  • Fractional laser ablation for the targeted cutaneous delivery of an anti-CD29 monoclonal antibody – OS2966

    摘要: Monoclonal antibodies targeting cytokines are administered parenterally for the systemic treatment of severe psoriasis. However, systemic exposure to the biologic increases the risk of side-effects including immunosuppression, whereas only a small fraction of the active molecules actually reaches the target organ, the skin. This preclinical study examines the feasibility of delivering a humanized anti-CD29 monoclonal antibody (OS2966) topically to skin using minimally-invasive fractional laser ablation. This approach would enable the targeted use of a biologic for the treatment of recalcitrant psoriatic plaques in patients with less widespread disease while minimizing the risk of systemic exposure. First, the effect of a wide range of laser poration conditions on skin permeation and deposition of OS2966 was tested in vitro to determine optimal microporation parameters. Subsequently, confocal laser scanning microscopy was employed to visualize the distribution of fluorescently-labelled OS2966 in skin. The results demonstrated that delivery of OS2966 into and across skin was feasible. Above fluences of 35.1 J/cm2, skin deposition and permeation were statistically superior to passive delivery reaching values up to 3.7 ± 1.2 μg/cm2 at the most aggressive condition. Selective targeting of the skin was also possible since ≥70% of the OS2966 was delivered locally to the skin. Although nanogramme quantities were able to permeate across skin, these amounts were orders of magnitude lower than levels seen following subcutaneous or intravenous injection and would result in minimal systemic exposure in vivo. The diffusion of fluorescently-labelled OS2966 into the skin surrounding the pores was clearly higher than in intact skin and demonstrated the feasibility of delivering the antibody at least as deep as the dermo-epithelial junction, a critical border region where inflammatory cells cross to promote disease progression. These preliminary results confirm that fractional laser ablation can be used for the cutaneous delivery of OS2966 and now preclinical/clinical studies are required to demonstrate therapeutic efficacy.

  • Synergistic Effects of Photo-Irradiation and Curcumin-Chitosan/Alginate Nanoparticles on Tumor Necrosis Factor-Alpha-Induced Psoriasis-Like Proliferation of Keratinocytes

    摘要: Psoriasis is a chronic inflammatory skin disease characterized by hyperproliferation of the epidermal cells and is clinically presented as thick, bright red to pink plaques with a silvery scale. Photodynamic therapy (PDT) using visible light has become of increasing interest in the treatment of inflammatory skin diseases. In this study, we demonstrate that a combination of curcumin-loaded chitosan/alginate nanoparticles (Cur-CS/Alg NPs) and blue light emitting diodes (LED) light irradiation effectively suppressed the hyperproliferation of tumor necrosis factor-alpha (TNF-α)-induced cultured human kerlatinocyte (HaCaT) cells. The Cur-CS/Alg NPs were fabricated by emulsification of curcumin in aqueous sodium alginate solution and ionotropic gelation with calcium chloride and chitosan using an optimized formulation derived from a Box-Behnken design. The fabricated Cur-CS/Alg NPs were characterized for their particle size, zeta potential, encapsulation efficiency, and loading capacity. The surrogate 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyl tetrazolium bromide (MTT) assay, to measure the relative number of viable cells, showed that the CS/Alg NPs were nontoxic to normal HaCaT cells, while 0.05 μg/mL and 0.1 μg/mL of free curcumin and Cur-CS/Alg NPs inhibited the hyperproliferation of HaCaT cells induced by TNF-α. However, the Cur-CS/Alg NPs demonstrated a stronger effect than the free curcumin, especially when combined with blue light irradiation (10 J/cm2) from an LED-based illumination device. Therefore, the Cur-CS/Alg NPs with blue LED light could be potentially developed into an effective PDT system for the treatment of psoriasis.

  • Treatment of psoriatic skin lesions with a new Er:Yag laser technology: A case series study

    摘要: Erbium: YAG (Er:YAG) laser with RecoSMA technology. This laser emits thousands of microbeams of energy causing superficial epidermal ablation and a separation of dermal fibers due to a mechanical-acoustic and resonance effect. The aim of this study is to evaluate the clinical efficacy of Erbium: YAG (Er:YAG) laser with RecoSMA in the treatment of stable psoriasis vulgaris lesions. A questionnaire completed by 112 patients enrolled in the study. Photographs taken before each session, the analysis of the dynamics index PASI and histological studies were analyzed to visually monitor the clinical progress. The analyses were carried out with the help of computer software. The results after treatment of average number of sessions per course showed the complete clinical remission of the skin lesions in over 87% of the patients treated. The Er:YAG laser and RecoSMA have demonstrated the high effectiveness of the laser method of treating skin manifestations of the disease and improving the quality of life of patients.

  • 310 nm UV-LED Attenuates Imiquimod-Induced Psoriasis-like Skin Lesions in C57BL/6 Mice and Inhibits IL-22-Induced STAT3 Expression in HaCaT Cells

    摘要: Ultraviolet light-emitting diodes (UV-LEDs) are a novel light source for phototherapy. This study aimed to evaluate the therapeutic effects of UV-LEDs on psoriasis. Importantly, 310 nm UV-LEDs have not been studied in psoriasis in vitro and in vivo. Effects by 310 nm UV-LED and 311 nm narrowband ultraviolet B (NBUVB) irradiation were compared for suppressing IL-22-induced activation of STAT3 expression using cell viability assay, western blotting, and immunocytochemistry. C57BL/6 mice, were topically treated with imiquimod (IMQ) for 6 consecutive days and observed degenerative changes. Test groups were irradiated by 310 nm UV-LED and 311 nm NBUVB. Phenotypic observations, histopathological examinations, and ELISA were conducted with skin and blood samples. STAT3-dependent IL-22 signalling and effects in keratinocytes are negatively regulated by 310 nm UV-LED, which significantly ameliorated IMQ-induced psoriasis-like dermatitis development and reduced Th17 cytokine levels (IL-17A, IL-22) in serum and dorsal skin. Histopathological findings showed decreases in epidermal thickness and inflammatory T-cell infiltration in the UV-LED-irradiated groups. Quantitative PCR confirmed UV radiation energy-dependent decrease in IL-17A and IL-22 mRNA levels. The results demonstrated that UV-LEDs had anti-inflammatory and immunoregulatory effects. So, UV-LED phototherapy inhibits psoriasis development by suppressing STAT3 protein and inflammatory cytokines and could be useful in treating psoriasis.

  • Efficacy and safety of 308-nm excimer lamp in the treatment of scalp psoriasis: a retrospective study

    摘要: Background: Scalp psoriasis is a major therapeutic challenge due to the hindrance caused by hair. Treatment with the 308-nm excimer lamp is purported to provide many benefits over conventional phototherapy. This retrospective study evaluates the efficacy, safety, and effective dosage of 308-nm excimer light in the treatment of scalp psoriasis. Methods: We retrospectively reviewed the medical records of patients with scalp psoriasis who received treatment with 308-nm excimer light. Clinical and epidemiological data as well as details regarding treatment were statistically analyzed to determine the treatment outcomes. Results: Twenty patients with scalp psoriasis were included in the study. Their mean age was 47.45 ± 17.93 years. Eleven patients responded to treatment at the end of 10 sessions. The median baseline Psoriatic Scalp Severity Index (PSSI) was 12 (range, 3-32). At the end of the protocol, the median PSSI was 4.5 (range, 0-24), indicating a statistically significant reduction (P<0.001). Common adverse effects included erythema, irritation, and desquamation. Conclusion: The 308-nm excimer light appears to be an effective and safe modality that requires short treatment time. The modality could be considered as an alternative or adjuvant treatment for scalp psoriasis.

  • Serum TGF-?2 and TNF-?± During Psoriasis Therapy with Narrowband Ultraviolet B

    摘要: Although it is now an accepted concept that narrow-band UVB therapy is an efficacious therapy for psoriasis, the relationship between the response rate and the potential effects on serum cytokines is less well-established. The purpose of this study was to investigate the correlation between the response rate and the changes of serum TGF-β and TNF-α necessary for understanding the underlying mechanisms of narrow-band UVB phototherapy. NB-UVB is effective against psoriasis without any obvious side effects and can significantly decrease serum TNF-α and promote TGF-β level of psoriasis patients. Additionally, measurement of TGF-β and TNF-α in serum could be considered as biomarkers of psoriasis activity during NB-UVB therapy.

  • Design of an Algorithm for Automated, Computer-Guided PASI Measurements by Digital Image Analysis

    摘要: The Psoriasis Area and Severity Index (PASI) is the most accepted method for psoriasis severity scoring. However, a prominent level of subjectivity and a low intra- and inter-rater reproducibility was reported. Therefore, an accurate and reproducible measure of psoriasis severity is needed, especially in the setting of registration studies for systemic anti-psoriatic drugs. Herein we describe a robust, user-friendly, computer-guided technology that allows for automated PASI measurements after total body imaging and digital image analysis. For this purpose, a novel image processing software for PASI calculations was developed, which was combined with a commercially available, automated image capturing system. Our data shows, that the software was able to accurately calculate the proportion of psoriatic skin surface as well as the severity of erythema, induration, and desquamation by anatomic region. In a pilot clinical validation the time-efficient technology showed a high reproducibility and high levels of agreement to results attained by PASI-trained physicians. Therefore, automated computer-guided PASI measurements hold the promise of significantly reducing the physicians’ workload while ensuring a high level of reproducibility and standardization.

  • The Antipruritic Effect of Phototherapy

    摘要: Phototherapy is widely used to treat inflammatory skin diseases such as psoriasis and atopic dermatitis. Repeated suberythemogenic doses of UV-light reduce inflammation in these diseases and ultimately may lead to a complete disappearance of cutaneous symptoms for weeks or months. Chronic pruritus is an important and highly distressing symptom of many of these inflammatory skin diseases. Interestingly, pruritus is also reduced or completely abolished by UV-treatment of psoriasis and atopic dermatitis, and sometimes reduction of pruritus is the first indication for skin improvement by phototherapy. The cutaneous nervous system is an integral part of skin anatomy, and free nerve endings of sensory cutaneous nerve fibers reach up into the epidermis getting in close contact with epidermal cells and mediators from epidermal cells released into the intercellular space. Stimulation of “pruriceptors” within this group of sensory nerve fibers generates a neuronal signal eventually transmitted via the dorsal root and the spinal cord to the brain, where it is recognized as “itch”. UV-light may directly affect cutaneous sensory nerve fibers or, via the release of mediators from cells within the skin, indirectly modulate their function as well as the transmission of itch to the central nervous system inducing the clinically recognized antipruritic effect of phototherapy.
