To study the efficacy of laser peripheral iridoplasty in the treatment of eyes with primary angle closure and plateau iris syndrome, unresponsive to laser peripheral iridotomy, using anterior-segment OCT as a tool.

Laser iridoplasty significantly lowers IOP in eyes with primary angle closure and plateau iris syndrome, especially in cases with appositional closure and synechiae <180 degrees. However, its effect is minimal in eyes with extensive synechiae (>180 degrees). Post LPI gonioscopy is recommended in all primary angle closure suspect/primary angle closure patients to identify cases where iridoplasty may be beneficial.

The study did not clearly define the control arm in the methodology. The effect of iridoplasty on synechiae of long duration presenting with raised IOP was minimal, and the critical period for its effectiveness is not well defined.
