperative outcomes were evaluated at one and three months postoperatively. Measurements of optical quality (using OQAS II?), contrast sensitivity (using CGT-2000?), automated visual field examination, and evaluation of defocus curve were performed three months postoperatively. Results: No statistically significant differences between the two groups were found in three-month postoperative distant and near (40 cm) visual acuities and optical quality. However, intermediate (63 cm, 80 cm, and 100 cm) visual acuities were significantly better in the trifocal group. Distant contrast sensitivity (5 m) under mesopic conditions was significantly better with the bifocal lens, whereas near contrast sensitivity (30 cm) under mesopic and scotopic conditions was significantly better with trifocal lens. There was no statistical difference between the groups under photopic conditions. In the defocus curve, the visual acuity was significantly better at intermediate distance in the trifocal group. Conclusions: Trifocal diffractive IOLs provide significantly better intermediate vision than bifocal IOLs, with equivalent postoperative levels of distant and near vision and ocular optical quality. Further, they provide better near contrast sensitivity under scotopic condition compared to diffractive bifocal IOLs.
