  • Ranibizumab versus laser therapy for the treatment of very low birthweight infants with retinopathy of prematurity (RAINBOW): an open-label randomised controlled trial

    摘要: Despite increasing worldwide use of anti-vascular endothelial growth factor agents for treatment of retinopathy of prematurity (ROP), there are few data on their ocular efficacy, the appropriate drug and dose, the need for retreatment, and the possibility of long-term systemic effects. We evaluated the efficacy and safety of intravitreal ranibizumab compared with laser therapy in treatment of ROP.

  • Effect of Low-level Laser Therapy on Orthodontic Tooth Movement

    摘要: Prolonged orthodontic treatment duration is detrimental in terms of increased incidence of caries, root resorption, and reduced patient compliance. The aim of this randomized, clinical trial was to evaluate the effect of low-level laser therapy (LLLT) on the rate of orthodontic tooth movement (OTM) and pain control. Materials and Methods: This single-blind study included twenty participants requiring extraction of all first premolars. Randomly selected split-mouth design was used. One side was irradiated with 810 nm diode laser (dose of 5.0 J/cm2) at 10 points for 10 s. Irradiation was performed just after loading canine retraction forces and on days 3rd and 7th. Every 21st day, the force level of coil spring was adjusted, and LLLT protocol was repeated till retraction was complete. Measurements were recorded on study models to evaluate the rate of retraction. Results: Significant increase in OTM was observed on the side exposed to LLLT when compared to control side (P < 0.05). Statistically significant difference in pain perception was observed during first 2 days only between lased site and control site (P < 0.05). Conclusion: LLLT is a reliable tool for enhancing OTM and is effective in relieving pain at parameter settings and protocol used in this study.

  • A comparative evaluation of the effectiveness of low-level laser therapy, ultrasound therapy, and transcutaneous electric nerve stimulation in the treatment of patients with TMDs: a prospective study

    摘要: Introduction A temporomandibular disorder (TMD) is considered to be multifactorial with several treatment modalities available to provide symptomatic relief. The use of non-invasive biological techniques with minimal or no side effects is highly recommended. The present study evaluated the effectiveness of low-level laser therapy (LLLT), therapeutic ultrasound (Th.US), and transcutaneous electric nerve stimulation (TENS) for the management of TMDs. Methodology A total of 45 patients (16 males and 29 females) with an age range between 20 and 50 years having temporomandibular joint (TMJ) disorder with TMJ pain, joint sounds, limited mouth opening, and pain in muscles of mastication were included in the study. The patients were randomly divided into 3 equal groups where group I was given class IV A low-level laser therapy (indium gallium arsenide phosphide [InGaAsP]) at a wavelength of 940 nm biweekly for 4 weeks. Group II was given therapeutic ultrasound in continuous mode at a frequency of 1 MHz and intensity of 1–1.25 W/cm2 for 3 min/session (3 sessions of 3 min each in every visit). The patients in group III were given TENS therapy with low-intensity current for a duration of 30 min, with a pulse rate of 0.11 Hz biweekly. All the patients were followed up twice a week until 1 month corresponding to total eight visits in 4 weeks to check for reduction in pain intensity, number of tender points, joint sounds, and maximal possible mouth opening. Results There was a statistically significant greater reduction in pain (p < 0.005), greater increase in mouth opening (p < 0.005), and more reduction in tender points (p < 0.005) in LLLT group. The significant comparative results could be observed in the first week of therapy itself. The results revealed that LLLT provided most significant symptomatic relief followed by therapeutic ultrasound and least improvement was observed in TENS group. Conclusion Although all the three physical therapies improved the signs and symptoms of TMDs, LLLT was found to be most effective in relieving pain, improving mouth opening, and reducing the number of tender points from the baseline until the end of the treatment compared with therapeutic ultrasound and TENS. Hence, it can be concluded that LLLT is a safe and clinically effective therapy for managing patients with TMJ pain and reduced mouth opening.

  • State of the art of Thulium laser enucleation and vapoenucleation of the prostate: a systematic review

    摘要: Tm:YAG laser operates at a wavelength of 1940-2013 nm in a continuous wave mode, which enables smooth incision and rapid vaporization of prostatic tissue, with a shallow penetration of only 0.2 mm. These characteristics make Tm:YAG a versatile laser, allowing the surgeon to perform resection, vaporization, and vapoenucleation. This systematic review aims to summarize the current evidence of safety and efficacy, long-term durability, impact on sexual function following Tm:YAG laser enucleation and vapoenucleation of the prostate and to compare the outcomes of these procedures versus other surgical treatments of benign prostatic hyperplasia.

  • Low dose fentanyl infusion versus 24% oral sucrose for pain management during laser treatment for retinopathy of prematurity—an open label randomized clinical trial

    摘要: To compare the efficacy of low dose fentanyl infusion and 24% oral sucrose in providing optimal pain relief during laser for retinopathy of prematurity (ROP), we enrolled fifty-eight spontaneously breathing preterm infants undergoing laser. The preterm infants were randomized to either fentanyl infusion (1 mcg/kg/hr) or 24% oral sucrose (2 ml). We evaluated and compared the proportion of time spent crying during the procedure, salivary cortisol before and after the procedure, premature infant pain profile- revised (PIPP-R) scores during the procedure, apnoea during and after the procedure, need for mechanical ventilation, and feed intolerance and urinary retention 24 h after the procedure between the two groups. We found that the proportion of time spent crying during the procedure was significantly less in the fentanyl group [62.5% (50.7–74.2) vs 73.8% (55.6–83.4); P = 0.02]. Average PIPP-R score during the procedure was significantly less in the fentanyl group [7.2 vs 9.0; (mean difference of ? 1.8; P = 0.01)]. There was no difference in other outcomes between the two groups.

  • Persistent Erythema Ab Igne Occurring After Low-Level Laser Therapy for Lipolysis

    摘要: Erythema ab igne (EAI) is a chronic skin condition characterized by blanchable, reticular, erythematous, hyperpigmented macules and patches and can develop from prolonged exposure to infrared radiation in close proximity. It is often asymptomatic; however, some patients report mild skin sensitivity and pruritus. Low-level laser therapy (LLLT) uses red light for non-invasive body contouring.1 Current literature suggests that LLLT provides statistically significant fat reduction in the absence of serious adverse events.2 Although EAI after prolonged electronic device use has been reported, it is not considered an adverse effect of LLLT. Here, we report a case of persistent EAI post-LLLT for lipolysis and body contouring.

  • Laser Therapy for Actinic Purpura

    摘要: Laser therapy has been previously reported as successful in the treatment of postprocedural ecchymoses. However, laser therapy has not been formally evaluated in the context of actinic purpura. Actinic purpura, also known as solar purpura, is a relatively common manifestation of photoaging that falls under the broader classification of dermatoporosis, or skin fragility secondary to photoaging. It is estimated that 12% of adults older than 50 years experience actinic purpura, with the proportion increasing to 30% for adults older than 75 years. Although a benign condition, actinic purpura can be an unsightly nuisance for patients, which may even affect their quality of life. With a normal resorption time of approximately 10 to 14 days, this condition may have a continued significant impact on patients given that it can occur with minor or unnoticed trauma. Previous reported attempts at treating and preventing actinic purpura have included topical retinoids, topical human epidermal growth factor, and combination topical formulations.

  • Use of Lasers for Iron-Induced Accidental Tattoos

    摘要: BACKGROUND Awareness of accidental tattoos after iron injections with paravenous leakage is low. No larger patient cohorts have been analyzed. The lesions are a burden to the patients. The only known treatment is laser therapy. OBJECTIVE To characterize the clinical appearance and development of iron-induced tattoos and to demonstrate safe and efficient removal with quality-switched (QS) and picosecond lasers. MATERIALS AND METHODS The authors conducted a retrospective systematic analysis on patients who presented at the Dermatology Department of the University Hospital of Zurich between Year 2008 and 2017 with accidental hyperpigmentations after iron injections. From 29 collected patients, 13 received laser treatment and were analyzed with reference to the lasers used, including wavelength, fluence, spot size, intervals, number of sessions, and overall success. The authors defined the latter as a complete removal or as patients’ satisfaction. RESULTS The authors treated 13 patients and completed the treatment in 8 patients, with an average of 5.6 sessions. No complications occurred. CONCLUSION Spontaneous regression of iron-induced tattoos is possible within 1 to 2 years, but not guaranteed. Removal with QS ruby, Nd:YAG, and picosecond lasers can be achieved within a mean number of 5.6 sessions. Assuring a valid indication before intravenous iron administration is important.

  • Feasibility of transgingival laser irradiation for antimicrobial photodynamic therapy

    摘要: Aim: Diode lasers are commonly used for antimicrobial photodynamic therapy (aPDT). This study aimed to assess the feasibility of transgingival laser irradiation during aPDT and evaluate whether the photosensitizer can be activated. Materials and Methods: Four diode laser settings were assessed for transgingival irradiation: 120 mW, 80 mW, 60 mW, and 40 mW. Fifteen soft-tissue pieces from a pig’s lower jaw were prepared. The specimens’ thickness was measured and transgingival laser irradiation was performed. A digital power meter measured laser power on the other side of the tissue. The power outcome after staining of the nonbuccal aspect of the tissue with photosensitizer dye was assessed similarly. Results: Transgingival laser irradiation (average soft-tissue thickness: 0.84 ± 0.06 mm) resulted in di?erent power transmission depending on the power settings and photosensitizer. The lowest values were observed with the 40 mW setting and photosensitizer (median 3.3 mW, max. 5.0 mW, min. 2.3 mW, interquartile range 1.2), and the highest at 120 mW without photosensitizer (median 41.3 mW, max. 42.7 mW, min. 38.0 mW; interquartile range 1.5). Conclusions: This study indicates that transgingival irradiation may be suitable for aPDT, since power transmission through the gingival tissue was observed in all specimens. However, the decrease in laser power caused by both the soft tissue and the photosensitizer has to be taken into account.

  • Successful Treatment of Adult-Onset Recurrent Respiratory Papillomatosis with CO <sub/>2</sub> Laser and Photodynamic Therapy

    摘要: Recurrent respiratory papillomatosis is a noninvasive benign epithelial tumor caused by human papillomavirus. Clinically, it featured rapid growth, multifocus, and frequent recurrence. Though a number of therapies have been investigated, the recurrence after treatment is always a challenge. In this report, we describe a 27-year-old male patient with recurrent respiratory papillomatosis who was treated with CO2 laser therapy followed by 5-aminolevulinic acid photodynamic therapy (ALA-PDT). There was no adverse reaction after treatment and no recurrence during the follow-up time.
