研究目的
To compare the safety and efficacy of micropulse cyclophotocoagulation (MP-CPC) and transscleral continuous wave cyclophotocoagulation (CW-CPC) for the treatment of refractory glaucoma in the pediatric age group.
研究成果
Both MP-CPC and CW-CPC are effective in lowering IOP in pediatric refractory glaucoma, with similar efficacy. However, MP-CPC has a lower rate of complications, pain, and inflammation, making it a safer option, especially for retreatments.
研究不足
A significantly higher number of eyes had previously undergone CW-CPC in the MP-CPC group (88% vs. 50%), which could influence results. Only a single treatment session was studied; multiple treatments or alternation were not evaluated. Grading of anterior uveitis was not possible due to examination constraints. Short follow-up period (6 months), relatively small sample size, and use of Perkins tonometer for IOP measurement were limitations.
1:Experimental Design and Method Selection:
A prospective interventional study was conducted on children below 12 years of age with uncontrolled glaucoma. The study compared MP-CPC and CW-CPC modes using the IRIDEX Cyclo G6 laser and IRIDEX G-probe, respectively.
2:Sample Selection and Data Sources:
Included 45 eyes of 36 children requiring cyclophotocoagulation from September 2016 to August 2017 at Abureish Children’s Hospital, Cairo University. Selection depended on the availability of the G6 probe; if not available, CW-CPC was used.
3:List of Experimental Equipment and Materials:
IRIDEX Cyclo G6 laser (for MP-CPC), IRIDEX G-probe (for CW-CPC), Perkins tonometer for IOP measurement, caliper for corneal diameter, A-scan ultrasonography for axial length, chloral hydrate for sedation, general anesthesia with sevoflurane, topical prednisolone 1%, cycloplegic eye drops, systemic NSAIDs.
4:Experimental Procedures and Operational Workflow:
Laser procedures performed under general anesthesia. For MP-CPC: 2000 mW power,
5:3% duty cycle (5 ms on, 1 ms off), continuous sliding motion for 100-120 seconds in superior and inferior quadrants. For CW-CPC:
1500 mW power, 1500 ms duration, transillumination to locate ciliary body, approximately 15 shots per quadrant. Postoperative care included medications and follow-ups at 1 week, 2 weeks, 1 month, 3 months, and 6 months.
6:Data Analysis Methods:
Statistical analysis with SPSS for Windows version 15.0.1. Used independent t-test, Fischer exact test, Kaplan-Meier survival analysis, Mantel-Cox logrank test. P-value <0.05 considered significant.
