To retrospectively evaluate the features of undiagnosed breast cancers on prior screening breast magnetic resonance (MR) images in patients who were subsequently diagnosed with breast cancer, as well as the potential utility of MR-computer-aided evaluation (CAE).

MR-CAE has the potential for reducing actionable, overlooked findings because more plateau or washout delayed enhancement patterns were found for actionable findings than for underthreshold findings, which can improve lesion conspicuity in cases showing greater background parenchymal enhancement or multiple distracting lesions.

Small sample size, use of only one MR-CAE program, inclusion of only malignant cases missed at prior MRI and benign biopsied cases, potential mismatched cases due to long intervals, and possible increased false positive rates if all washout components are interpreted as suspicious.
