研究目的
To challenge the currently accepted treatment parameters of waiting 1 year following burn injury to treat the resultant scarring by assessing the impact of treating burn and trauma wounds at less than 3 months of injury on scar formation.
研究成果
Early intervention with fractionated CO2 laser may be a revolutionary new technique to improve visual and functional outcomes in moderate to severe acute burn injury. Data suggest that the standard 1‐year delay in treatment time should come into question due to efficacy, short treatment course, and lack of adverse events of early treatment.
研究不足
Inability to use uniform depth settings across patients and small sample size. The total number of patients who completed the trial was 19.
1:Experimental Design and Method Selection:
Randomized clinical trial investigating the treatment of burn wounds within first 3 months of injury using fractional ablative CO2 laser treatments at 8‐week intervals.
2:Sample Selection and Data Sources:
19 patients with moderate‐to‐severe acute burn injuries less than 3 months from injury. Wounds were a mix of partial and full‐thickness injury.
3:List of Experimental Equipment and Materials:
Fractional ablative CO2 laser (Lutronic Inc., Boston, MA), Nikon D300 camera, optical coherence tomography (OCT) for scar depth and epidermal thickness measurement.
4:Experimental Procedures and Operational Workflow:
Patients were numbed topically and orally medicated for pain management. Each patient was treated with a fractional ablative CO2 laser in dynamic mode. The laser spot size was 120 μm, depth ranged from 500 to 800 μm, and density was 10% per pass with varying pass number as determined by dynamic scanning technology.
5:Data Analysis Methods:
MSS ratings pre‐ and post‐treatment were evaluated using a paired‐samples t test. OCT analysis demonstrated changes in vascularity and epidermal character. Histological evaluation revealed differences in the integrity and organization of the skin structure between treated and untreated areas.
