研究目的
To investigate the use of indocyanine green angiography with a fluorescence imaging system for detecting parathyroid glands and predicting postoperative hypoparathyroidism in patients undergoing thyroidectomy.
研究成果
The study concludes that indocyanine green angiography using a fluorescence imaging system is a safe, easy, and effective method for intraoperative detection and protection of parathyroid glands, as well as for predicting postoperative hypoparathyroidism. It allows for quick evaluation of parathyroid vascularization and function, aiding in decisions for autotransplantation to prevent complications.
研究不足
The sample size is small (26 patients), which may limit the generalizability of the results. Further large randomized controlled trials are needed to provide more definitive evidence.
1:Experimental Design and Method Selection:
The study employed an intraoperative fluorescence imaging system combined with indocyanine green dye to evaluate parathyroid vascularization and function during total thyroidectomy. Visual and fluorescence-based scoring systems (grades 0-2) were used to assess parathyroid glands.
2:Sample Selection and Data Sources:
26 patients who underwent total thyroidectomy at Zhuhai People's Hospital from May 1 to August 8, 2018, were recruited based on surgical indications including malignant biopsy results, clinically worrying lesions, compressive symptoms, recurrent cancer, recurrent cyst, thyroglossal duct cyst, thyrotoxicosis, and cosmetic issues.
3:List of Experimental Equipment and Materials:
Fluorescence imaging system INTRAOPERATIVE NAVIGATION SYSTEM (INS) (Digi-MIHI-I-001, Digital Precision Medicine Technology Co., Ltd), indocyanine green dye, harmonic scalpel (Johnson & Johnson Inc.), general anesthesia equipment, and laboratory tools for serum calcium and PTH measurements.
4:Experimental Procedures and Operational Workflow:
Patients received general anesthesia, a neck incision was made, thyroidectomy was performed using a harmonic scalpel, ICG dye was injected intravenously, and fluorescence imaging was conducted to score parathyroid glands. Postoperative follow-up included serum calcium and PTH measurements on POD 1, 7, 14, and at 6 months.
5:Data Analysis Methods:
Data were presented as mean values with standard deviations and compared using chi-square tests. Statistical significance was set at p<0.05, and analysis was performed using SPSS software version 22.0.
