To investigate patterns of subsequent progression of localized retinal nerve fiber layer (RNFL) defects and to quantify the extent of progression in normal-tension glaucoma (NTG) patients.

There were nine patterns of subsequent progression of localized RNFL defects, with continuous widening towards the macula being the most common. Initial temporal progression, especially in inferior fundus defects, may indicate a higher risk for further RNFL loss. Red-free fundus photography is effective for detecting subtle progression not easily seen with other methods, but further studies with OCT are warranted for comparison.

The study is limited by not applying optical coherence tomography (OCT), which might provide more objective measurements. Difficulty in defining borders of diffuse atrophy using red-free photography could affect categorization. The sample size is small (33 eyes), and the study is retrospective, which may introduce selection bias.

Two examiners independently reviewed serial photographs to categorize progression patterns (e.g., widening, deepening, new defects) and measured angles using the software. Discrepancies were resolved by a third examiner. Serial disc stereophotographs and visual field tests were also evaluated for progression.

Statistical analysis was performed using MedCalc software. Intra- and inter-observer agreements were assessed with k-values. Angular widening was quantified in degrees, and patterns were summarized with frequencies and percentages.
