  • Impact of light conditions on reading ability following multifocal pseudophakic corrections

    摘要: Purpose: To examine the impact of light intensity and temperature on reading performance following bilateral pseudophakic multifocal presbyopic correction. Patients and methods: This is a prospective clinic-based trial conducted at the Department of Ophthalmology in the University Hospital of Alexandroupolis, Greece. Three groups of patients were formed (G1: patients with bilateral bifocal implantation, G2: patients with bilateral trifocal implantation, and control group: patients with bilateral pseudophakic monofocal implantation). Reading ability was quantified with the Greek version of MNREAD chart with minimal reading speed at 80 words/min for the following light intensities (25, 50, and 75 Foot-Candles [FC]) and temperatures (3,000, 4,000, and 6,000 K). Preferred light conditions for reading were assessed, as well. ClinicalTrials.gov Identifier: NCT03226561. Results: Control group demonstrated significantly lower reading ability at all light combinations with maximal ability at 75 FC and 6,000 K (0.58±0.18 logMAR). Bifocal group presented a light-dependent reading ability that ranged from 0.45±0.08 logMAR (25 FC and 3,000 K) to 0.40±0.11 logMAR (75 FC and 4,000 or 6,000 K). Trifocal participants presented the best reading ability that was light intensity-independent; however, their performance was reduced at 6,000 K. G1 and G2 preferred primarily intermediate light temperature, while control participants preferred cold light temperature. Conclusion: Multifocal pseudophakic corrections improve reading ability; however, they present variable efficacy according to the light conditions.

  • Artificial pupil versus contralateral balanced contact lens fit for presbyopia correction

    摘要: Purpose: To assess and compare the effects of contact lens-based artificial pupil design and contralateral balanced multifocal contact lens combination (CBMCLC) on visual performance. Methods: This randomized crossover study conducted at the University of Valencia, Spain included 38 presbyopic patients using an artificial pupil contact lens in the nondominant eye and a CBMCLC. After a month of lens wear, the binocular distance visual acuity (BDVA), binocular near visual acuity (BNVA), defocus curve, binocular distance, and near contrast sensitivity and near stereoacuity (NSA) were measured under photopic conditions (85 cd/m2). Moreover, BDVA and binocular distance contrast sensitivity were examined under mesopic conditions (5 cd/m2). Results: Visual acuity at an intermediate distance and near vision was better with the CBMCLC than with the artificial pupil (p<0.05). Statistically significant differences were found in contrast sensitivity between the two types of correction for distance (under mesopic conditions) and for near vision, with the CBMCLC exhibiting better results in both cases (p<0.05). The mean NSA values obtained for the artificial pupil contact lens were significantly worse than those for the CBMCLC (p=0.001). Conclusion: The CBMCLC provided greater visual performance in terms of intermediate and near vision compared with the artificial pupil contact lens.

  • Short-term visual result after simultaneous photorefractive keratectomy and small-aperture cornea inlay implantation

    摘要: Purpose: To report the short-term results of simultaneous photorefractive keratectomy (PRK) and small-aperture cornea inlay implantation (KAMRA) surgery in treating presbyopia. Methods: Simultaneous PRK and KAMRA inlay surgery was performed on 21 patients from July 2015 to March 2016. Follow-up exams were conducted at 1, 3, and 6 months postoperatively. Our patients were also divided preoperatively into three categories: myopic, hyperopic, and emmetropic. Over the 6-month period, the main outcome measures were uncorrected near visual acuity (UNVA), changes in best-corrected distance visual acuity, and mean spherical equivalent refraction. Results: At 6-month follow-up, 83% (10/12) of patients had a monocular UNVA of 20/40 or better, and 75% had a binocular UNVA of 20/40 (J5) or better. At 6 months, the overall mean refractive spherical equivalent (MRSE) was -0.60 D (±0.42 standard deviation [SD], range: -1.38 to -0.13) with a mean change of -0.43 D (±1.19 SD, range: -1.5 to 2.63) compared to preoperative data. Overall, 91% (10/11) of patients were within a ±0.5 D range of our target -0.75 D for KAMRA use. One-hundred percent (5/5) of the hyperopes, 50% (1/2) of emmetropes, and 100% (4/4) of myopes met the targeted range. At 6 months, the MRSE for the hyperopic subgroup (n=5) was 0.33 D (±0.20 SD), the MRSE for the emmetropic subgroup (n=2) was -1.19 D (±0.19 SD), and the MRSE for the myopic subgroup (n=4) was -0.66 D (±0.36 SD). Conclusion: Based on preliminary results and a small sample size, it seems that simultaneous PRK and KAMRA is effective and predictable. There are multiple advantages in performing simultaneous PRK and KAMRA including a shorter recovery time and less steroid use than the two surgeries performed separately.

    摘要: By 2020, it is estimated that 2.1 billion people will be presbyopic, and the demand for spectacle independence in this group is growing. This review article provides an overview of the three commercially available corneal inlays for the correction of presbyopia. Safety, efficacy, visual outcomes, and complications are analyzed for all three inlays according to published peer-reviewed data.

  • Laser Vision Correction for Regular Myopia and Supracor Presbyopia: A Comparison Study

    摘要: A study to compare femto-presbyLASIK to standard myopia femto-LASIK refractive surgical correction with a total of 45 candidates was performed. The goal was to identify a more speci?c set of indications for presbyopia LASIK treatments. The results showed thoroughly good uncorrected visual acuity for myopia (decimal: 1.01 ± 0.15) as well as for presbyLASIK (decimal: 0.78 ± 0.17) corrections. Astigmatism was comparable in both groups and did not change signi?cantly from preoperative (0.98D ± 0.53 SD) to postoperative (1.01D ± 0.50 SD). Our study results suggest, that presbyLASIK treatment is as safe and e?ective as regular LASIK myopia correction and can hence be recommended to treat presbyopia.

  • Corneal Refractive Procedures for the Treatment of Presbyopia

    摘要: Purpose: Refractive surgery has been in use for a long time and is evolving at a fast pace with several new corneal procedures being used for the correction of presbyopia. The purpose of this article is to give a comprehensive review of the literature to evaluate the outcome and success of different corneal refractive surgical procedures in presbyopic patients. Methods: We performed a comprehensive search on PubMed to identify published reports of the various procedures utilized in the past and present to correct presbyopia. The outcomes of these procedures were recorded. Results and conclusion: We found that varying rates of success have been reported with these procedures. The results of our exhaustive search are presented in this report for review.

  • Clinical Evaluation of Functional Vision of +1.5 Diopters near Addition, Aspheric, Rotational Asymmetric Multifocal Intraocular Lens

    摘要: To evaluate postoperative outcomes and visual performance in intermediate distance after implantation of a +1.5 diopters (D) addition, aspheric, rotational asymmetric multifocal intraocular lens (MIOL). Patients underwent bilateral cataract surgery with implantation of an aspheric, asymmetric MIOL with +1.5 D near addition. A complete ophthalmological examination was performed preoperatively and 3 months postoperatively. The main outcome measures were monocular and binocular uncorrected distance visual acuity (UDVA), corrected distance visual acuity (CDVA), uncorrected intermediate visual acuity (UIVA), distance corrected intermediate visual acuity (DCIVA), uncorrected near visual acuity (UNVA) and distance corrected keratometry, and manifest refraction. The Salzburg Reading Desk was used to analyze unilateral and bilateral functional vision with uncorrected and corrected reading acuity, reading distance, reading speed, and the smallest log-scaled print size that could be read effectively at near and intermediate distances. The study comprised 60 eyes of 30 patients (mean age, 68.30 ± 9.26 years; range, 34 to 80 years). There was significant improvement in UDVA and CDVA. Mean UIVA was 0.01 ± 0.09 logarithm of the minimum angle of resolution (logMAR) and mean DCIVA was -0.02 ± 0.11 logMAR. In Salzburg Reading Desk analysis for UIVA, the mean subjective intermediate distance was 67.58 ± 8.59 cm with mean UIVA of -0.02 ± 0.09 logMAR and mean word count of 96.38 ± 28.32 words/min. The new aspheric, asymmetric, +1.5 D near addition MIOL offers good results for distance visual function in combination with good performance for intermediate distances and functional results for near distance.

  • Patient-reported outcomes of multifocal and accommodating intraocular lenses: analysis of 117 patients 2–10 years after surgery

    摘要: The purpose of this study was to determine the satisfaction levels of patients at least 2 years after cataract surgery implantation with bilateral accommodating or bilateral multifocal intraocular lenses (IOLs) and to determine the relative rate of spectacle independence and adverse symptoms in that same time frame. Design: Patient questionnaire administered in a single-center private practice at least 2 years after cataract surgery with presbyopia-correcting IOL implantation. Methods: Patients who had undergone uncomplicated cataract surgery with an accommodating or multifocal IOL implant were eligible for inclusion. Patients with visually significant non-IOL-related postoperative morbidity were excluded. Patients with astigmatism or residual refractive error were not excluded. The main outcome measure was patient satisfaction at least 2 years after IOL implantation. Results: Sixty-eight patients who received accommodating lenses and 49 patients who received multifocal lenses completed the questionnaire. The mean age of all patients was 75.7 years at the time of survey; the mean number of years since cataract surgery was 5.4 years. Overall, there were no significant differences between the two groups or within each group between the different lenses used. About 90% of patients in each group were “very satisfied” or “somewhat satisfied” with their vision at least 2 years after the initial surgery. Conversely, only one in eleven patients found his vision to be worse than expected. Conclusions: The majority of patients who received either accommodating or multifocal IOLs remain satisfied with their lens of choice more than 5 years after the original surgery. Glare and halos remain more noticeable in patients who received multifocal lenses.
